Oakwood Bungalow provides nursing and personal care and support for up to 10 younger adults with learning disabilities. It is situated on a main bus route, to the north east of Chesterfield within a residential area, close to local amenities. The home aims to provide a stable and consistent lifestyle for service users, which enables them to develop within an environment, which is supportive to their individually assessed needs. The home`s philosophy of care is based on O`Brien`s Life Principles and Accomplishments, including the promotion of normalisation in terms of the daily life of each service user. Care delivery is planned, based on a recognised assessment model and within a framework of risk management. Service users bedrooms are all single occupancy, each having a wash hand basin. There are separate male and female toilets and bathing/shower facilities. There is a choice of lounge space and access to a small well-kept garden area. A dedicated laundry facility is provided, although those service users who are able can undertake their own laundry with assistance from staff. A team of Registered Nurses and care workers supports the Manager and management of the home. The current standard fees are £1068.38 per week.

What the care home does well The home continues to provide good facilities for services users, in particular service users bedrooms. What has improved since the last inspection? The majority of the requirements from the previous key inspection have been complied with by the time of this inspection. The Registered Manager is now back working full time at the home and this has benefited both the service users and the staff group.There has been a reduction in the number of serious incidents that resulted in a notification to the Commission for Social Care Inspection. This has created a cohesive staff group which is working constructively with service-users. The home has a good percentage of qualified and appropriately trained staff. The home has increased its registration from ten to eleven. There have been no other major service changes. This was an unannounced key inspection and took place over a total of seven hours. An inspector from Experts by Experience accompanied the inspector from the Commission for Social Care Inspection and was present for three hours of the inspection. During the site visit both inspectors made a tour of the home and spoke with several residents and staff. The inspection activity during this site visit was to assess the service against the key National Minimum Standards and these are identified through the report. There were ten service-users in the home on the day of the inspection; the home currently has no vacancies. Additionally, time was spent in preparation for the visit, looking at the service history, the previous inspection report and the Annual Quality Assurance Assessment questionnaire. Records were examined relating to the service users and the general running of the home. The Commission for Social Care Inspection sent out the ten “Have Your Say” questionnaires, and received seven completed questionnaires from serviceusers who confirmed they were happy at the home and were looked after by the staff. The Homes Statement of Purpose, Service user Guide and inspection report from the Commission for Social Care Inspection were displayed in the main entrance. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 2. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Arrangements are in place to ensure that service users’ needs are fully assessed and met prior to admission. This ensures that all potential service users holistic needs are appropriately met. EVIDENCE: The majority of the service users who are admitted to the home have their needs assessed by social workers or through the care management system. The single assessment then forms part of the planned care service users receive. Also the home undertakes their own individual comprehensive needs assessments. This was in accordance with a recognised care model and provides a person centred record of their individual needs, including identified strengths and needs, long-term goals, and evaluation. As discussed with the Registered Manager this could be developed further to include service user’s life story. There was evidence on record to show that care management were reviewing the care needs assessment. However, the Registered Manager stated that some Authorities are not pro-active in reviewing and returning care needs review documentation. The Registered Manager agreed to look into this. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 6,7 and 9. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. There is a care planning and review system in place, which ensures that service users individual needs are met. Service users’ are supported to achieve an independent lifestyle and participation in all aspects of the life at the home is encouraged. EVIDENCE: During the visit care plans of two service users were examined. The staff member for each service user has compiled the care plans. Evidence was seen of care plans being reviewed on a regular basis. All service users case tracked had a comprehensive care plan, which was in accordance with their assessed needs and formulated within a risk assessment framework. All care plans were very detailed and comprehensive including services users’ individual lifestyle preferences and choices. Daily records are also maintained on each service user. Care plans had been signed by either the service user or their representatives. Oakwood Bungalows Nursing Home DS0000002068.V351982.R01.S.doc Version 5.2 Page 10 The inspector from Experts by Experience discussed with the care staff about making care plans more personal and in a more easily understood format. The Registered Manager confirmed that this has already started and the inspector