Rock House is registered to care for 41 older people with dementia. It is situated in it`s own extensive grounds, within easy reach of Tickhill and Doncaster. The original part of the building overlooks the main road into Tickhill, and is in character with the locality. There are a number of lounge/dining areas that accommodates all people comfortably. A staircase leads to the first floor bedrooms. A chair lift provides access to this area. A single storey extension has been added to the main building, and has a high specification providing the people with single room accommodation. The bathrooms are of extremely high standard; the facilities include walk in/wheel in shower, and baths with hoists. All bedrooms are tastefully decorated and people have personalised them to their own tastes. The fees at Rock House at the time of the inspection were £390 to £426. These fee charges only applied at the time of inspection, more up to date information may be obtained from the home.

What the care home does well Health care needs are identified and met by suitably qualified staff. A District Nurse said, "The staff are very helpful when we visit and knowledgeable about people needs". The environment was well maintained and an excellent standard of cleanliness was observed for the people who lived there. One relative told us "The home is always extremely clean whenever I visit and my relatives room is always tidy and well presented". What has improved since the last inspection? The manager had worked very hard on improving the care provided to ensure the health care needs of the people are met. A new staffing structure was in place with a full time deputy in post to support the manager and ensure people`s needs were met.The maintenance and renewal programme had continued and many rooms had been redecorated and new furniture purchased. This ensued a high standard was continually maintained for the people living in Rock House. A training programme had been implemented and all staff had received mandatory training to ensure people`s needs were met. An activities co-ordinator is now in post and people told us that she is very good and gradually introducing different activities both group and 1 to 1 sessions, this ensured peoples social needs were being met. What the care home could do better: There is a stair lift used to access the upstairs rooms the provider is looking to provide a vertical passenger lift, which would further improve the access to the upstairs for the people. The manager had received a complaint since the last visit, this was dealt with following the procedure however, the manger had learnt from this process the need to provide more detail in the response to complainants to ensure all the concerns raised are responded to. This could have prevented the complaint being taken to The Commission for Social Care Inspection. The quality rating for this service is 3 star. This means that the people who use this service experience excellent quality outcomes. This was an unannounced visit, which took place on the 21st April 2008 it commenced at 09:00 and finished at 15:15 hours. The visit included talking with people living at the home, a number of relatives, visiting professionals, the manager and four staff. A walk around the building to gain an overview of the facilities and we checked a number of records. Some surveys forms were sent to people who live at the home, their relatives and professionals. At the time of this visit five were completed and returned to the Commission. The comments received were very positive. The manager completed an annual quality assurance assessment (AQAA) and returned this prior to the visit this focuses on how well outcomes are being met for the people using the service. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. We have made this judgement using a range of evidence, including a visit to this service. People in the home had their needs assessed prior to moving into the home and each was provided with a written contract and statement of terms and conditions to ensure these could be met. EVIDENCE: Suitably qualified staff carried out the pre admission assessments on people who wanted to move into the home. The assessments were very detailed with all peoples needs identified, ensuring that the home could meet their needs before a place was offered to them, Every person in the home had a contract, and terms and conditions issued at the time of moving in. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 & 10. People who use the service experience excellent outcomes in this area. We have made this judgement using a range of evidence, including a visit to this service. Each person in the home had a plan of care with clearly identified needs, these were met, and people were treated with respect. Medication procedures protected people. EVIDENCE: Three people in the home were case tracked; this meant their plans of care were looked at in detail. The plans had identified the needs of the people with good recordings of the measures to take to meet their needs. The plans were regularly reviewed; people and their relatives were involved in this process. This ensured their views were listened to and their needs met. Relatives said, “I am aware of the plans and have been involved in changes and reviews”. All health care needs were met and regular input from health care professionals was obtained. Their advice was followed and well documented in the plans ensuring the wellbeing of the people who lived in the home. We spoke to a district nurse who said, “The care provided in this home by the staff is excellent, the staff communicate to us if there are any problems and ask for advice there is a good working partnership”. The manager had liaised with the district nursing team to improve care delivery and it had been agreed that the district nurses visited each Monday and Thursday to check on all people and then visited other days on request. This system has improved the care provided to ensure people’s needs were met. People were treated with respect and privacy and dignity upheld. During the visit we observed staff interacting very well with people and their relatives. One relative told us, “The staff are lovely they look after people very well, I am very happy with the care my relative receives”. One person told us, “The staff are very kind and look after me very well”. Medication policies and procedures safeguarded the people. Medication was well documented on receipt, administration and return. All staff who administer medication regularly receive medication training and undergo competency assessments to ensue people are protected. