Care Homes for Older People Page 3 of 8 What we found: We visited the home because a previous inspection found medicines were not always being handled safely. The visit was carried out by a pharmacist inspector and lasted approximately three hours. We looked at medicines records, medicines stock and their storage arrangements. Overall we found some good improvements in the way medicines were handled that helped make sure medicines were being given to people correctly. We looked at the records of medicines given to people and found significant improvements. Records of medicines received into the home were properly recorded and there was a clear system of stock control that made accounting for medicines simple and easy to do. The general arrangements for medicines returned to the pharmacy were improved and records were accurately made. Stock was promptly disposed of when no longer required to help prevent any mishandling. We looked at a more detailed sample of medicines, particularly medicines not contained in the specially dispensed system (this is made by the pharmacy to make giving medicines simpler to do). We found two examples of medicines not adding up showing they had probably not been given to people correctly. We discussed these mistakes with the manager and gave advice on how to prevent this happening again. We looked at the times medicines were given and found some medicines were not given at the right and best time for people. Records were not always clearly highlighted with the right time and we saw two specific examples when medicines that should be given before food were given after. Giving medicines at the wrong time can stop them working properly. We checked how controlled drugs (medicines that can be misused) were handled. The cupboard used for storage was now properly attached to the wall making it secure. A special register was used for record keeping and this was usually completed correctly with all entries properly witnessed. Secure storage and detailed records help prevent mishandling and misuse. The arrangements for the storage of medicines had been further improved since our last visit and all medicines were now properly and securely stored. We saw an example of a medicines audit (checks on the medicines) that had been completed the month before. We gave some advice on how to develop this further to help make sure all medicines handling procedures are regularly and thoroughly checked. We looked at how staff were trained and how their competency when handling medicines was assessed. On our previous visit we had advised the manager to record any formal observations of staff when handling medicines but no paperwork to support this had been developed. We gave some advice on how to do this because regular recorded competency assessments on staff help make sure they have the necessary skills to handle medicines safely.
