What the care home does well Throughout the inspection there was a relaxed and friendly atmosphere in the home with lots of interaction between people living and working at the home.The main meal of the day was observed. There was a choice of main meal and people were shown both meals to assist them to make their choice. People who required physical support with their food were assisted in an unhurried and dignified manner. As part of the homes quality assurance system they send out satisfaction surveys to relatives of people living at the home. Returned surveys seen showed a high level of satisfaction and some people had written very positive comments about the home and the staff. Staff spoken with felt that they received good training and said that there were opportunities to share ideas and make suggestions about the running of the home. Staff said that the management of the home was approachable and they were aware of the ability to take serious concerns outside the home. What the care home could do better: There is evidence that the home has taken all aspects of the complaint seriously and have put additional training in place where shortfalls were identified. No requirements have been made as a result of this inspection. The manager demonstrated a commitment to ongoing improvement and said that they welcomed the views of all interested parties to ensure that a good standard of care was provided to people living at the home. Accommodation is provided over two floors. All bedrooms are single occupancy. A passenger lift, assisted bathroom and call system are provided. There is an accessible garden at the rear of the property. The home is registered with the Care Quality Commission, to provide personal care for up to 23 people who have a dementia. The Registered Manager is Jill Camm and the Registered Provider is Maners Care Ltd. The Rectory has been approved by Somerset Social Services as a special rate care (specialist residential care) home providing enhanced care for people suffering from Care Homes for Older People Page 2 of 8 2 7 0 8 2 0 0 9 Brief description of the care home dementia. The current fee rate is £486.36 per week, this does not include hairdressing, chiropody and toiletries. Care Homes for Older People Page 3 of 8 What we found: In January 2010 concerns about the home were raised with the Care Quality Commission and the Local Authority about some care practices at the home. As a result of this the Local Authority held a safeguarding meeting on the 11th February 2010. This short random inspection was to follow up on issues raised in the complaint and discussed at the meeting. The home has looked into the concerns raised and clear documentation is available in the home. They have co-operated fully with the safeguarding procedures and the manager stated, during this inspection, that the concerns raised had been a wake up call to review their procedures. The complainant raised concerns about the moving and handling practices at the home. In response the the complaint the home acknowledged that, whilst they do not condone bad manual handling practices, there may have been times when under pressure that staff failed to follow manual handling guidelines. As a result all staff have now received up dated training in moving and handling. Staff who have undertaken a National Vocational Qualification in care at level 3 have completed the module moving and positioning individuals others have received up to date in house training. Staff spoken with during the inspection confirmed that they had received this training. The complainant stated at the safeguarding meeting that since the complaint they had noticed a conscious effort by staff to improve. Concerns were raised that a relative had witnessed a staff member forcing food into a service users mouth. This referred to a person living at the home who at times requires full physical support to eat meals. There is a care plan in place for assisting this person with meals which gives clear instructions for staff to follow. The minutes of the safeguarding meeting acknowledged that because of the way the person needed to be assisted it could look, by someone unfamiliar with the persons specific needs, as though they were being force fed. At the time of this inspection we observed two people being assisted to eat their main meal. Staff sat with the people and assisted in an unhurried and dignified manner. Other concerns raised were that the floor was sticky and that the window in a bedroom was not restricted. At the time of the inspection all areas seen were clean and fresh. It was observed that when liquid was spilt on the floor it was quickly washed and an appropriate sign was put in place to warn people that the floor was wet. The manager gave assurances that all upstairs windows are restricted to minimise the risks of people falling from them.