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 & 15. People who use the service experience good outcomes in this area. We have made this judgement using a range of evidence, including a visit to this service. People exercised choice and control over their daily lives and activities ensuring their needs were met. EVIDENCE: An activities co-ordinator has been in post since January 2008. There is 15 hours to dedicated activities, however the co-ordinator works full time so will also do 1 to 1 sessions at other times to meet peoples needs. Activities were varied and changed depending on the choices of the people who attended on the day. The manager told us that outings were being considered. People were being asked what they would like to do and this would hopefully be implemented in the summer to meet people’s needs. Rock House Residential Home DS0000069114.V364155.R01.S.doc Version 5.2 Page 13 The people and their relatives we spoke to all said that contact with family, friends and the local community were encouraged by the staff and relatives were always made welcome. A varied, balanced and wholesome diet was provided for the people with good choices available. We observed a meal, which was well presented, there was good communication to the people from staff when it was served, seconds were offered, people were asked if they were finished before plates were taken and a choice of drinks were also offered. This ensured peoples needs and choices were met. One person told us, “The meals are cooked on the premises to a very high standard”. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18. People who use the service experience good outcomes in this area. We have made this judgement using a range of evidence, including a visit to this service. People who lived in the home were listened to and protected. EVIDENCE: There was a comprehensive complaints procedure, which was clearly displayed, in the entrance hall. All people we spoke to were aware of the procedure and told us they would either speak directly with the manager or staff. The manager had received two concerns, which were both regarding one incident. The second concern was raised through the Commission for Social Care Inspection. The provider had fully investigated the concerns however the response to the complainant was brief and did not show the full outcome. If more detail had been incorporated it may not have resulted in the concern to the Commission for Social Care Inspection. Good records were kept of the investigations and outcomes. This showed they had been fully investigated, acted on and taken seriously. All staff had received training in adult protection; all staff we spoke to had a good knowledge of the procedures and what to do should an incident occur. Staff were also aware of the whistle blowing policy, which safeguards people in the home. People who use the service experience excellent outcomes in this area. We have made this judgement using a range of evidence, including a visit to this service. The home was well maintained clean, pleasant and comfortable ensuring people lived in a safe environment. EVIDENCE: The environment was maintained to an excellent standard. It was homely and well decorated. The standard of cleanliness observed during the visit was excellent. There was a maintenance and renewal programme, many rooms had been redecorated since the last visit and new furniture purchased. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 & 30. People who use the service experience good outcomes in this area. We have made this judgement using a range of evidence, including a visit to this service. Staff were appropriately trained, the recruitment procedures were robust ensuring people were in safe hands at all times, had their needs met and were protected. EVIDENCE: People’s needs were met by the staff on duty and the skill mix was appropriate to the assessed needs of people. A new deputy was in post, which provided a good management structure to ensure people’s needs were met. The manager had registered with skills for care and most staff were doing an NVQ qualification either level 2 or 3 ensuring people are in safe hands at all times. Mandatory training was up to date and there was clear evidence that this was maintained and refresher courses completed within timescales. The manager had worked very hard on the training of staff over the last 12 months and all new staff attended training during induction, which covered 7 mandatory training units to ensure staff were appropriately trained to meet peoples needs. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 & 38. People who use the service experience excellent outcomes in this area. We have made this judgement using a range of evidence, including a visit to this service. Management and administration safeguards people, good health and safety policies and procedures were in place ensuring the safety of people in the home. EVIDENCE: The manager was qualified and experienced to run the home. She had achieved the registered managers award and continually kept herself updated to ensure the homes stated purpose, aims and objectives were met. A relative told us, “The staff all work well together and the people are well looked after”. Staff told us, “The manager is very good, it is a good place to work and there is good communication between management and staff, which ensures we look after the residents in our care to the very highest standards”. Quality monitoring was carried out; the manager did regular audits and had regularly sent quality-monitoring questionnaires to people in the home and their relatives. This ensured the home was run in the best interests of the people who lived there. The home managed some people’s finances and personal money. The manager told us these were managed by the homes administrator they were well recorded and all receipts were kept, ensuring peoples financial interests were safeguarded. The home had a comprehensive health and safety policy. We were able to evidence that regular maintenance of equipment and systems was carried out. Risk assessments were carried out on all safe-working practices, regular audits were carried out on the building and all accidents were properly recorded and reported ensuring people in the home were safeguarded.
