If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service Harmony House is a purpose built care home and is registered to provide residential and nursing care for up to 57 men and women. The home is situated close to junction 3 of the M6, with bus links to Nuneaton town centre and is near by to the George Eliot Hospital. The home is managed by Southern Cross/Ashbourne Ltd. The single room en suite accommodation is situated on two floors. The service provision on the ground floor is Annual Service Review Page 2 of 7 for those people who require assistance with personal care. The first floor service provision is for those who require nursing care and qualified nursing staff are available at all times on this unit. Access to the first floor is via passenger lifts/stairs. Garden and patio areas are accessible to all residents including those with limited mobility who require wheelchairs. Information about the home is in a Statement of Purpose and a Service Users Guide which is made available to all residents. The scale of charges at the time of the last key inspection were for residential care £359.00 to £467.36, nursing care £510 to £568.84 and for continuing care £669.84. These fees are subject to change. Additional charges are made for chiropody, hairdressing and personal items and sundries such as newspapers. The manager confirmed these fees are detailed in the Service User Guide for the home. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at information that we have received or asked for since the last key inspection to the home. This included: The Annual Quality Assurance Assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for the people using the service. It also gives us some numerical information about the service. Information we have about how the service has managed any complaints. What the service has told us about things that have happened in the service. These are called notifications and are legally required. Information collected at the previous key inspection of 1 July 2008. Comment cards completed by nine people who live at the home and five relatives. What has this told us about the service? The Home sent us their Annual Quality Assurance Assessment (AQAA) when we asked for it. It was clear and gave us some information about what they are doing now and what they want to do in the future. We looked at the information in the AQAA and all the other information and our judgement is that the home is still providing a good service. During the last inspection it was identified that care plans needed to be reviewed in regards to end of life care. We are told that new plans are in the process of being finalised with the Primary Care Trust and are to be implemented over the next 12 months. We are also told that there is a comprehensive care planning process that covers all aspects of daily living and associated risks to the people who live in the home. An issue recommended for improvement during the last inspection was the regular review of care plans. The AQAA identified that this remains an area which the home feel they need to do better. We are aware though our notifications that some residents had developed pressure sores (red or broken areas to the skin) or had been admitted to the home with them and that these had deteriorated requiring the home to seek specialist support. It was established there had been delays in treatment being given. The AQAA does not state any changes in practice or make it clear how this aspect of care is being managed to ensure pressure areas are effectively managed. Some comments from people living at Harmony House on what they do well include: Annual Service Review Page 4 of 7 respect privacy, well looked after by staff support me clean carpets Comment cards show that four out of nine people living at the home feel that the staff always provide the care and support they need, four feel they usually do and one person says they sometimes do. Some comments from relatives include: I find the ongoing care excellent, X always looks comfortable and clean. The standard of care is usually very high. The care plan is followed and we are usually informed of alterations and changes well in advance. We are told in the AQAA about how equality and diversity is met, this states: Service users have a person centred care plan, and are valued as an individual and their opinions and religious beliefs are respected and encouraged to practice these. We promote independence of service users as much as possible. We have anti discriminatory policies and procedures, which are updated regularly and are on display within the home and also available on hard copy for all staff. At Harmony House we are introducing a personal preference plan for all new service users which we are encouraging relatives to assist in filling out if the service user is unable to do so. This covers all aspects of daily activity both before and after admission. The information is transferred into the care file to ensure that the service users wishes, wants and desires are carried out as they would like. We are told that the service have received one complaint since the last inspection. This was related to the management of personal care for a person living in the home. The manager carried out a full investigation and we are told actions were taken to help prevent some of the issues identified from reoccurring. Comment cards from people at the home show that four people feel that staff always listen and act on what they say, one person feels they usually do and four people feel they sometimes do. One person states you should only ever tell a member of staff about any problems (as opposed to reporting this to management). However comment cards from relatives show that all feel any concerns raised are dealt with appropriately. Since the last inspection we are told the following improvements have been made: The development of a more comprehensive Activities Schedule. Upon room vacation, new carpets and full decoration has been carried out. More staff have been recruited so that the home have their own bank of staff. What are we going to do as a result of this annual service review? We have looked at the information available to us including the AQAA and completed surveys and we are satisfied that the home is still making improvements to ensure the Annual Service Review Page 5 of 7 home continues to provide a good service. We will continue to monitor incoming notifications in regards to the development of any pressure sores or incidents that could impact on the welfare of people living at the home. We will also monitor that the service takes action to ensure the home has a registered manager. At the time of writing this report, we not going to change our inspection plan and will undertake a key inspection by 1 July 2010. However, we can inspect the service at any time if we have concerns about the quality of service or the safety of people using the service.
