What the care home does well The service was aware of the need to evidence actions to improve the quality of its care for people at risk of developing pressure sores. We noted that staff had wound care training and trained nursing levels had been raised in accordance to a requirement in the previous inspection report. Due to the limited focus of this inspection, we did not gather evidence on this occasion relating to areas of practice that the service has previously achieved good outcomes for, such as coments from residents (and their supporters) that they have developed positive relationships with staff, enjoyed activities and entertainments, received support to maintain links with the wider community ,and benefitted from the relaxed and homely atmosphere. In the course of this inspection, people living at the home informed us of the forthcoming Christmas parties and spoke warmly about current and former staff. However, the findings of this inspection indicated that continued improvement for the prevention and management of pressure sores was needed. What the care home could do better: We found that the service was benefitting from the input of the PCT liaison nurses; there were no concerns regarding the documentation for the residents known to have pressure sores or leg ulcers. Other issues of concern detected at this inspection identified that staff were not consistently applying principles of good care; for example, we met a resident with sores between their toes that did not have a care plan for their health care problem although they were receiving medically directed treatment. We noted that staff needed to be more vigilant, since we found a discontinued prescribed cream in a bedroom and hazardous cleaning fluids (owned by a resident) were not kept in a lockable facility. Date of last inspection Brief description of the care home Manor Farm is one of a group of care homes run by Trees Park (East Ham) Ltd. It is a purpose built dual registered residential and nursing home. It is registered to take 81 residents over the age of 65 years with dementia care needs, general residential and general nursing care needs. The home is divided into three units. The top floor is residential while the ground and first floor units are for residents with nursing needs. There are single and double rooms, all with en-suite facilities. This unannounced random inspection was conducted due to concerns identified regarding how this service addressed health care needs of the residents, with particular reference to the prevention and management of pressure sores. These concerns have been subject to investigations and strategy meetings by the Newham Safeguarding Adults Team in recent months. The aim of this inspection was to review the care for any residents identified to have a pressure sore at the time of our visit. The nursing staff within the care home receive very regular support by the Newham Primary Care Trust (PCT) liaison nurses; therefore, we reviewed our findings with a member of this team during the course of this inspection. The inspection has been focused upon the reading of six care plans, checking staffing levels and skills mix for the nursing units, and checking upon training arrangements.
