What the care home does well Leftwich Green Community Support Centre offers short stay residential care, intermediate care and day care. Full and comprehensive information is provided about the provision, the facilities and care and support provided to help people make an informed choice about suitability. A nurse advisor works within the centre twenty hours per week and provides medical advice and support to service users and staff. General records within the home are well maintained and the manager and staff are able to evidence the work undertaken and the ongoing improvements made. Staff members are appropriately trained and competent. Training opportunities are widely available and staff members are keen to train and to gain qualifications. There is a low staff turnover and many staff members have worked at the centre for my years. The recruitment process is very thorough and staffing levels and the skill mix of staff are appropriate to meet the needs of service users. The complaints system is robust and staff members are alert and aware to protect service users.Domestic staff members ensure that the premises are kept clean and all staff members help to provide a welcoming and friendly environment. All people using the service and spoken with during the site visit were complimentary about the care and support provided by staff, the general standard of housekeeping, and the quality of the food provided. Clear lines of accountability exist within the centre, and the quality of the service provided is closely and regularly monitored. Any critical comments about the service are received positively and complaints investigated thoroughly to achieve resolution. What has improved since the last inspection? Since the last site visit in August 2006 there have been many improvements to the service. The information in the needs assessment documentation provides clearer information about the level of needs and support necessary. Staff members have continued to attend training and have gained qualifications. The centre has achieved a Platinum Food Safe Award. An activities co-ordinator has been appointed and a notice board for carers has been created in the reception area. Some areas of the centre have been refurbished and redecorated and all fire doors have been fitted with automatic closures. What the care home could do better: The medication administration records could be monitored more frequently and closely to ensure the recording of medication is clear and accurate. This home is registered for a maximum of 32 service users to include:* Up to 32 service users in the category of OP (Old age, not falling within any other category) * Up to 5 service users in the category of PD (Physical disabilities aged between 55 and 65 years) 3rd August 2006 Date of last inspection Brief Description of the Service: Leftwich Green is a purpose built community support centre owned and managed by Cheshire County Council. It is situated in its own grounds in a residential suburb of Leftwich, a mile from the centre of Northwich. The fees at Leftwich Green Community Support Centre are based on a financial assessment of the individual. The premises provide accommodation on two floors for up to 32 service users for short break or respite care. The service is mainly intended for older people aged 65 years plus. However the centre can accommodate up to 5 people aged 55-65 with physical disabilities, provided that the total number of service users does not exceed 32.This busy centre has a rapidly changing population of service users and can accommodate people needing intermediate care. Bedrooms are situated on both floors and are single rooms with wash hand basins installed. Individual bedrooms are personalised and comfortably furnished, can be locked and are equipped with a television. All bedrooms, bathrooms and communal areas are fitted with an emergency call system. There are several specially equipped bathrooms, which can be accessed according to need. Day care is offered for up to 22 service users, in a lounge, which is central to the building but used solely for that purpose. Workers from a local day centre provide the day care support. This is an overview of what the inspector found during the inspection. This unannounced site visit took place on 20th September 2007 over almost eight hours to assess if service users’ needs were being met. Prior to the site visit the registered manager had completed an Annual Quality Assurance Assessment form to provide up to date details of the service. The registered manager had prepared a useful file of evidence in readiness for the site visit, to support the information and to share with the inspector. Time was spent talking with some of the people who use the service and two stakeholders completed survey forms prior to the site visit to give their views about the care and support provided by Leftwich Green Community Support Centre. A tour of the building was undertaken to assess its suitability to provide a comfortable, homely and safe environment for people cared for and staff. The tour included six bedrooms, shared areas such as lounges and dining areas, bathrooms, toilets and the kitchen and laundry areas. Two members of the management team and several care staff contributed to the inspection process. Three people using the service were spoken with and commented on the services provided. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. This judgement has been made using available evidence including a visit to this service. The centre provides intermediate care, respite care and day care at times to suit individual demand and offers a very flexible service to a changing population of people with a wide range of care needs. EVIDENCE: There is a clear statement of purpose and good written information about the centre in the form of an information folder. People are now also able to take a virtual tour of the centre on the Cheshire County Council website.
