The New Thursby is registered with the Commission for Social Care Inspection to provide care and accommodation for up to 40 older people who require nursing care. Care Homes for Older People Page 3 of 9 What we found: We visited the home because previous inspections had found some shortfalls in the way medicines were handled. The visit was carried out by a pharmacist inspector and lasted five and a half hours. The new acting manager, the director of nursing services for Century Healthcare and two nurses were spoken with. Overall we found improvements in the way medicines were handled that helped make sure people were getting their medicines correctly. We found some general improvements in the way medicines records and stock were organised. Records of medicines received, given to people and disposed of were usually clear and accurate. When we checked the stocks of medicines against the records we found that these usually added up showing medicines were given to people correctly. We looked at the times medicines were given and found continued improvements in the general organisation of the medicines rounds. Medicines were being given at the right time in relation to food intake and clear records were being made to show that this was happening. Giving medicines correctly and at the right time helps make sure the health and wellbeing of people is protected. We looked at a sample of care plans and records and found continued improvements in the way information was recorded for when required medicines. In particular we saw some good information about how painkillers were to be used. However a care plan and the general information about how someones insulin was to be used was not clear enough and on three occasions in the last week the wrong dose was given. Another medicine used for treating seizures was not recorded on a persons current list of medicines even though it was in stock and highlighted in their care plan. Having up to date information about medicines helps make sure they are given to people correctly. We checked how controlled drugs (medicines that can be misused) were stored and recorded. A secure cupboard was used for storing them and suitably witnessed records were being made. However, we found three occasions in the last two months when staff had forgotten to replace some strong pain relief patches at the right time, this could have resulted in these people suffering unnecessary pain. We looked at how medicines were checked and audited. We saw two formally recorded audits that were suitably detailed, these had found mistakes and discrepancies that the manager had then acted upon to help prevent them happening again. Further medicines handling training had been provided to all nursing staff since the last inspection and all nursing staff had been formally assessed by the manager as competent in handling medicines. Efficient recorded checks of the medicines and regular training and competency assessment help make sure medicines are handled safely and help make sure staff have the necessary skills.
