It has a railway station one mile away and is close to the town centre. Beechcroft offers 24-hour personal care, for up to 25 adults. All places are available to older people, and six places are available for persons with physical disabilities. There are communal lounges and a conservatory area for people to make use of. The home has very nice gardens and there is a patio area for people to enjoy. The Home is well maintained and there is a continuous programme of re-decoration. We carried out this specialist pharmacist inspection of the residents medicines to see if they were being managed safely. We looked at medicines, how they were stored and how medicine records are made and to see if they show that residents are given their medicines to the doctors directions. We looked at the facilities for storing medicines and found that the medicines were in secure facilities and at the correct temperature. Medicines were packed, where possible, in a calendar pack that makes it easy to see at a glance whether the medicines have been given correctly. The main storage cupboards were well organised, with each residents medicines kept together in a basket, one in each cupboard, for internal and external use. Facilities for looking after controlled drugs (those needing special security because of being liable to abuse) were satisfactory. If medicines needed cool storage there was a dedicated fridge with an integral monitor showing that it was running at the correct temperature. It was evident that a great deal of effort had been put into keeping the medicines records. The audit trail of records of receipt, medicines given or refused etc. and those disposed of when no longer required can be compared with the amounts of medicines in stock. This can show that the medicines have been given properly. Some of the records were excellent and complete and showed the complete audit trail. Others were not so clear and it was neceassary for the manager or senior carer to explain what the records meant. A note was made of amount of medicine held at the end of one record sheet carried to the next. The records of this were entered in the boxes for medicines received rather than the box titled carry forward. Some medicines have no dose specified in the printed directions because it changes too frequently. Staff remember this information or a rough note is included which was not accountably documented, to show who made the change or what the change is. eg. a residents dose of warfarin changed but the record did not include details of the change, who made it and when.
