Parklands is a small privately owned care home, registered to provide nursing care and accommodation for thirty older people. The home is situated in the Woolstone area of 0 6 0 5 2 0 0 8 Milton Keynes. It is a short journey to the city centre where a varied range of amenities, which are accessible to those with disabilities, can be found. Parklands has twenty-two single bedrooms and four shared rooms. All rooms were being used as single rooms at the time of the inspection. Some rooms have ensuite shower rooms and the rest ensuite bathrooms, which may not be accessible by those with disabilities. The lounge/dining room has four sets of French doors, which give immediate access to the patio area and grounds beyond. Extensive well-tended gardens surround the home.

What the care home does well As this was not a full key inspection this aspect of care was not assessed at this visit but will be assessed at the next key inspection. What the care home could do better: The proprietor must ensure that a manager applies for registration with us and that the lead nurse has a clear job description which describes her role and responsibilities, if the manager of the home is not be a registered nurse. Regular quality monitoring visits to the home must be made and the reports of these visits should be available for inspection. These should be undertaken by a competent person and there must be evidence that the proprietor is told of the outcome and that action to address deficiencies is undertaken. These visits should include a review of the complaints records to ensure that these are dealt with in a timely way. The views of residents and their families should also be sought. Guidance on undertaking these visits is available on our website www.csci.org.uk. The Commission for Social Care Inspection must be notified of adverse events in the home in a timely way. Information about making these notifications is also available on our website. The upgrades to the shower rooms must be completed. The proprietor must ensure that if a room is advertised as having ensuite facilities and higher fees are charged for that room, that the facilities are in safe working order and that the resident can access them. The proprietor must notify us formally of any change in the use of the building which is registered as a care home. The approval of the local fire and safety authority must be sought to ensure that lack of access to parts of the building do not compromise residents` safety should there be a fire. Care Homes for Older People Page 3 of 9 What we found: The home was last inspected on 6th May 2008. The registered manager for the home resigned in November 2009. We have received a concern about the provision of bathing facilities for residents and have received information that a business was being run from the upstairs offices of the home, about which we had not been notified. The purpose of this random inspection was to confirm the management and registration arrangements, ensure that the bathing facilities for residents were satisfactory and to ascertain the nature of any business being run from the homes registered premises. The wife of the proprietor is currently managing the home. She has appointed a lead nurse to advise her on the care of residents and to lead on the personal and health care of residents. She said that she would be applying for registration with us (The Commission for Social Care Inspection), but has not yet done so. The lead nurse does not as yet have a job description which describes the role of lead nurse and also her role in advising the manager on matters relating to the nursing care of residents. The home is expected to advise us of adverse events in the home (notifications). It was noted that we had not received any notifications since November 2008. The manager confirmed that there had been no deaths in the home. She said that she had one notification of a serious nature which she believed had been faxed to us but did not have the fax cover note. She agreed to send it to us with the outcome of the investigation which was currently being undertaken and to ensure that all future notifications were sent sent to us in a timely way. The responsible individual is expected to ensure that regular, at least monthly, quality monitoring visits are undertaken to ensure that residents are happy with the standard of care that they receive. These had been undertaken by the current manager until November 2008. The manager said that there were plans for the proprietors daughter to undertake these in future. The manager said that she had not reviewed the complaints records as part of the visits that she had undertaken and the format did not indicate that residents or their families had been spoken to on these visits. A tour of the building was undertaken. A new boiler has been installed which has the capacity to run the heating and hot water systems. It is currently running the heating system. The old boiler is running the hot water system through the original underfloor pipes. Some of the ensuite bathrooms, which are not accessible for residents who have disabilities and cannot get into a bath, have been converted into shower rooms but not all. The manager said that this was being held up by the fact that new pipework running through the loft spaces would have to be installed. She said that this should be completed during the next 2 weeks but said that realistically she expected this to be complete by the end of March 2009. The shower room for the person who had raised the concerns was being installed on the day of our unannounced visit. The manager said that she expected it to be complete in two weeks and that an electric shower would be installed if the new pipework was not completed in time. The manager confirmed that a staff agency was being provided from the upstairs offices of the home. This was a new initiative and to date the staff agency was supplying technical and management staff. The manager said that there were plans to Care Homes for Older People Page 4 of 9 provide care staff but that this aspect of the business had not been developed as yet. We were unable to access the office to verify this, as the staff were not working that day. A separate office door has been installed from the street and agency staff do not have to enter the main part of the home or any area used by service users to access their offices. The agency offices do however remain a part of the homes fire safety system and any changes in the use of the building must be agreed with the local fire safety officer to ensure that residents safety is not compromised by lack of access to parts of the building.
