Mount Pleasant is a Care Home registered to provide personal care and accommodation for up to 50 people in the category of Older Persons. What the care home does well Staff who administer medicines have received training and their ability to carry out this role has been assessed. During the inspection, medicines were given to people in a safe and courteous manner. Steps have been taken to ensure that accurate records of the use of medicines are kept. The receipt of medication into the home was accurately recorded. What the care home could do better: Best practice is not followed in all aspects of the management of controlled drugs. It is not always clear from the medication administration record (MAR) when a medicine has been discontinued by the doctor. There is no written procedure on supplying medicines to people leaving the home temporarily on social visits. Each member of staff who administered medicines had a training file which confirmed their ability to handle medicines safely. A list of specimen signatures and initials of staff authorised to give medicines was kept with the medicine charts, so that staff signing the charts could be easily identified. We watched the lunch-time medicine round and saw that the procedure for administering medicines followed good practice guidance. Each person prescribed pain relievers when required was asked if they needed the medicine at that time. We found that accurate records of the receipt and disposal of medicines are kept. Medicine charts are audited by the manager to make sure that administration records are properly completed. Medicines supplied to the home in boxes or bottles are dated when first opened so that the amount of medicine remaining can be checked against the record of administration. We noted that medicines that were no longer prescribed by the doctor were still listed on some peoples medication charts, with the comment none supplied this month. Allergies were recorded for some people, but there were no statements on the charts of other people to confirm that they had no known allergies to medicines.We spoke to two people who took responsibility for taking their medicines themselves, and saw the lockable drawers for storing medicines in their rooms. We saw that storage facilities for medicines were clean and tidy. Medicines in current use are stored in medicine trolleys that can be chained to the wall of the medical room. The room is kept locked. Some medicines were stored in a metal stationery cupboard which, although fitted with an extra lock, was not very secure. The home had no paperwork to show that the cupboard used to store controlled drugs met legal requirements. We carried out a stock check of a sample of controlled drugs kept in the home and found that the stock levels matched the records. We noted that the home does not routinely carry out stock checks of its controlled drugs, nor is a kit available for destroying controlled drugs no longer required. A record of the return of a controlled drug to the supplying pharmacy was only signed by one person. The companys medicine policy and an up-to-date reference book on medicines (the British National Formulary) are available for staff use.We asked about arrangements for people to take one or more doses of medicine while away from the home, and were told that medicines are sometimes given to people going out during the day in envelopes.There was no written guidance for staff on how medicines should be supplied for social leave.
