What the care home does well The health, personal and social care needs of individuals are set out in their individual care plans and are generally being met. A range of policies and procedures are in place and systems are in place to ensure these are implemented. The expert states the residents who are most articulate in this home seem to be generally satisfied with it and were complimentary about the staff, the care and the surroundings. The service supports people to continue practising their religious faith. What the care home could do better: The home must be vigilant to ensure that monitoring charts and risk measuring assessments are accurately taken for all individuals according to their needs at the time of assessment. The manager must ensure that the home`s safeguarding policy is properly adhered to. All staff are aware of the policy and if necessary arrange retraining of staff in safeguarding practice. Daily records could improve further providing more detail of care given that reflects actions identified in individual care plans. Systems of auditing could be improved to identify shortfalls in practice and improvements necessary to ensure that nurses and carers perform according to the homes` procedures. Care Homes for Older People Page 3 of 8 What we found: This was a random unannounced inspection conducted over one day on 25th February 2009 and undertaken by two inspectors. Following a safeguarding adults alert made in January by a visiting nurse to the home who reported her concerns about the care of one individual, the inspection was targetted to examine concerns raised in the alert, focusing on care practises and procedures to ensure the ongoing safety and welfare of people living in the home. During this inspection we sampled six files of people identified as being vulnerable or having a high level of nursing needs, including the file of the individual about whom the alert was made. We examined assessments, care plans, risk assessments and monitoring charts found in these files and additionally looked at daily records of individual activities, minutes of staff meetings, records of accidents, incidents and their notifications to CSCI. We were assisted by the registered manager and her staff throughout the day. We also examined compliance with previous requirements as their timescales had expired and were relevant to the issues we examined during this inspection. Ensure that risks identified in individual assessments accurately reflect the risks identified to individuals. 17/06/2009 2 8 13 The manager must ensure 17/06/2009 that the homes safeguarding policy is properly adhered to. She must ensure that all staff are aware of the policy and if necessary arrange retraining of staff in safeguarding practice. All individuals must be safeguarded from abuse or inappropriate practice at all times. Care Homes for Older People Page 6 of 8 Recommendations These recommendations are taken from the best practice described in the National Minimum Standards and the registered person(s) should consider them as a way of improving their service. No Refer to Standard Good Practice Recommendations 1 7 Care plans for individuals with pressure sores should record alternative aids that may be identified and used to assist and support them outside of their rooms without which individuals may need to be nursed on a mattress. It is recommended that better records are kept of any action taken in response to changes in residents individual needs. Ensure that all residents are weighed at least monthly or more frequently if necessary. Frequency of weighing must be made clear in individual care plans if there are any variations to the frequency stated in the homes policies and procedures. The home to provide evidence of individual weight measurements. Robust systems of auditing are needed to identify shortfalls in adherence to the homes procedures and to ensure that both nurses and carers undertake appropriate tasks accordingly. Daily records could improve further to provide more detail about individual needs or actions taken with individuals that better reflects and relates back to information provided in care plans.
