Stapleton House is a purpose built home providing both personal,nursing and dementia care for older people. It is a two storey building serviced by two passenger lifts. All bedrooms except one are en-suite, and there are lounges, one with an adjoining conservatory, and separate dining rooms. The corridors and doors are wide and allow access for people using wheelchairs. There is a 12-bedded dementia care unit situated on the first floor. This is a selfcontained unit with individual bedrooms, dining room and lounge. There are adapted bathrooms, showers and toilets close to all resident areas. The home is located in a quiet, discreet area with well-laid gardens to the rear overlooking a green belt area, which offers a picturesque view from some bedrooms and the communal areas. The home is close to the town centre of Jarrow, to all amenities, and public transport, including the Metro railway system. Car parking facilities are available.

What the care home does well Everyone was being cared for using appropriate equipment and the records are in place for the care being given. Staff were pleasant,organised and made sure all care was given discreetly and in private. The home was clean and had no offensive odours.Staff had sufficient equipment to prevent the spread of infection and were seen to follow good practice guidance. Staff said that they had completed mandatory training and there were no health and safety issues identified at the time of the visit. What the care home could do better: Staffing levels at night time must be reviewed to make sure people living in the home receive a high standard of care at all times. People living on the Dementia Care unit must not be left unsupervised at any time. Also, the specialist bath in the dementia care unit must be either repaired or replaced so that sufficient numbers of bathing facilities are available for those living in that unit. Care Homes for Older People Page 3 of 9 What we found: The random inspection was carried out to look at the areas identified in concerns raised with the Commission for Social Care Inspection(CSCI) about staffing levels overnight and lack of equipment to prevent infection. The visit was undertaken on the 3 February 2009 and started at 8:45 pm and finished at 10:30 pm. The qualified nurse and care staff assisted throughout the visit and the registered manager was telephoned on the 4 February 2009 so that feedback from the visit could be given. On arrival four staff were on duty including one first level registered nurse. A look around the home found many people in bed or staff were busy assisting them prepare to go to bed. There were five people sitting in the downstairs lounge.Four of them said they were waiting for the staff to come and help them. One person still had a visitor who said she regularly helps her relative at night time. There was a supper trolley containing remnants of jam and bread sandwiches and lots of uneaten cold toast . People had been offered tea but staff said they dont like milky drinks including hot chocolate and horlicks. The kitchen was accessible and food stuff available should people be hungry. Upstairs most people were in their rooms either waiting for assistance or asleep. Six people currently live on the upstairs separate Dementia Care Unit .Four people were in bed asleep and two people were walking up and down the corridor. Staff said they just potter and the others could get up at any time. People who are very vulnerable and can become distressed,disruptive and anxious very quickly were left without adequate support for some time. The staff said that this happens at this time of night and in the morning when people are asking for help to get ready for the day. Once everyone had settled one carer remained on this unit. As there was no senior carer on duty the qualified nurse was responsible to ensure everyone received their medication.This takes some time as there are two nursing floors and a separate Dementia Care Unit. The nurse was very organised and supervised and directed the staff to try and make sure each area was covered.However there were occasions when people could not have help as staff were busy in another part of the home. As one carer should remain on the Dementia Care unit, this left two carers and one Care Homes for Older People Page 4 of 9 nurse for the two nursing floors.Given that most people need two staff to help them with all of their care needs and staff need to have breaks during a twelve hour shift there are insufficient numbers of staff at busy times to meet peoples assessed needs and ensure they remain safe. All of the staff were pleasant,organised and knew peoples care needs very well.They made sure they carried out their work quietly and efficiently. The home was clean,generally tidy and had no offensive odours.The specialist bath on the Dementia Care unit has still not been repaired or replaced and people have to go out of the unit should they want a bath. There was plenty of equipment including gloves, aprons, barrier nursing equipment red bags, Hibiscrub, Hibisol,access to liquid soap and paper towels.The clinical waste was being disposed of appropriately and sluice disinfectors were in working order. Staff said there had been occasions when continence aids had not been available,however this was when people were being re assessed and this was now resolved. Staff said there are regular meetings and they had voiced concerns about staffing on night duty.They said because of the numbers of people living in the home four staff were enough and this had been the norm. The dependency and geography of the home has not been taken into account when staffing overnight. The manager was contacted the next morning to give feedback and she is aware of some of the staffs concerns. There were no health risks and staff confirmed that they had completed mandatory training.
