We looked at the records of three people who have pressure ulcers. We found that the service conducts assessments of the wounds and reassesses them at appropriate intervals. Related assessments of pain, nutrition, mobility and mositness are also completed. If the condition of the wound or person changes, the service re-evaluates the treatment plan as soon as any evidence of change is noted. The service assesses wounds in the context of the persons overall physical and psychological health. Adequate dietary and fluid intake is provided to prevent malnutrition that is compatible with individuals wishes or condition. The service liaises with other health care professionals to discuss treatment and uses a validated wound management formulary devised by the Shropshire Tissue Viability Group. The service has adequate provision for privacy when changing dressings. It has adequate provision for the disposal of soiled dressings and hand washing facilities. Four qualified staff have attended the Shrophsire wound conference in 2008.
