What the care home does well People are cared for in a clean and comfortable environment and the home is decorated to a good standard. All the people we spoke with made positive comments about the care they received and the positive relationship they felt they had with staff. Observations indicated that there was enough staff on duty to support people who required help, and that staff interacted with people in a positive way, being mindful of maintaining their independence, privacy and dignity. The management and staff work well as a team and they receive regular support and a varied training programme that helps them to meet peoples needs to a high standard. New staff are recruited using a robust system that makes sure that people are suitable to work with vulnerable people. They also receive a comprehensive induction to the home which prepares them well for carrying out their job. People are encouraged to make choices about their daily lifestyles and individual likes and dislikes are catered for. The home has a good system in place to enable people to express their views and makes sure that the home is operating safely. What the care home could do better: The home continues to operate at a high standard and no requirements or recommendations were made as part of this inspection. Date of last inspection Care Homes for Older People Page 2 of 9 Brief description of the care home The Cedars is a care home run by Lincolnshire County Council. It is a purpose built two-storey home that offers care and accommodation for older persons on a permanent and respite basis. It is also registered for the short term care of people who have dementia. The home is situated to the north of the town, in a residential area off the main road. There are local facilities available and the town of Gainsborough is approximately a mile away. It is set in large, well maintained grounds, with gardens to the front and rear. There is also a safe garden for the use of residents who have dementia, which is also private. Ample parking is available to the front and side of the property. The home operates on policies and procedures established by Lincolnshire County Council Social Services Department. There is a day centre attached to the home, but it is not regulated by the Care Quality Commission. At the time of the inspection the manager confirmed that fee rate ranged from £363.90 to £445.66 per week, depending on peoples assessed needs. Hairdressing and private chiropody are not included in the fees. Information about these costs as well as the day-to-day operation of the home, including a copy of the last inspection report, is available at the home. Care Homes for Older People Page 3 of 9 What we found: This was an unannounced random inspection which included a visit to the home. This type of inspection is carried out to monitor how the home is operating. The visit lasted two and a half hours and we also took into account previous information held by us. This included the previous key inspection and annual service review reports, as well as any information we have received since they took place, such as notifications about any incidents at the home. We took a partial tour of the building during which we spoke with some residents and staff. We looked at various documentation including, care plans, risk assessments and staff training records. We also assessed how people had been consulted about the running of the home and the quality of the service being provided. The manager was not available, but we spoke to the two team leaders to learn about any changes which have taken place and if there were any risks to the people living in the home. Prior to visiting the home we sent out surveys to residents and staff to ask their opinion of how the home was operating, twelve people returned these and their opinions are included in this report. When we walked round the home the rooms we saw were personalised and homely. The people we spoke to said that they were very happy staying at the home. They commented positively about the meals saying they are very good and the food is wonderful. Other comments included, I love staying here and I dont want to go home, its lovely here. There is a full time activities coordinator who provides a varied selection of stimulation that is designed to meet the needs of the people who are staying at the home. One of the small lounges has been changed into an activities room, on the day of the visit some people were doing jigsaws and others were getting ready to do craft work. In the dementia unit one resident told us that she regularly went out shopping with staff and we discussed the vegetables and flowers growing out in the garden. Staff said that residents were encouraged to help in the garden and the produce was used in the kitchen. There is also a small kitchenette which is used by people rehabilitating to practice their everyday skills, such as baking and making drinks. The surveys we received confirmed that people knew how to make a complaint and that they know who to speak to if they are unhappy. People told us that they had no complaints, they praised the staff and said that they delivered support in a friendly but professional manner. Care records showed that staff had assessed peoples needs and completed care plans which identified how to meet their individual needs, as well as how any potential risks would be minimised. A robust recruitment system had been followed to make sure that staff were suitable to work with vulnerable people. This included obtaining written references and undertaking a criminal record bureau check (CRB) and a formal interview. Staff confirmed that they had undertaken training including; induction, fire training, first aid, moving and handling, infection control, the safe administration of medications and dementia training. Care Homes for Older People Page 4 of 9 Returned surveys indicated that people received the care and support they needed and that staff were available when they required assistance. People also confirmed that staff always listened to them and acted on what they said to them. One person told us, the girls are first class. People told us that the home was well managed. When we asked them what the home did well residents comments included, everything, I have a comfortable room, a TV, good food and people are available when I need them, food is very good, company, people to talk to, helpful staff, attention when needed and warm and friendly, I like it here. No one made any negative comments either on the day of the visit or in the returned surveys. Staff also confirmed that the home was operating well and that they were supported to do their jobs. One person told us that it was an excellent place to work. Another said, the care team are very good at motivating people to reach their full potential in a caring and positive way. When asked if anything could be improved most staff did not comment. However one person thought that more outings would be beneficial for residents. Others were concerned about the possibility of the County Council closing the home, this was also commented about by some service users. There is an effective quality assurance system in place to consult people about how the home is operating and check that policies and procedures are being followed. This includes questionnaires, meetings and audits of the building and working practices.