from Experts by Experience viewed the new style format. This is in line with a more Personal Centred Planning approach and will run in tandem with the present clinical model. Care plans could be in different formats such as DVD, CD or pictures and include life stories, favourite things, and wishes for the future. The Registered Manager recognised that staff will need further training in this approach to care planning. Service users confirmed that they have access to an independent advocate who visits the home on a regular basis. However the inspector from Expert by Experience noted that there was little information about this service in the home. Service users’ files also contained a wide range of related risk assessments in such areas as behavioural/psychological issues, matters affecting physical health, tissue viability, nutrition, mobility, risks associated with health and safety and social activities. Activity timetables are now being included in service user care records. Staff will record when service users attend or do not, as one service user stated they were not enough activities for them in the daytime. Risk assessments were also monitored and updated as required. All service users have a named nurse and key-worker. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12,13,15,16 and 17. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. There were arrangements in place to enable service users to maintain and develop appropriate relationships. The home provides a well-balanced and varied menu. EVIDENCE: The care records of two service users provided detailed needs assessment and care planning information regarding their social, recreational, educational and occupational activities both within the home and outside in the community. Service users have access to IT facilities however the access is limited; one service user stated they would like to have their own computer. There were a variety of activities on the day of the visit however services users who spoke with the inspector from Experts by Experience said that they would like more activities, as sometimes they get bored. Information on service users’ records indicated that contact with family and friends were appropriate. Service users wishing to speak with family and friends by telephone have to use the home main telephone line. The inspector from Experts by Experience noted that there are little opportunities for service users who wish to have friends and family to stay. The relationships observed between care staff and service users appeared open and good-humoured. Service users are encouraged to participate in the running of the home, and several service users indicated they would like to take a more active role in daily household tasks and routines. The daily routines are flexible with the service users being able to make their own decisions about how they spend the day. Services users who spoke to the inspector from Experts by Experience did comment that they would like to do more cooking and see to their own laundry. From examination of the menus the home is providing a healthy well-balanced and nutritious diet. The service users made positive comments about the meals and the cook takes their preferences into account before compiling the weekly menus. The fridge temperatures were seen and were within a safe range. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 18,19, and 20. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users receive personal and health care support in a way, which promotes their independence and is in accordance with their preferences and beliefs. EVIDENCE: Records examined and discussions with staff, indicate that service users’ health and personal needs were being met. Service users have their own personal health action plan which records all their personal and heath care needs. Service users were generally healthy and records showed that staff promptly contacted the appropriate medical services if required. All service users attended services within the community including doctors opticians, podiatry, dentist, and audiologist. Records examined showed that specialist advice and relevant reviews were routinely obtained from Ash Green Community Hospital for people with learning disabilities. The home runs their own health promotion groups “ Well men and women clinics.” Service users spoke with the inspector from the Experts by Experience and said it was good to have their own money; they helped the staff with the cleaning and could make their own drinks. The arrangements for receipt, storage, administration and disposal of medication were also examined and found to be satisfactory. The home has a clear audit trail for all medication administered. Only qualified staff administer the medication. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 22 and 23. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has robust procedures in place to protect service users from harm. EVIDENCE: The home has a complaints procedure, which is included in the service users guide which all service users have a copy of. The complaints procedure is also displayed throughout the home. Oakwood have developed a format which is accessible to service users as several of the service users are not able to fully communicate their concerns. The procedure contains the new complaints address of the Commission for Social Care Inspection and informs the complainants that they are able to contact the Commission at any stage of the complaints process if they wish to do so. The Registered Manager confirmed that the home had good links with the Derbyshire Advocacy Service, and they support the service users in making complaints should they wish to do so. The Advocacy Service also takes service user meetings within the home. Several service users have been attending a self-advocacy group and comments from