What the care home does well The staff rotas show there are enough staff planned to be on duty to meet the nighttime needs of people using the service. The registered Manger confirmed that there are usually nine staff on night duty and this includes a qualified nurse. The policies displayed around the home show there are steps to be taken by night staff to make sure there are enough staff available to meet peoples care and support needs. This involves contacting senior managers to arrange staff cover for those who do not turn up for duty because of sickness.The staff rota for one day in October confirmed that two people were on sick leave. Discussion with the manager and one senior manager found that the staff on duty had not done everything they could to make sure there was enough staff available to meet the needs of people in the home. One unit manager said they did not receive a telephone call from the staff in home. However, once they had been contacted they attended the home and made sure staff were enough staff available. People are protected by the way medicines are dealt with as peoples medicine records show they are properly administered and recorded. Staff training records confirmed that people giving medicines are trained to do so correctly and safely. Discussion with people living in the home found they are happy with the staff and the services provided. One visitor to the home says "the staff are wonderful and caring". Another says "they are always made to feel welcome", and their relatives "quality of life as improved tremendously since coming to live in the home" What the care home could do better: The policies and procedures and rotas show there is a system in place for making sure there are enough staff on duty to meet peoples nighttime needs. However, there was evidence to show that on one day in October 2008 night staff did not fully follow the policy and procedure to make sure there was enough staff available to cover for staff sickness. The registered manager confirmed this and said they did not know the reason why the policy was not followed. The medicine administration records for one day in October 2008 shows that some peoples medicines were given earlier that they should have at 20:00 instead of the recommended 22:00. The manager said the intentions of the Unit Manager giving the medicines was good as they thought they were helping the night staff as they thought they may be short staffed due to sickness. Some peoples records including daily and medical records and the accident book and Regulation 37 reports indicate that there were no incidents on the evening of the 11 October 2008. Page 2 of 8 Brief description of the care home The Hemsworth Park Care Centre continues to provide accommodation, personal and nursing care to 54 younger adults who have a physical disability and 39 older people who may also suffer with Dementia, and 3 within this total who may have a diagnosis of terminal illness. Respite care is also offered. All accommodation provided is single and there is a passenger lift and assisted bathing provided. Prospective service users are invited into the home to test drive the services provided. Some residents attend community based day care services and in house activities are arranged for those who wish to participate. The home is divided into distinctive units with each having there own lounges and dining rooms. There is a cafe provided for the use of service users and visitors There are two small kitchenettes provided for more independent service users. Some take advantage of the local shops, post office and public houses nearby. The home is close to Hemsworth Water Park and some residents enjoy views of lake and surrounding countryside from their bedrooms. On the 1st November 2007 the manager said that the fees for living in the home ranged from #380.00 to #450.00 per week with extra charges for Chiropody (#10) and hairdressing (starting at #4.10). Information regarding the services provided and the role of the CSCI can be obtained from the home. The home is situated in Kinsley, is on a main bus route and close to the M1/A1 link roads. All services and amenities are only a few minutes journey away at Hemsworth. For those service users who require transport the care centre has two mini busses one of which has a tail lift for wheelchair users. Care Homes for Older People Page 3 of 8 What we found:
This visit to the home commenced at 09:30 and ended at 14:30. This visit was to look at the staffing levels in the home on nights and to look at the management systems in place to make sure there are enough staff available to meet the nighttime needs of people living in the home. We also looked at the way medicines are dealt with and in particular at night. Wee discussed these issues with the Registered Manager, Unit Managers, care staff and Nurses. We spoke to people living in the home and two visiting relatives. Wee looked at the Staff rotas, sickness records, and time sheets. We also looked at the accident book and Regulation 37 reportable incident reports. We also looked at a sample of peoples records including assessments, care plans, daily and medical records. The medicine administration records were also seen on one unit and this included the record of Controlled Medicines.
