What the care home does well There is a good and committed staff team, which is working well together and with other professionals to provide a good standard of care to people who use the service. The premises are well maintained and well decorated and kept to a good standard of cleanliness. This helped to make the place safe, comfortable and pleasant. The day-to-day management of the home had improved. The new manager was providing good leadership to the staff team. This has helped staff to improve their practice as well as the overall quality of the service. What the care home could do better: Although the care planning system had improved and was much more robust, there is a need for staff to improve the way they carry out care plan reviews. They should be clear about the information they use to carry out such reviews and record them. This should help to understand and justify the outcomes of such reviews. The quality monitoring and quality assurance methods in use at the home had improved. We advised that the reports of the unannounced visits by the responsible individual (the owner) should be used more effectively as a tool for improving the management and overall quality of the service. Care Homes for Older People Page 3 of 8 What we found: This random inspection was carried out on 8 October 2009, staring at 10.00 and finished at 15.00 hours. The aim of this inspection was to assess the compliance of the service with the requirements and recommendations that were made following our key inspection of May 2009 and to assess the overall improvement in service provision, to the benefit of people who used it. We found that the new manager had improved the homes procedure and practice relating to the assessment of needs of people before they were admitted. Staff were able to get sufficient information about identified needs and were therefore better equipped in planning the individuals care and support. Care plans had been improved. They were based on identified needs and comprised of relevant actions to meet such needs. Care plans were reviewed regularly. However, there was a lack of information about how such reviews were carried out. Medicines management had improved. Records relating to the receipt, administration and disposal of medicines were appropriately maintained. People who lived at the home and relatives told us that they were satisfied with the arrangements for the provision of care and support. In particular, they stated that they were satisfied with the seating arrangements in the front lounge and that they did not think the use of this lounge affected the privacy and dignity of people. The manager and her staff confirmed that personal care was always provided in private. Staff said that they had been provided with training and guidance on the promotion of privacy and dignity and this benefited people in their care. We noted that social and recreational activities for people who lived at the home were organised on a monthly basis and information about each weeks activities was appropriately publicised within the home. The activities co-ordinator stated that she spent time consulting with people about their preferred activities and ensured that these were catered for. Social care needs were also included in individual care plans. Staff were observed spending more one-to-one time with people that they were caring for. People who lived at the home and their relatives said that they had taken part in various activities held at the home and social events in the locality. We also noted that meals were served in congenial settings and in a manner that promoted the dignity of people who lived at the home. The physical environment was well maintained. Both the private accommodation and communal areas of the home appeared clean, tidy and hygienic. People who lived at the home said that they found the place safe, pleasant and comfortable. We checked the management of complaints and safeguarding concerns and found that this was appropriately undertaken. The manager confirmed that the relevant policies and procedures had been reviewed and improved. We checked management arrangements of the service. The new manager explained that she had applied for registration with this Commission. She was well supported in her duties by an administrator and by the owner. The owner was completing reports of his Care Homes for Older People Page 4 of 8 monthly unannounced visits to the home. We advised that these reports could be used as a monitoring tool to improve the service. There were other quality assurance methods being used at the home and staff were using them with more confidence. This ensured that the overall quality of the service was appropriately monitored and improved for the benefit of people who used it. We were satisfied that the requirements and recommendations that we made at our last inspection had been complied with.
