What the care home does well In talking with individuals who live in the home they were very positive about the support they receive "I like it here" "its very friendly and staff are all very good". One individual told us "its very relaxed and everyone gets on which is good" and "I get the help I need" and "am much better then I was". From observation staff it was evident that there are good relationships with individuals and staff and staff have a good understanding of individuals. They spoke in a way which illustrated a good knowledge of mental healthneeds and an understanding of how to support individuals particularly where there was a change in mental health. Staff spoke of good working relationships with the manager and owner being "approachable" and "hands on". One staff member told us "if there are any problems we can approach the manager". They described the atmosphere of the home as "family orientated", with good communication between staff. Staff work in teams of two providing a consistency of care and continuity which is important in this type of environment. The environment of the home has improved with the building of an extension which has resulted in improved accommodation for individuals and modern facilities such as shower room. We looked at the requirements that had been made at our last inspection and found they had been met with risk assessments in place, good recruitment practice and supervision available to staff. What the care home could do better: We identified one area for improvement where we have made a requirement and this is in advising ourselves through Reg 37 notifications of incidents and concerns regarding the health and welfare of individuals in the home. Specifically where there are Safeguarding concerns or aggressive incidents which result in harm to others. We discussed with the manager the need to records any discussions about disclosures from CRB. In relation to the Safeguarding concerns this related to a visitor to the home we recommend that for additional security particularly where there is only one staff member on duty a "spy hole" be fitted to the front door of the home. This will enable staff to see clearly who is at the front door wanting to come into the home. Random inspection report

It provides a supportive environment where residents can pursue their chosen life style within an agreed and risk assessed programme. The House is situated close to local amenities and walking distance from the shops and the beach. This was an unannounced inspection and visit to the home as part of the inspection. The focus of the inspection was to look at the requirements made at our last inspection in June 2009, to examine admission assessments and care plans, staff recruitment and training records and accident and incident recording. There was also an opportunity to talk with individuals about living in the home and staff members about their view of the quality of care provided at Kismet. We also looked around the home and was able to see a number of the individuals accommodation. We looked at two pre-admission assessments and care plans they were comprehensive and through in providing information and history of the individuals psychiatric history. Care plans include risk assessments around behaviour and changes in mental health and a new format has been introduced which is more person centered and these are being completed with the individuals. They provided information about personal history and memories, hobbies and interests, beliefs and daily routines. Staff records relating to recruitment showed full and detailed employment information, CRB and POVA had been obtained with two references. For one individual there was information disclosed as part of the CRB there was no record this had been discussed at interview. Regarding this matter the manager advised that the applicant had been honest and open in interview about the disclosures and this had been discussed as part of the interview.Training had been undertaken as required including SOVA, Medication, First Aid, Food Hygiene, Mental Health Awareness. We discussed all staff undertaking Breakaway training and the manager advised this is being planned. Incident records were looked at and had been completed as required and there had been one incident where individual was physically aggressive towards staff member. We had not been advised of this incident through a Reg 37. Further the manager advised of a Safeguarding concern (possible financial abuse) which had been referred to the local authority and a strategy meeting had been held. Again we had not been advised of this concern through a Reg 37 notification. This inspection has not resulted in a change in the quality rating of the home in that this requires a full inspection. The manager asked how this could be addressed and we advised their writing to ourselves requesting a full inspection so that the rating can be reviewed. From this random inspection and its limited nature it is the inspectors view that the rating would be adjusted subject to other areas which were not examined at this inspection and no concerns being found which would warrent no change in the rating. It should be noted that Quality Ratings are now being withdrawn and a new system is being looked at and this is currently the subject of consultation. What the care home does well:
