Moray Lodge Care Home offers respite care to service users who have mental health needs, excluding learning disability and dementia. Northamptonshire County Council provides the service in partnership with the Northamptonshire Primary Care Trust, and weekly fees are based upon the level of support required and dependency level. Moray Lodge is located in the village of Duston close to Northampton Town centre with access to a range of community facilities and services. Private accommodation is in single bedrooms and there is a range of communal areas including a games room. Accommodation charges are not made however people using this respite service pay #5.00 per day for meals prepared by the home.

As part of this Annual Service Review of Moray Lodge we looked at all the information that we have received, or asked for, since the last key inspection. This included reviewing information provided within the The Annual Quality Assurance Assessment (AQAA) that was sent to us by the provider of the service. The AQAA is a self-assessment tool that enables the provider to reflect on the service and self assess how they meet the Care Homes for Younger Adults (18 - 65), National Minimum Standards (NMS). The NMS covers eight outcome areas, choice of home, individual needs and choices, lifestyle, personal and healthcare support, concerns, complaints and protection, environment, staffing and conduct and management of the home. It gives the provider the opportunity to tell us what the service does well, what they could do better, where they have improved and areas for future improvement. It also provides us with numerical information about the service. The provider is legally required to notify the Care Quality Commission (CQC) about events that happen in the service which may affect the health, safety and welfare of people using the service, these are called notifications of events under Regulation 37 of the Care Homes Regulations 2001. We reviewed information we received from stakeholders and other Health and Social Care professionals, who are involved with the care of people using the service, and any concerns, complaints or safeguarding information we may have received since the last key inspection of the service. What has this told us about the service? Moray Lodge sent us their AQAA when we asked for it. It was clear and detailed giving us all the information we asked for. The provider continues to tell us through the submission of Regulation 37 notifications, events that have happened since our last key inspection, this shows that they continue to manage the service well. Information within the AQAA told us that people using the service and their advocates are consulted in many ways to gain their views about the quality of the service and equality and dignity is interwoven into all aspects of the service. The AQAA gave detailed information on how the service meets the seven NMS outcome areas. The areas where the service identifies they do well are in offering both respite and short term rehabilitation for people who have enduring mental health problems. This umbrella of support enables the service to utilise their extensive experience within Annual Service Review Page 3 of 6 mental health, which is formated by them in their policies. Our service is jointly monitored by NHT and NCC. The unit has a steady staff compliment, which enhances the care provision. Areas where the service identifies improvements have taken place are in always looking for ways to improve, both from the service users and staff points of view. In addition, they are endeavoring to improve the environment. Areas where the service identifies they could do better is to continue with ongoing staff training and monitoring of systems in place. The data provided within the AQAA confirms that of the sixteen care staff working at the home fifteen hold National Vocational Qualifications (NVQ) in care at level 2 or above. The AQAA identifies the policy on aggression toward staff was last reviewed in 2004 and the policies on emergency admission and detention under the Mental Health Act 1983, equal opportunities and diversity and anti- oppressive practice were last reviewed in 2006. These policies would benefit from review to ensure they remain in line with current practices and legislation. Since the last key inspection CQC have received no complaints about this service The AQAA confirms that no complaints or safeguarding alerts had been made direct to the service. Reportable events through the Regulation 37 notification process has informed CQC of three self medication errors. There has been eight instances were people using the service have absconded and later returned to the home. There has been one instance involving a small fire within the home that resulted in a person having to receive medical treatment. We were informed that in April 2009 of the home being broken into on two occasions one involving petty cash being stolen and a staff car being broken into within the car park. We were informed in June 2009 of another attempted break into the home and also another incident of vandalism to a staff car in the car park. The police were involved on each of these occasions and provider has stated that security is being reviewed. What are we going to do as a result of this annual service review? The next inspection of this service will be based on the Fees and Frequency Regulations 2007 and the assessment of risk of the service. Further clarity will become evident as the new registration and inspection system under the Health and Social Care Act 2008 is confirmed. Our opinion of the service has not changed.
