What the care home does well Residents say they enjoy living at Garlinge Lodge and are very happy there. The home provides a comfortable environment and the standard of cleanliness is good. There is an open and friendly atmosphere with good communication between residents, staff and visitors. Staff are good at helping new residents to settle in. There is encouragement for residents to partake in activities suited to their preferences and capabilities. Residents enjoy the meals. What the care home could do better: Care plans should be developed to better show that personnel preferences are discussed with residents. The plans should also give staff clearer guidance about how to enable each individual and how to encourage their independence safely. The wording of the relatives` survey form needs to be amended to make it clearer that it is directed to relatives and not to residents. Date of last inspection Brief description of the care home The premises at Garlinge Lodge were converted over 30 years ago as a care home for elderly people. It provides residential care for up to 14 people over the age of 65. Care Homes for Older People Page 2 of 8 2 8 0 5 2 0 0 9 Brief description of the care home It is owned and operated by Sira Care Home Ltd, with Mr Kritanand Ramtale as Managing Director and Mr Rajiv Jugdharree as co-director and home manager. The Homes staffing team comprises the Manager, senior care staff and care staff who work a roster that gives 24-hour cover. The Home also employs other staff for catering and domestic duties. Current fees range from £342 to £525 per week. Residents pay separately for hairdressing, chiropody, personal toiletries and newspapers at cost. Care Homes for Older People Page 3 of 8 What we found: The focus of this unannounced Random Inspection was care planning, the storage of medicines and the homes quality assurance systems. We were in the home from 9.15 a.m. until midday and spoke with some residents, the proprietor and Manager and some staff. Parts of the home and some records were inspected. Three care plans were inspected in detail. All three care plans are up to date and have been reviewed recently. Mr Rajiv Jugdharee, the registered manager, was able to show that, consequent to a safeguarding case conference, an additional part has been added to care plan. This is called Ensuring Dignity and gives information about providing intimate care for each resident. There was some discussion about how the care plans could be developed to better show that personnel preferences are discussed with residents. The plans should also give staff clearer guidance about how to enable each individual and how to encourage their independence safely. The standard of daily record keeping is generally good and cross references to other records such as accidents forms. Some entries are descriptive of the care given and convey how the resident has responded during the day or how they feel emotionally. In all three care plans one night staff member repeatedly writes Checked hourly, appeared to have slept well. The Manager agrees this is not informative. We discussed if it is necessary for every resident to be checked hourly. It was agreed that, where residents do not want to be checked, this can be recorded in the care plan and implemented within a risk assessed framework. Residents medicines are now kept in lockable metal cupboard which is more in line with latest guidelines from the Royal Pharmaceutical Society than the previous arrangements had been. The home has some quality assurance processes including resident and relative surveys. At the Key Inspection conducted on 28th May 2009, the Manager had said it was intended to include health care professionals and Care Managers in future surveys. He was able to show that residents were surveyed in September 2009 and relatives were sent surveys in March 2010. It was agreed the wording of the relatives survey form needs to be amended to make it clearer that it is directed to relatives and not to residents. Mr Jugdharree said he intends to launch the first survey of Care Managers and other health care professionals by the end of June 2010. There is a garden with a patio area to the rear and side of the home that residents can have access to with assistance. At the Key Inspection conducted on 28th May 2009, it was recommended handrails be fitted alongside the paths in the garden so residents could use the garden more independently and safely. Since then, rails have been fitted in the areas where there are immediately available attachment points such as walls. Mr Jugdharree said it is intended to provide further rails to make all parts of the garden safe in the near future. The standard of cleanliness in the parts of the home seen is good and residents Care Homes for Older People Page 4 of 8 commented on how clean their rooms are kept. Residents are very complimentary about the meals at the home and say the cook provides good quality meals During our visit, we observed an open and friendly atmosphere with good communication between residents and staff.
