What the care home does well People are content to share a home together and they told us that staff are very helpful and kind to them. Some people told us on the survey forms that staff "look after us well". Some said the staff were good at "cooking" and other people told us about staff helping them to have good holidays. An interactive television game (the WII) was provided and people have told us that they enjoy using this when staff set it up for them. There is a consistent staff group with only one change during the last year. People benefit from staff knowing their needs and helping them to manage their own health. What the care home could do better: Some action was taken during this inspection visit, but the providers must now ensure all door entries are made safe with appropriate permanent door strips to hold the carpet in place. They must also remove or replace and permanently cover the exposed wiring in the entrance hall. This is to avoid any risks to the health and safety of people living in the home. They must ensure the premises are kept in a good state of repair externally and internally, and this includes bathroom facilities. This is so that they are fully maintained to provide a long term home for the people living there. There must be a system in place to ensure all furniture and fittings are repaired or replaced as needed so that all people have good quality items in their individual rooms. They still need to ensure all staff receive training in Infection Control so that sufficient action is taken to prevent the potential spread of infection. The registered providers must prepare a written report on the conduct of the home at least once a month. This is so that they can demonstrate they are monitoring and improving the quality of the service provided. Care Homes for Adults (18-65 years) Page 2 of 8 What we found: When we carried out the last key inspection of Field House on 1st September 2009 we required action to be taken over the next three months in order to meet the Care Homes Regulations. We carried out this Random Inspection in order to find out what action had been taken. Before the visit we looked at information we had received since the last inspection. This included completed survey forms from nine people that live at the home and from five staff. We also received a completed Annual Quality Assurance Assessment form in March 2010, which gave us some basic information about how the home is run. During the visit we were able to see and speak with three people who currently live there. All others were either on holiday or at day services. We listened to their views, looked at the premises and examined some medication and health records. We also spoke with staff, who gave us information about their training. We found that some action had been taken to meet some of the requirements we had made, but there were still some areas that needed further action. Peoples personal support and health needs are met and improvements have been made in the way staff manage medication. Staff had started to use Health Action Plans with people and these will give individual people more control and understanding about their own health. People told us about going for walks and having exercise. Most staff had received further training from a Boots pharmacist trainer and the Medicine Administration Records showed that staff were recording their actions accurately. We looked at some creams people had prescribed to them and none of these were out of date as they had been when we last visited. As at the two last inspection visits we again found that the home is kept clean for people, but the premises and furniture are not fully maintained. We read in returned survey forms that decor, repairs and maintenance had been neglected. When we visited we found that nothing had been changed since our last visit. One person had still not been able to move back into his chosen bedroom, due to the ceiling damage there. In the ground floor bathroom there were still tiles missing and damp walls. Some people had good quality furniture in their bedrooms, but others still had old, damaged bedroom furniture, with some door handles missing and the top surrounding a washbasin in one persons room still had no edging. The carpet on a landing had still not been replaced and was very worn. In the doorways of two bedrooms we found the carpet to be causing a trip hazard and the senior support worker took some action during the visit to make a temporary repair with packing tape. We also found some wires exposed in the entrance hall and action was taken to make a temporary cover. We did not look into the cellar on this occasion, but staff told us that no changes had been made there. People benefit from consistent management of the home, but the quality of the service is still not fully monitored to ensure the home is run to promote the best interests and safety of people living there. The manager was not available during this visit as she was accompanying some people on holiday. A senior support worker assisted in her absence. There were no reports available from the provider about his assessment of the quality of Care Homes for Adults (18-65 years) Page 3 of 8 the service, but staff told us that he still visits once a week to bring money. There was no evidence that the risks we found around the premises had been identified for action prior to our visit. Staff had received training in Moving and Handling since the last inspection, but none had received updated training in Infection Control. The manager had appropriately notified us of one incident when someone had attended hospital following an accident.
