The owners are Mr and Mrs Lee. Mrs Janet Eastgate is responsible for the day-to-day administration of the home and works closely with the manager of the home who is Mrs Charlotte Molesworth. The home is in landscaped grounds with a summer house, a japanese garden, patio and seating areas
Annual Service Review Page 2 of 7 1 7 1 1 2 0 0 8 in a quiet area of Gosberton village, which is some 6 miles from the market town of Spalding. The local church is next to the home and there are footpaths to the local amenities such as post office, shops and pub. Car parking is available at the front and rear of the home. The home was opened in 1989 with a ground floor extension (the Heesom wing) with en-suite single bedrooms, built in 2000 to provide further bedroom and communal areas. The original building is a converted grade II listed building with the second floor providing space for administration services. The home has 80 of rooms as single bedrooms and lounge and dining areas. A passenger lift is in place to serve the ground and first floors of the home. What did we do for this annual service review? We looked at all the information that we have received, or asked for, since our last key inspection. This included: The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Surveys returned to us by people using the service and from other people with an interest in the service. Information we have about how the service has managed any complaints. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection and the results of any other visits that we have made to the service in the last 12 months. Relevant information from other organisations. What other people have told us about the service. What has this told us about the service? The manager sent us their annual quality assurance assessment (AQAA) when we asked for it. It was clear and and very detailed and gave us all the information we asked for. The manager told us that she continues to ensure that a thorough assessment is carried out when admitting all prospective people to the home by an experienced and appropriately trained senior member of staff, either in their home or elsewhere. The person is supported and encouraged to be involved in the assessment process along with relatives and other professionals. On admission a key worker is assigned to each person. The manager told us that they have improved collaboration with the local doctors. They also support the people to manage their own health care needs including visual, hearing, oral and continence management. They assist the people in planning for routine health care visits and keeping appointments. In addition care plans have been extensively reviewed this year and staff have been trained about ensuring that care is person centred. They have also provided training in order to work towards obtaining a nationally recognised Gold Standards Framework for terminal care. They have also produced key worker handbooks for staff, purchased a new hoist and produced photographs of key staff in the entrance lobby and on each floor of the home. The manager also told us that they have increased their catering rating from good to excellent following an inspection by Environmental Health Officer and have appointed two volunteers to assist the Diversional Therapist. They have also involved the people in the recruitment and running of the home and introduced regular group residents Annual Service Review Page 4 of 7 meetings and a introduced a suggestion box. They have also refurbished the kitchen and installed secondary double glazing to downstairs rooms. They have also provided a Pergola leading to the summerhouse and improved signage to all communal rooms with pictures. Over the last year they have amended their pre admission forms to include a section on the Mental Capacity 2005 to promote a more person centred approach. In addition they now have an arrangement with NHS Lincolnshire to provide Intermediate Care. We looked at the information in the AQAA and our judgement is that the care home is still providing a positive service and that they know what further improvements they need to make. The manager told us that they hope to further improve and update and modernise their presentation pack for prospective enquiries, including the service users guide. They also want to produce an audio version of the service users guide. They would like consider installing digital televisions in bedrooms in readiness for the digital switch over and convert first floor double rooms to single rooms with en-suite toilets. We sent out 7 surveys to the people receiving the service. We received 2 replies both of which were very positive and included, the care home is excellent in every way, very caring and friendly at all times and they provide good activities. There is nothing to show us that the home is not meeting the needs of the people living in the home. Staff continue to be recruited correctly with a check by the Criminal Records Bureau. Staff continue to receive an induction training programme. Their training has included mental capacity and deprivation of liberty safeguards to identify and protect the peoples rights and choices. They have increased the number of staff with nationally recognised National Vocational Qualifications to 60 of staff with NVQ 2 with a further 24 having achieved NVQ 3. In addition, 10 of care staff are currently working towards their NVQ 2 with catering and domestic staff also working towards NVQ 2. The manager also told us their training centre has been in operation for 12 years and does not only train staff at this home but also carries out external training at other homes through its affiliation with Lincolnshire Development. The home continues to be registered with Anglia Ruskin University to provide training and placements for student nurses and has been approached by the University of Nottingham to provide placements for student nurses in the future. We sent out 4 surveys to staff. We received 3 replies which included, we get good training, but we need more staff so that we can put into practice what we have learnt and the home is well run with good communication. We consider all who reside in the home and respect their needs, confidentiality and offer them respect. There is nothing to show that staff do not have the knowledge and support to provide the care for the people living in the home. The home continues to let us know about things that have happened over the last year and they have shown that they have managed issues well. They and us have received no complaints over the last 12 months. We were made aware of a safeguarding adults issue which was investigated by Lincolnshire County Council and found their was no evidence to support the allegation. They continue to work well with us and have shown us that their service continues to provide positive

We are not going to change our inspection plan, and will do another annual service review inspection by November 2010.