Leftwich Green Community Support Centre Leftwich Green 517984.doc Version 5.2 Page 10 Every person using the service is referred through Cheshire County Council Care Management procedure either via Social Services or Health. Each person has a full and comprehensive needs led assessment before admission and a detailed care plan. Discussions take place between the care manager and the care co-ordinator or a senior care support worker to ensure identified care needs can be met. The staff members in the Community Support Centre help to complete further assessments and actively promote rehabilitation to help people being cared for to return home as independent as possible. The centre can simply provide regular respite care or a more supportive setting in which people can regain physical strength and confidence. The four care files sampled included the care file of a person receiving intermediate care. All four care files provided much evidence of a multi disciplinary approach to assessment, care and review. Since the last site visit it is evident that efforts have been made to ensure that all pertinent assessment information is transferred to the centre records. The initial assessment records were well maintained. Overall care needs were identified, assessed and met and the care plans and care programmes were fully completed and positively written. Self –care abilities were described and background information prior to admission was provided. The social history information in the intermediate care file was a little scant and could have provided the care staff with more detail. One care programme checked, had not been signed by the service user, the key worker or carer. Advice regarding these matters was given at feedback to the site visit. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 and 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Health and personal care needs are assessed, addressed and met and people using the service are referred to health professionals as appropriate. Frequent and careful monitoring of the medication administration could bring about improvement in the recording of medication and ensure the records are clear and accurate. EVIDENCE: The management and care staff members are very experienced in the care of older people and are familiar with age related difficulties. The centre is equipped to meet the care needs if older service users and the four care files checked provided evidence of appropriate referrals being made for specialist health care. Each of the four service users had been admitted following full and thorough assessments of need and each had a plan of care including information from health and social services and primary carers.

All contacts with health professionals and outcomes of healthcare visits were well documented and risk assessments had been thoroughly completed to identify and reduce potential risks. The ongoing records of care provided evidence of continuity of care and close monitoring, and gave details of close liaison with medical professionals to address emerging health care needs. Service users from outside the immediate area are temporarily registered with a local General Practitioner, during their stay. A nurse advisor is based at centre for twenty hours each week to provide health care support to service users and a local pharmacy provides support and advice. Within the last twelve months all senior care support workers and acting senior care support workers, the team manager and care co-ordinator have completed a distance learning pack and attained a certificate in Safer Handling of Medicines. All the medication administration records were checked from the 1st of September 2007 onwards and improvements in the accuracy of record keeping were noted. The records contained photographs of service users and a record of their bedroom number was provided to help aid identification. Information was provided to staff members about self -medication, allergies experienced and omission codes to be used. Some photographs were not secured into the files, two unexplained gaps in recording were found and one record had been altered. One surname had been recorded using two different spellings and the frequency of administration, was indicated by using Latin abbreviations instead of numbers. Advice was given regarding these matters at feedback to the site visit, as there is a need to frequently and closely monitor the medication administration records to ensure the recording of medication is clear and accurate. Each member of staff had been issued with infection control guidance to enable them to provide appropriate care and support and to keep the centre free from infection. One of the people receiving care said that they had been apprehensive about receiving care at the centre, as this had been their first planned stay. They said they had received a warm and friendly welcome from care staff and everything had been explained to make them feel at home and reassured. During the site visit care staff were seen to encourage service users to retain their independence and to support them. Staff members were knowledgeable about the people being cared for and a genuine concern for the health and welfare of people being cared for was evident. During the site visit staff members were observed to anticipate needs and interactions with service users were friendly, respectful, kindly and reassuring. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 and 15. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. There are opportunities for leisure and recreational activities to suit service users’ expectations, preferences and capacities. Community involvement is encouraged and people using the service enjoy the flexibility and individual approach to caring within the centre. The food provided is varied, appealing and appetising and provides people using the service with a good standard of nutrition. EVIDENCE: An activities co-ordinator had been employed solely to organise activities for short stay service users and spent time with service users to find out their preferences for activities. An activities newsletter and activities file was seen and provided evidence of a range of planned activities. People being cared for had discussed interesting newspaper articles, completed giant crosswords, word searches and quizzes. In craft sessions, glass and tile mosaics had been made, people had been painting and had decorated hand made gift boxes.
