Care Homes for Older People Page 3 of 8 What we found: We carried out this visit to look at how medicines were being handled because we found serious shortfalls on our last visit. A specialist pharmacist inspector checked a sample of medicines stocks, medicines records and care plans. We spoke with five care staff, two people who live in the home and gave detailed feedback to the registered manager at the end of our visit. Overall we found good improvements that meant medicines were now being handled in a much safer way. We found medicines records and stock to be well organised. Records of medicines received into the home, given to people and disposed of were accurate so medicines could easily be accounted for. Our checks and the homes own systems showed medicines were now being given and recorded in a much safer way. Medicines stocks were checked at most medicines rounds so that any mistakes were highlighted promptly so action could be taken to help prevent them happening again. Records had been improved because specific warnings about medicines such as taking them before food were now properly highlighted and our checks showed that these instructions were being correctly followed. Giving people their medicines at the right time and dose is important because it helps maintain their health and wellbeing. We saw good improvements in the recording and administration of medicines that were not taken, for example, when people were asleep or refused them. Staff usually returned to people to give them their medicines at a later time that fitted into their daily routine and respected their wishes. We found good improvements in the way when required medicines were handled because detailed care plans had been written that described how to use them properly and the manager said these were now being regularly reviewed. External medicines such as creams had clear plans about how to use them. However, we found the records of them being applied were not always signed by the care staff that applied them so there was a risk they might not be used properly. Some people were looking after their own medicines. One person that was managing their own inhalers did not have a risk assessment or care plan so there was a risk they might not receive the right amount of support to do it safely. When we checked their medicines stock and spoke with them it was difficult to know if they were using the inhalers properly because records were not clearly made when new supplies were given to them. We gave some advice to the manager and care staff about how to improve this because safely supporting people to look after their own medicines is important in helping them retain some of their independence. We checked how medicines liable to misuse, called controlled drugs, were handled. We found they were securely stored in suitable cupboards and when we checked the current stocks we found these were correct and all records were properly witnessed. Having secure storage and witnessed records helps prevent the mishandling and misuse of controlled drugs. Since our last visit the supplying pharmacy had carried out a detailed audit and the managers had strengthened the medicines checking and auditing systems so mistakes Care Homes for Older People Page 4 of 8 were now being promptly found. Care staff had received and were due to attend more medicines handing training in the next month or so. Staff competency assessments had been carried out in the past and medicines round were being regularly observed to make sure staff were following the correct procedures.
