What the care home does well People are encouraged to take control over their daily lives and receive help and support in a personal and flexible way. Complaints and concerns were listened to and appropriately acted on. People were safeguarded by policies and procedures, staff knowledge and training in the protection of vulnerable adults from abuse. Identified risk to people had been assessed and managed well. The home provided a domestic, homely environment that was flexible to meet people`s individual needs. The manager was committed to improving standards delivered by the service and could clearly describe the changes she intended to make. These service users will be accommodated in Number 10 Premier Street. A maximum of 12 service users will fall within the category of MD, mental disorder, excluding learning disability or dementia. Minimum staffing levels as specified in the Residential Forum Guidance in Care Homes for Younger Adults shall be maintained. No more than 15 service users will be accommodated at any one time. On admission, services users will be adult ie aged between 18 and 65 years. Date of last inspection Brief description of the care home Premier Homes is comprised of five separate houses located in Premier Street, Old Trafford. The office for the service is located at 8 Premier St. The home is registered to provide care and accommodation to fifteen residents with mental health needs. Three residents live in each house and all the residents have a single room. Each house is linked to a central fire and security alarm system. The home is situated close to pubs, shops, the local Metro Link tram network and bus routes. Good relationships exist between the residents and their neighbours. The range of fees charged at the home is from £333.87 to £417.18 Care Homes for Adults (18-65 years) Page 3 of 9 What we found: This report is based on information gathered by the Care Quality Commission (CQC) since the last inspection visit on 11 September 2007 and supporting information received in the Annual Quality Assurance Assessment (AQAA) submitted prior to this visit. The AQAA is a self assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. This random visit was unannounced, which means that the manager and staff were not told that we would be visiting. The visit took place on Tuesday 1 June 2010. During this visit we looked at personal and healthcare support and concerns and complaints. We also looked to see if the requirements made at the last inspection visit had been met. This report is an overview of what we found during the visit. References to we or us in this report represent the CQC. As part of the visit we spent time examining relevant documents and files. We also spent time talking with some people living at the service, several members of staff which included the manager, the deputy manager and the Responsible Individual (RI) of the service. We also walked round three of the houses and looked at the communal areas. Since the last inspection visit a new manager and deputy manager have been appointed. They are in the process of reviewing everybodys care plan and implementing new documentation. The aim is to improve the documentation around peoples individual needs, wishes and personal preferences. The manager and deputy manager are in the process of completing a pen portrait for each person living at Premier Homes. The pen portrait is information gathered from the person about what is important to them. In addition to this a my life plan is also being completed. This contains detailed information about peoples likes, dislikes, personal preferences and wishes around all aspects of their daily lives. This information is then incorporated into the persons personal plan of care. This helps ensure that people are supported in an individualised way that meets their individual needs and personal preferences. People spoken to during this visit confirmed that daily routines are flexible and staff respect each persons right to decide how they want to spend their time. The AQAA identified and the manager confirmed that support is provided for people to access a full range of healthcare professionals as and when needed. For example one person regularly attended the hospital for blood tests. Everybody was registered with their own GP and people saw the chiropodist or diabetic nurse when needed. The manager and deputy manager said it was their intention to implement a new recording tool for healthcare appointments so that the information would be easier to access. A requirement was made in the last inspection report that to ensure risks to people are minimised, risk assessments and risk management plans must be consistently undertaken according to peoples needs and must be reviewed on a regular basis. Evidence was seen that this requirement had been met. In the care files looked at there Care Homes for Adults (18-65 years) Page 4 of 9 were appropriate risk assessments that had been incorporated into the care plan and they had been regularly reviewed. The manager and the RI both confirmed that they had recently changed to a new medication system. The system is called Bio Dose and all medication, including liquid medication comes ready dispensed in pre packed pots. The only medication that will not be in the pot will be PRN medication. We saw that medication was stored securely. Medication had been signed into the home and there were no gaps in the recording of medication given to people. It was noted that some of the medication administration records (MARs) had been signed as S. This letter was noted included in the key and there was no written explanation as to what S stood for. The RI said S was signed by staff when a person was administering their own medication. This was discussed with the RI and the manager who made assurances that S would be added to the key or a written explanation would be provided on the MAR. This would ensure that the records were clear and easily understood. Those people, who had been assessed as able to do so, administered their own medication. We saw risk assessments and then a care plan was implemented accordingly. We saw that a weekly table count is undertaken to ensure people are receiving medication as prescribed by the GP. We were informed that the GPs original prescription comes to the home for checking before it is sent to the pharmacy for dispensing. This ensures that medication received from the pharmacy can be checked against what the GP prescribed. Medication to be returned to pharmacy was recorded in a return book and held securely. We were informed that all staff have received medication training to ensure they are competent in administering medication. The complaint procedure was on display in the hallway of every house. People spoken to said that they knew how to make a complaint and thought that any complaints made would be properly dealt with. The AQAA identified that complaints are monitored by the in house quality assurance officer at least quarterly. There was a complaint file which contained details of one concern raised by two people. It had been appropriately dealt with. The manager said that they talk daily to people living at the home and are in regular contact with peoples family. We were told that people are encouraged to raise any concerns or complaints they may have. The management team also make themselves available if anybody wants to talk to them. In addition to the informal chats we were told that the intention is key workers, service users and anybody the service users wishes to invite will have regular meetings to discuss all aspects of their daily lives and this will include any concerns or complaints. Policies and procedures were in place to protect people from abuse and we were told that all staff had received appropriate safeguarding adults training. The manager was able to Care Homes for Adults (18-65 years) Page 5 of 9 clearly describe the actions to be taken in the event of an allegation of abuse being made. There was a requirement in the last inspection report that the deployment of staff at the home must be reviewed in order that peoples needs are consistently met. This requirement had been met.
