What the care home does well There is evidence that residents` needs and feelings are taken into consideration when medicines are given. Many of the residents` medicines were handled and recorded correctly. What the care home could do better: Medicine records need to be clear, complete and accurate at all times. The quantities of medicines in traditional packs should be monitored. This shows that the medicines have been given correctly and there is no mishandling. Residents` medicines, including dressings, feeds and sundries must be stored hygeinically, off the floor. Care Homes for Older People Page 3 of 8 What we found: We carried out this medicine inspection because the lead inspector requested it, to be sure that medicines were being handled and recorded correctly. The visit took about three hours. It involved looking at residents medicines and associated records and speaking with staff and residents. Then we fed back our findings to the manager. We found that, although there were some shortfalls in the record keeping, medicines were mostly safely managed. This protects the health and wellbeing of the residents. We looked at how medicines were stored and found them in secure facilities. This makes sure that unauthorised people cannot get to them. There was a labelled space on the shelves of the store cupboard for each residents medicines. Where possible, the medicines in use were in a calendar pack that contains a months supply, separated according to the day and time. This makes it easy to see the doses that have gone. When these are compared with the records it shows whether the medicines have been given correctly. There was no system to record amounts of medicines in traditional containers to compare with the records. The cupboard was used to store other goods. It had become untidy and feeds and some monitored dose packs were stored on the floor. It was agreed that this would be sorted out. The manager told us that all the staff on the specimen signature list (kept in the folder with the medicine records) had been trained to manage residents medicines. The manager said that concern had been raised that staff did not always wait with residents, to see that their medicines had been taken properly. A procedure is now in the medicine folder and the manager supervises staff so that this is now done correctly. We looked at the medicine records. Many were clear and complete. There were some unexplained gaps in the records of giving medicines. There is a system to record residents not having their medicine, using letter codes. The reason was not always clear. Also when staff recorded the receipt of a medicine into the home they did not include the date of receipt. Two residents were supported to take their own medicines and care was taken to give two residents their night medicines at 21:00 hours because they wished to go to bed early. The manager explained that staff would be offerred support and supervision to improve the record keeping.
