What the care home does well Care staff present as professional, well organised, and have a friendly, welcoming attitude that serves to sustain a relaxed atmosphere within Sycamore Lodge. People living at Sycamore Lodge appear well cared for in their personal appearance and their demeanour supports the conclusion that they feel safe and at ease. Individual bedrooms are personalised and the rooms and communal living areas were clean and homely in appearance. The Manager and care staff are knowledgeable about the needs of the people they support and each staff member is actively encouraged to pursue further training so that everyone living there benefits from a staff team that is able to do a good job. Care staff and the people living at Sycamore Lodge benefit fromhaving a Manager that presents as approachable, experienced, and conscientious. The staff team continue to be good at recognising when residents need help from other health care professionals to improve their health and well being. What has improved since the last inspection? The quality rating for this service is 2 stars. This normally means the people who use this service experience good quality outcomes. The focus of inspections undertaken by the Commission for Social Care Inspection (CSCI) is upon outcomes for service users and their views of the service provided. This inspection was conducted by as follows: This was an unannounced inspection that took place just after lunchtime and lasted just over four hours. Sycamore Lodge was inspected using the method of ‘case tracking’. This is a method that enables us to make judgements about the standard of care being provided to individuals living at the home. On this inspection, as well as touring the premises and viewing a sample of bedroom accommodation and communal living areas, records relating to the care being provided to four of the adults living at Sycamore Lodge were inspected. In addition, a sample of the records relating to the recruitment, supervision, and training of staff were inspected, and the opportunity was taken to meet in private with the Manager, Deputy Manager, and care staff to discuss the day-to-day running of the home and the support provided for the people in residence. The majority of adults living at Sycamore Lodge have profound difficulties with communication and comprehension. Therefore the judgements made in this report are based upon observations of staff going about their duties, feedback from the Manager and staff team, as well as the interactions observed at the time between members of staff and the adults they support and care for. A review of all the information received about Sycamore Lodge since the last inspection in 2007, as well as the findings of that inspection, was also considered when planning this latest inspection. There were no requirements made as a result of this inspection. The admission process is robust and provides assurances that the needs of residents entering Sycamore Lodge can be met. This judgement has been made using available evidence including a visit to this service. EVIDENCE: A significant of people living at Sycamore Lodge have been admitted from long stay hospital care and there is comprehensive pre-admission information on each person. There is detailed documentation regarding each person’s assessed needs and the support they require. Any decision to accept a new service user will be conditional upon a full assessment of need and there is evidence that the documents recording such information are in place. The care team present as knowledgeable about each person’s assessed needs and the way in which these needs have changed over time since admission to Sycamore Lodge. Residents assessed needs and personal goals are reflected in their individual plan and they are assisted to live full and active lives. This judgement has been made using available evidence including a visit to this service. EVIDENCE: There is documentary evidence that the changing needs of the service users are accurately reflected in the records relating to the level of support being provided. Each person, according to their abilities, is encouraged to make choices that affect their daily life at Sycamore Lodge. There is evidence of risk management that enables more capable people to retain a level of independence. One person regularly takes a walk beyond the grounds of Sycamore Lodge and this is a routine that gives this person pleasure and the freedom to enjoy this independent activity. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12, 13, 15, 16 and 17 Quality in this outcome area is good Residents are able to take part in appropriate activities, some of which involve them in the local community. Residents are offered a healthy diet, which they enjoy. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Visitors are made very welcome at Sycamore Lodge. All care plans inspected provided information on how each person spends their time. Activities vary considerably and are dependent upon each individual’s preferences and ability to participate. As noted in the previous inspection report holidays are arranged for people, with members of the care team accompanying them to provide continuity of support. Care plans also identify that there are trips arranged to local pubs for meals. The minibus is also used to take people out into the community and this was being used on the day of inspection. There is a varied diet and the consensus is that people enjoy their meals. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 18,19,20 Quality in this outcome area is good. Personal support is flexible and staff respond appropriately to the changing needs of each service user. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Each plan of care is regularly reviewed and there is documentary evidence of this. There is a health plan in place for each service user and arrangements are made for regular healthcare check-ups. Routines for bathing, retiring to bed, and other personal activities are flexible. Each person is encouraged and enabled to look after their personal appearance as much as they can and choose what they like to wear. People were observed to be comfortable with the care team and there was relaxed manner in the way each member of staff went about their duties. Care plans and diary notes show that each person has regular access to healthcare services including dentists, opticians and chiropodists. Staff again confirmed that residents were supported to access these services ensuring that their health needs are met. There was documentary evidence of appointments having been made for these professional services. The care team receive training from the Pharmacist in the safe handling and administration of medicines. There is documentary evidence and verbal confirmation from members of the care team that this training is provided. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 22,23 Quality in this outcome area is good. The Manager ensures that there is the appropriate training and guidance for staff that enables them to help protect service users from the risk of harm or abuse of their rights. This judgement has been made using available evidence including a visit to this service. EVIDENCE: There are policies and procedures in place governing the protection of vulnerable adults and each staff member is required to adhere to these. All members of staff have completed their Protection of Vulnerable Adults (POVA) training. Recruitment of staff is governed by robust procedures to ensure that all appropriate precautionary checks are conducted prior to them working with service users. There is documentary evidence that Criminal Record Bureau clearances have been taken up for each member of staff. The care team present as sensitive to the body language and demeanour of people with little or no communication skills. They are able to judge if someone is unhappy or in discomfort by changes in their behaviour even if that person is unable to articulate how they feel. Sycamore Lodge is a comfortable, clean, and well-appointed house that is homely and welcoming. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Sycamore Lodge is situated in a residential street in Nottingham. The interior of the house is furnished to a good standard throughout and is appropriately lit and ventilated. Rooms are kept at a comfortable temperature. All communal areas were seen to be clean and in a good state of repair. There were no unpleasant odours in the house. Bedrooms are personalised with each service user’s belongings and personal preferences. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 32,34,35 Quality in this outcome area is good. Staff appear competent, motivated, and respect the service users they support. This judgement has been made using available evidence including a visit to this service. EVIDENCE: It was evident from observation that the care team take an interest in each individual and are committed to providing them with the support they need. Staff appear to know the likes and dislikes of each person and to manage the manner in which they work with that person accordingly. The Manager ensures each member of staff regularly receives one-to-one supervision. Each staff member’s training needs and work performance is being regularly reviewed and monitored. There is documentary evidence that appropriate records are kept of these supervisions. Sycamore Lodge employs male and female staff and they are of varying ages and different ethnic and cultural backgrounds. This contributes towards ensuring that different cultural and social needs can be supported. Staff training records show that regular training continues to be provided. This includes Learning Disability Award Framework training and National Vocational Qualification (NVQ) to a minimum of level 2. She presents as enthusiastic and committed to her role. Sycamore Lodge is efficiently and sensitively managed. There is a good standard of record keeping, with information being accurate, pertinent to each person’s care needs, and readily accessible to the care team.
