Harbour House cares for 18 older people and is a two story detached property that has been extended and stands in its own grounds close to the harbour. It is set in aAnnual Service Review residential area near to local amenities and a reliable bus service provides access to local towns. There is disability access on the ground floor and three lifts are provided internally. One is a full shaft lift, the second a stair lift providing access to the bedrooms on the first floor and a third upstairs to compensate for the different levels on the first floor. Sixteen of the bedrooms are single and five have en-suite facilities. The one double bedroom exceeds the recommended size and is currently used as a single room. Communal space is good with a choice of two sitting rooms and two dining rooms and toilets and bathrooms are located throughout the care home. The home is committed to delivering care in a manner that meets the needs of each resident and recognises individual choice and preference. Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection or annual service review. This included: the annual quality assurance assessment (AQAA) that was sent to us by this service (the AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service, and it also gave us some numerical information about the service); Surveys returned to us by people using the service and from other people with an interest in the service; Information we have about how the service has managed any complaints; What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement; The previous key inspection and random inspection. What has this told us about the service? This service sent us their annual quality assurance assessment (AQAA) when we asked for it. It was clear and gave us all the information we asked for. We looked at the information in the AQAA and our judgement is that the care home is still providing a good service and that they know what further improvements they need to make. The AQAA told us the following changes have been made as a result of listening to people that use the service: Service users expressed interest in the improvements we have made to the outside areas of the home as well as the new dining room conservatory. We discussed the governments capital grants with some of the residents, and they were involved in how we furnished the new patio garden built around the new conservatory. Despite the very poor weather last year the residents have made good use of this area. We have introduced an activity organiser role because some residents could potentially have had their needs lost in the pressure from others who do not welcome such activities. We felt that they may have been inhibited from expressing an alternative view. The Annual Quality Assurance Assessment also told us they plan to make these further changes: Last year we completed a new conservatory dining room which has received great acclaim. Before its completion two residents said they would not be happy to eat in a conservatory as they imagined it could be too hot. As a result of this we decided to include automatic (electric) roof ventilators, which will open or close at preset temperatures, and blinds to the glass roof to ensure shade. This has worked very well and all residents enjoy using the room. They are proud to take their visitors to view it. This new dining room is located next to the kitchen whereas previously we had two dining rooms at some distance from the kitchen. Residents now say their meals are hotter and staff are available in the dining room all the time. This has left us with two vacated dining rooms which we intend to convert into two new large en-suite bedrooms. We will monitor the temperature in the new dining room throughout 2009 Annual Service Review Page 4 of 6 (when the summer may well be better than in 2008) and the residents experiences, and if the temperature is not comfortable then we will seek ways to modify this; possibly by installing air-conditioning (as we have done in our sister home, Tremethick House), although we are advised that this will not be necessary. The Annual Quality Assurance Assessment also told us: The home is run and managed for the benefit of the residents, with respect for individual choices which are protected and supported by all staff. The environment is well maintained, clean and well planned for the comfort of the service users, with good facilities throughout. Contacts with the local community are maintained through visitors, visiting groups, church services, a visiting library service and regular visits out to places of interest. Residents families are involved in the daily lives of many service users, and often come to social events arranged within the home. We provide in-house training and expertise in care and human resources. Our on-going refurbishment programme has taken been enhanced by the new conservatory which is now successully in use as a dining room. This has improved the mealtime experience and service provision for residents, and has helped to create an environment offering a great deal of choice in how residents choose to live and spend their day. The kitchen has been altered to accommodate the new access into the conservatory dining room, and the laundry machines have been replaced. Residents have said how much they like the improvements. We have a new level access patio area to the side of the home, accessed from the french windows to the new conservatory, which has proved popular with residents when weather permits. The AQAA also informed us that staff recruitment, retention, and training is a priority. Reviews of policies and procedures occur. The AQAA also told us that they have received one complaint since the last inspection, which was investigated and resolved within 28 days. We received 5 responses to surveys sent out to staff and 2 from users of the service. Comments were all positive and included: It is a lovely home to be in as our every need is considered, and The food is excellent as is the care. One comment received from a staff member was I thoroughly enjoy my place of work, we are a close and happy team of workers. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, and will do a key inspection by 4th March 2011. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