them were positive, and they described it has “ expressing themselves”. However the group has now stopped running due to the lack of funding. Service users spoken with during the visit stated they would speak to staff or Registered Manager if they had any problems. Oakwood Bungalows Nursing Home DS0000002068.V351982.R01.S.doc Version 5.2 Page 16 Records seen indicated that no complaints had been received from service users or their representatives about their care since the last inspection. The Registered Provider signs the complaints book after each Regulation 26 Visit. The Commission for Social Care Inspection has not received any formal complaints about this service since the last inspection. The policy on adult protection has been updated and now references this as Safeguarding of Adults. The policy is comprehensive and covers all aspects of Safeguarding as outlined in the National Minimum Standards. The Registered Manager confirmed that all staff have received training on the safeguarding of adults, and training on handling violence and aggression. The home maintains a record of all incidents of aggression and as discussed with the Registered Manager these have diminished in recent months. There has been one new safeguarding issue since the last inspection and this was investigated under Derbyshire Safeguarding procedures. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 24 and 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users were accommodated in a safe and well-maintained environment, which suits their needs and promotes their independence and privacy. EVIDENCE: The Inspectors carried out a full tour of the home, accompanied by the Registered Manager and service users. All communal areas were inspected together with staff facilities. Service users’ bedrooms were inspected with their agreement and all rooms had been decorated and furnished to their personal choice and were being personalised. Communal areas were generally bright airy and well lit. The Registered Manager stated that they had a rolling programme for refurbishment of the home and had identified areas which required attention. Several parts of the home had received on going “wear and tear “from service users. Service users bedrooms were personalised, well decorated and equipped, as were the bathrooms and toilets. Service users spoken with were satisfied with their living environment. The kitchen and laundry areas were clean and suitably equipped. The home has satisfactory hygiene procedures in place. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 32,34, 35 and 36. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has robust recruitment procedures in place to safeguard service users. EVIDENCE: The home has a good percentage of staff who hold a NVQ level 2 or above. The home has registered with the Skills for Care Council national data set. The home operates with twenty-eight full or part time staff, this includes qualified nurses where nursing care is required. Staffing levels at the home were examined and remain satisfactory and meet the needs of the service users. There are currently no staff vacancies. The home has a robust recruitment procedure in place, which ensures that their staff are suitable to work with vulnerable people. Several staff personnel records were examined which confirmed that thorough employment checks were carried out. All new staff are required to provide two references, a full employment history, have Criminal Records Bureau clearance and complete a three-month probationary period. The staff personnel records were well presented and organised. The Annual Quality Assurance Assessment documentation confirmed that new recruitment procedures have been strengthened following the pervious key inspection. From discussions with staff and from examination of records it is clear that good training and development opportunities are provided. Details of staff training together with training planned were provided by way of the Annual Quality Assurance Assessment questionnaire. All staff have undertaken training on the Mental Capacity Act 2007 and the Registered Manager is seeking to register staff for training on Personal Centred Planning.. All new staff under go a twelve-week induction programme as set out by the Skills for Care Council. Records examined and discussions with the staff confirmed that the home has a formal structure for supervision of staff. Annual appraisals are undertaken. This judgement has been made using available evidence including a visit to this service. Systems are in place to ensure that service users’ have a voice and their views are listened to. EVIDENCE: The Registered Manager is a registered Nurse in Learning Disabilities and has relevant knowledge and experience in caring for adults with a mental disability and or challenging behaviour. She has demonstrated her commitment to the role and has obtained a recognised managers award. She has a relevant job description. The home has a policy and procedures for assessing its quality of care. The Regulation 26 visits by the Registered Provider to the home include details of consultation with service users and staff. Questionnaires are given out to both staff and service users as part of the home’s quality assurance procedures. The Registered Manager reviews the home’s aims and objectives and compiles an annual report for the Registered Provider. The Registered Manager also attends residents’ meetings as a way of listening to services users views and opinions. The home has a suggestion box and informal coffee mornings are held for service users, their families and involved professionals. The information on the home’s health and safety procedures and maintenance was provided by way of the Annual Quality Assurance Assessment questionnaire. This confirmed that all the relevant checks had been undertaken.
