What the care home does well People who live in the home are happy. Their comments included `it is a nice place to live and clean`; `my activities like cycling, pub, local walks and cafe all going very well`; `the support from the manager and staff is good`; and `I like the meals`. People benefit from a range of activities including swimming, going to the pub, skittles, day centres, work placements, beauty sessions, singing and discos. Since the previous key inspection people who live in the home have been on holiday to different destinations including Haven, Cornwall and cycling. The home is supporting people to develop their daily living skills through preparing food and cooking, cleaning, shopping, gardening and laundry. A number of documents within the home have now been produced in symbol format. This makes them more accessible for some people. Staff told us that they enjoy their work and feel well supported. Staff recruitment processes protect people from the risk of harm. What the care home could do better: The registered provider must ensure that medication is stored securely. This is to promote safekeeping of medicines and protect people from risk of harm. The registered provider must ensure that they inform us of any notifiable incidents that occur at the home. Sharpe House is a large detached property arranged over two floors. The layout of the home would not be suitable for service users with a physical disability. All bedrooms are single and have hand wash basins. Care Homes for Adults (18-65 years) Page 2 of 9 2 4 0 6 2 0 0 9 Brief description of the care home Some bedrooms are located on the ground floor. The home does not have a passenger lift to reach the first floor. The home has dining room, lounge, conservatory, and two bathrooms, domestic style kitchen, laundry room and well-presented garden. Sharpe House is registered with the Care Quality Commission to provide personal care for up to 10 service users, under the age of 65yrs, who have a learning disability. The home is not registered to provide nursing care. The home is owned by JEDD International Ltd. The Registered Provider is Mr Declan Howlett. The home currently has a manager designate who is applying to register as manager with the Commission. The homes current fee range is £360 to £855 per week. Additional charges include those for toiletries, hairdressing, transport some activities and chiropody. The home has a car and service users currently contribute 50 of their DLA mobility component towards the cost of transport. Care Homes for Adults (18-65 years) Page 3 of 9 What we found: This random inspection commenced at 10.20am and was carried out over a total of four hours. The home has met all of the requirements and recommendations made at the previous key inspection on 24th June 2009. On the day of the inspection, eight people were living in the home. Four people were attending day centres and a work placement, one person was going for health checks, another person had gone out for a walk, and a couple of people were at the home. The manager designate was available throughout the inspection and was supported by a senior support worker and a support worker. We sampled two care plans. These contained a good level of detail, including peoples preferences, and provided good information for staff to follow. The manager designate told us that care plans and risk assessments are still being developed. Since the previous inspection, peoples individual goals have been identified and reviews have been carried out at regular intervals to ensure that peoples changing needs continue to be met. Daily records are now being kept and contained detailed information. Health plans have been introduced and there was evidence that people are supported to access healthcare appointments and professionals. One person who lives at the home has Epilepsy. The home keep a record of any seizures and a detailed action plan is in place for staff to follow. We looked at medication and medication records. The home uses a monitored dosage system. Staff administer medication and have recently completed training updates. The Medication Administration Record Sheets were fully completed. Where records had been hand written, two staff signatures had been obtained to ensure that the entries were correct. Medicinal creams had been dated on opening and discarded appropriately. We reviewed the stock of controlled drugs and this was found to be correct. We found that the medication cupboard does not have a good quality lock and is not secure. We advised the manager to contact the pharmacy to order a suitable cabinet. The home keeps small amounts of money for some people. Monies are stored individually. We checked two peoples monies. The balances were found to be correct. Written records of transactions are kept. These are signed by two members of staff. The manager designate showed us around the home. We sampled three bedrooms, bathrooms, kitchen, laundry, lounge and dining room. One person who lives in the home showed us their bedroom. We observed that people have personalised their bedrooms to meet their individual tastes and interests. One person who had recently had a new bedroom carpet went to the shop to choose the colour. Carpets have been replaced in several bedrooms and corridors. Some rooms have been redecorated. The home has an ongoing maintenance and renewal programme for further redecoration and refurbishment. The provider confirmed that smoke seals have been fitted to five fire doors; there is a programme in place to replace smoke seals to the rest of the doors. People who live in the home are being supported to develop the garden and a vegetable patch. The home was clean and fresh throughout. Care Homes for Adults (18-65 years) Page 4 of 9 We looked at two staff files. These were well organised and contained all of the required information to protect people from risk of harm. Two new members of staff had commenced their Skills for Care Common Induction. They had completed training courses in safeguarding adults, infection control, food hygiene, first aid and one had commenced their LDQ (Learning Disability Qualification). They were booked to attend training in moving & handling and health & safety. We spoke with two members of staff who confirmed that they had completed training, received regular supervision and felt well supported. Four staff who completed surveys told us that they have received relevant and updated training. Since the last inspection the home has appointed a new manager who is in the process of registering with the Commission. One person who lives in the home told us I like the staff and new manager, they are listening to me and help if I need it. Staff told us the new manager has only been with us a matter of weeks but staff have noted a big and vast improvement within the home and we have a new manager who is knowledgeable, professional and experienced. When the previous registered manager left the home, we were not informed. The home has now obtained a copy of the guidance so that they know when to send in a notification to the Commission. Accidents are recorded in a book that complies with Data Protection. Completed records are stored in each individuals care plan.
