What the care home does well The provider uses professional judgment in consultation with the GP and practice nurse. He has so far not encountered any problems for deciding on best treatment so therefore has not needed to refer to a specialist tissue viability nurse. The provider uses validated risk assessment tools alongside his clinical judgement and that of other healthcare professionals. The provider has taken guidance fully into account when exercising clinical judgement. He has made decisions appropriate to the circumstances in the individual person in consultation with the next of kin, GP and practice nurse. Staff are provided with opportunities for updates in training on wound care. Outcomes for people have been met in that health needs are regularly monitored and the home arranges for health professionals to visit as required. What the care home could do better: The provider agreed to record the clinical grade of any wounds. He also agreed to keep the nutritional risk assessment format used as a guide each month, as a formal record on file. Commission for Social Care Inspection Reports for this service are available from the provider or can be obtained from www.csci.org.uk Care Homes for Older People Page 3 of 7 What we found: We, the commission, conducted this random inspection to review the management of people who have pressure ulcers and need wound care. We looked at the records of three people who have pressure ulcers. We found that the service conducts assessments of the wounds and reassesses them at appropriate intervals. Related assessments of pain, nutrition, mobility and mositness are also completed. If the condition of the wound or person changes, the service re-evaluates the treatment plan as soon as any evidence of change is noted. The service assesses wounds in the context of the persons overall physical and psychological health. Adequate dietary and fluid intake is provided to prevent malnutrition that is compatible with individuals wishes or condition. The service liaises with other health care professionals to discuss treatment and uses a validated wound management formulary devised by the Shropshire Tissue Viability Group. The service has adequate provision for privacy when changing dressings. It has adequate provision for the disposal of soiled dressings and hand washing facilities. Four qualified staff have attended the Shrophsire wound conference in 2008.