Leftwich Green Community Support Centre Leftwich Green 517984.doc Version 5.2 Page 15 The post of activities organiser has again recently become vacant and efforts are being made to try to cover the vacancy with staff already employed. Leftwich Green Community Support Centre has encouraged community involvement and has welcomed visits from police cadets and visits and placements from students from a local private school and college. This involvement has introduced younger people to work within the care sector and has fostered good relationships within the community. Whilst staying at the centre, people are encouraged to maintain contact with family, friends and representatives as they wish. Visitors are made welcome and are asked to record their entry and departure in the visitors’ book for fire safety and security reasons. The visitors’ book is positioned on the reception desk in the foyer. The care files provided evidence of people being enabled to make choices during their stay. Preferred rising and retiring times had been recorded, personal likes and dislikes had be recorded and meal choices and preferred eating arrangements were noted to make each persons stay as comfortable as possible. In discussion with service users, several people expressed a high level of satisfaction about the range and quality of meals provided. Sample menus were seen and offered a wide choice of home made foods with alternatives available. Cooked breakfasts were available upon request and vegetarian and special diets were provided as necessary. For 2006/2007 Leftwich Green Community Support Centre achieved a Platinum Food Safe Award via Vale Royal Borough Council. The kitchen was checked during the site visit and was well maintained, well organised and clean with food safely stored. The cooks have worked at the centre for a considerable number of years and do all they can to ensure people receive wholesome food, which they enjoy. They ensure people staying at the centre are aware of food choices and provide menus so that the people being cared for are aware of the choices available. The cooks have recently introduced written handover sheets to aid communication with each other. They have commenced a stock file to make the re-ordering of food easier and to ensure the smooth running of the kitchen. This judgement has been made using available evidence including a visit to this service. Any critical comments about the service are received positively and are recorded and investigated thoroughly to achieve resolution. Staff members are alert to the potential for abuse to help protect the people cared for. EVIDENCE: The complaints file and compliments file were checked. Complaints and concerns were few, well recorded, investigated and responded to appropriately. Cheshire County Council has a robust complaints procedure and information about how to make a complaint was available to service users and their primary carers. Staff members are encouraged to assist people using the service to make their feelings known and all comments are welcomed. The compliments file provided much evidence of appreciation of the service and of satisfaction with the care and support provided. All staff members have a basic awareness of adult protection and have the Department of Health guidance ‘No Secrets’ available to them. A whistle blowing policy is available to staff to enable them to respond appropriately to any suspicions or evidence of abuse.

This training has helped to raise staff awareness and ensures they have the necessary skills to respond appropriately to help protect service users. This judgement has been made using available evidence including a visit to this service. Leftwich Green Community Support Centre is well maintained, well equipped, clean, decorated and furnished to a good standard, which helps to create a comfortable and welcoming environment in which to stay. EVIDENCE: The location and layout of the centre is suitable for the needs of people being cared for. The centre was purpose built by Cheshire County Council and is fully adapted and equipped for its purpose. There are sufficient bathrooms throughout the building fitted with equipment to aid mobility.
Leftwich Green Community Support Centre Leftwich Green 517984.doc Version 5.2 Page 19 A tour of the building was undertaken and included the communal areas, the kitchen, laundry, shared bathrooms and toilets, six bedrooms and the garden area. The communal areas provided a homely environment and all areas visited within the home were fresh and clean. Advice was provided at feedback as one bedroom was in need of redecoration and another needed replacement furniture. The garden contained a few older items of furniture and equipment awaiting removal and some raised flags on pathways had begun to create a tripping hazard. The area used by day care for storage, was very untidy and not in keeping with the rest of the centre. The kitchen and laundry areas were clean and well organised, with all equipment in working order. Supplies of protective clothing were available in the bathrooms and for staff entering the kitchen area. The laundry had accumulated many items of unclaimed clothing, which awaited disposal. A number of improvements to the centre have taken place during the last twelve months. All fire doors had been fitted with automatic closures, and new kitchen units had been fitted to all four lounge/dining areas. Two of the lounge/dining areas had also been decorated. A carer’s notice board had been provided in the reception area to provide useful information to support family carers and service users. Some staff members have received health and safety training and infection control training to help raise awareness, and quarterly Health and Safety checks are carried out. Discussion was held with several people receiving a service and all confirmed that they were happy with the standards of cleanliness within the centre and with the comfortable rooms provided. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 and 30 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Staff recruitment is thorough to ensure people using the service are protected. Appropriate staffing levels are maintained to ensure staff members have time to complete all care and support tasks. Staff members undertake training and qualifications to ensure they are confident and competent and people feel well cared for. EVIDENCE: The staffing rotas were checked for weeks commencing the 8th and 15th September 2007. They were well maintained and provided evidence of generous staffing levels to ensure staff members had enough time to complete all necessary care and support tasks. The existing staff team had covered any vacancies. The staff team at the centre are well trained, confident and competent. Staff members confirmed that they have opportunities for training, qualification and development. There is a low staff turnover with many of the staff having been employed at the centre for several years. There is a staff team of mixed ages and differing work/life experiences to support service users.

