Whitehaven Lodge is a large purpose built residential home providing care and support to 55 elderly persons, who have dementia relating to their old age. The home is located in Millbrook, the west of Southampton, approximately 2 miles from the city centre and is managed by Southampton City Council. The building is laid out over two floors with a passenger lift access to the first floor. Bedrooms are arranged in 6 separate living units. On the ground floor there is one large dining room and four smaller lounges, one of these is a smoking lounge. The home has a pleasant garden which incorporates a raised sensory flowerbed and a lawn. Garden furniture and a handrail enable residents to enjoy the garden to the full, particularly during the warmer months. There is an extra charge for items such as chiropody and hairdressing that are not included in the fee.

What the care home does well There is a good pre-admission process in place that staff followed to ensure that the home is able to meet people`s needs prior to admission. The service users said that they are well supported and the staff are very good. The service users told us that the meals at the home were good and choices were available to them. There is registered manager with responsibility for the day-to-day management of the service is supported by other staff. What the care home could do better: There are no requirements from this visit. Continued effort and support should be sustained in placing people into new accommodation. Care Homes for Older People Page 3 of 9 What we found: An unannounced inspection visit was undertaken over one day on 18 May 2010 as part of our monitoring process and this visit lasted four and a half hours. We sent out the Annual Quality Assurance Assessment (AQAA ) to the registered person at the service. This is one of the means of collecting data from the service. We also sent out service users and staff surveys to the home to seek their views of the service provision. During the visit we spoke to a number of the service users and the staff. We looked at the environment, the service users records and care plans and some staff records as maintained at the service and we observed care practices. The inspection took into account the homes (AQAA), which arrived when we asked for it and was completed satisfactorily. The AQAA is a self assessment tool that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. We have looked at any other information that we have received since the last inspection. We looked at the pre-admission assessment process that the home has in place and the records of two service users who have been admitted since the last inspection. This showed that a detailed assessment was carried out for one of the service user prior to admission. The manager was clear about the importance of completing this assessment to ensure that the service is able to meet the service users needs. The other service user was admitted as an emergency admission. The home is in the process of closure and the staff reported that only short term admissions were currently taking place. The care plans for three service users seen contained information gained from the pre assessment and had been developed since admission. The care plans contained good details of their assessed needs and action plans were in place to show how these would be met. These included assistance with personal care, moving and handling and medication management for the service user who was administering their own medicines. Information and referral from care manager was available as required. The manager reported that they receive good support from the local primary care trust and and the local surgery. The GP visited the service users at the home on request . One person who was having difficulty swallowing has been referred to the team for a swallowing assessment. Equipment such as a profiling bed, pressure relieving equipment were available as required. A service user was being supported by the district nurses team in managing their wound dressings. We looked at the process for the ordering, receiving and general management of service users medication. Medication was stored in a locked and secure medicine trolley, in a locked room and where a medical refrigerator was also housed. Appropriate facility for the storage controlled medicines was available. The manager confirmed that only senior staff who had completed medication training were responsible for the service users medicines. We talked to a staff member about disposal of medicines and found this was managed and recorded appropriately as per the homes procedures. No excess medicines were found during this visit. It was evident that the medicines management has improved and the home has a dedicated staff member who was responsible for the service users medicines management. Care Homes for Older People Page 4 of 9 A record of prescribed creams as administered by the carers was maintained. There is a daily audit for as required medication and staff recorded this as part of their checks. A sample of the medication administration record(MAR) charts seen indicated that these were completed as required. A review of medicines that are prescribed to be given regularly and not required should be undertaken to ensure that these are recorded appropriately. Staff must ensure that the MAR charts records reflect medicines accurately as per the prescriptions at all times to ensure that people receive their medicines correctly and safely. There is an activity programme in place and the service users we spoke to were satisfied with the current activity arranged for them. Planned activities included games, reminiscing, musical bingo, themed week and also more opportunity for 1:1 time as there are only eleven service users remaining. The home has adapted one of the rooms as an indoor garden area and the service users spoken to said that they had grown pansies and other things. Regular service users meetings were held and a monthly newsletter was provided to the service users. Regular church services were also held at the home and one of the service users said that they enjoyed the service. The meals at the home were well managed and people we spoke to were complimentary about the food provided. They told us that they were supported to choose from the daily menu and they were able to opt for something different from the menu and this would be provided. The lunchtime meal was seen and this looked well presented, balanced and nutritious. Hot and cold drinks were available to the service users throughout the day. Staff were available to offer support with meals as required and meals were not rushed. Comments about the food included the food is good. You can choose what you want and there is always plenty. I like my cup of tea and there is always plenty of tea here. There is a complaint process in place and a log was maintained of concerns raised. This included compliments about care and showed that concerns/ complaints are investigated and details of action taken were recorded. The service users said that they were satisfied with the care and the staff were very good. They said that they would approach the manager or the staff if they were not happy and were confident that they would be listened to. There has been one safeguarding alert that the home had referred to the safeguarding team. The manager said that this had been investigated by the team. However the home has not received the outcome of the investigation which should include any recommendation as appropriate so that the home can put in place an action if required to safeguard people living at the home.. The service has a registered manager who is responsible for the day- to day management of the service. It was evident from interactions observed that the staff and the service users had developed good relationships with each other. Comments were that it was difficult to keep motivation and morale up due to the impending closure but everyone tries their best. Care Homes for Older People Page 5 of 9 The service users we spoke to were anxious and angry about the current situation. They told that the decision has been made to close the home and they did not understand why they were still waiting. They found it very unsettling and wanted to move to their new home as soon as possible. The manager reported that a care manager has been appointed and was working with the home to speed up this process and this was dependent on vacancies in other homes. One of the service users had been out on a visit to another service and told us that they like the home and wish to move there soon.