All staff members have continued to progress to achieve National Vocational Qualifications. Total People are currently working with four members of staff towards their technical certificates and NVQ 2 in Care. One of the four had achieved NVQ candidate of the year in her category. The training records were checked. Of thirty -one residential care staff eighteen care staff have received training about caring for people with dementia. Nearly all care staff members have received moving and handling training and valuing diversity training. All senior care support workers have received emergency aid training to ensure there is a trained person available on each shift. Further infection control training, food hygiene training and training about mental health, depression and challenging behaviour is intended. The fire training for staff was up to date. Cheshire County Council are committed to providing training opportunities to ensure staff have the appropriate skills and knowledge to deliver the necessary level of care to service users. An individual record of training is maintained for each member of staff. The recruitment files for three recently employed members of staff were checked. Each file was very well organised, appropriate recruitment checks had been completed and written references had been obtained. This judgement has been made using available evidence including a visit to this service. There are clear lines of accountability and regular quality monitoring which helps to ensure that service users’ best interests are safeguarded and the health, safety and welfare of service users and staff are promoted and protected. EVIDENCE: The registered manager has many years experience in the provision and delivery of care to older people and continues to undertake training to inform her work.

The home care staff members have recently relocated to another building out of area. Home care currently retains a small office base at Leftwich Green. In April 2007 responsibility for the Day Services at Leftwich Green also became part of the centre team manager’s remit. As a result of the Transforming Cheshire Agenda, consultations are currently underway regarding the proposed closure of one Community Support Centre in Central Cheshire and a viability study will explore the possibility of providing a dementia unit at Leftwich Green. There are clear lines of accountability within the centre and close liaison and support from higher management within the County Council. The Locality Manager visits monthly to check the quality of the service and to focus on any areas needing improvement. One area of good practice was that a new format for recording these visits had recently been developed by County Council and issued to locality managers with guidance notes. Each visit will include inspecting the premises, records of complaints and other relevant records and the visit will also include discussion with staff, service users and carers. The reports will evidence how each service is meeting the National Minimum Standards and achieving outcomes for service users. Representatives of the Local Authority also regularly visit the centre to report on the quality of the care and the provision. The results of a quality standards survey had been collated and the results were on display for service users. 34 questionnaires were returned between April and June 2007.Of these, 96 of respondents said they felt safe and secure at Leftwich Green and the same number said staff were polite and respectful. 61 of respondents felt that the service definitely helped them and 39 felt it helped quite a lot. A number of additional comments were made. The things people liked about staying at Leftwich Green were the staff, the food, the care given, other service users, being local and having the same room. People thought that the mattresses were uncomfortable and they preferred blankets to duvets, they also didn’t like having to wait for staff to answer call bells. All comments were responded to positively and as a result, staff members have been reminded to answer bells as soon as possible including during their change over time. Mattresses are continuing to be replaced but because the centre needs to be able to wash bedding it means it is more practical to use duvets instead of blankets. Service users are safeguarded by the accounting and financial procedures within the centre. Two examples of service users finances and balance sheets were checked and were accurate and service users valuables held for safe keeping were well documented. Fire training records and fire precautions records were checked and were up to date and a fire risk assessment was in place. There was evidence of students being made aware of the fire procedure and what to do if fire alarms go off. The registered manager ensures that risk assessment are carried out and recorded in respect of all safe working practice topics.
