The latter is an area of care which the service is developing as a specialism. Field Farm House is a converted and extended farmhouse in a rural location just outside the city of Hereford. A schedule of fees is available from the home, and a copy of this inspection report can be viewed at the home. This was an unannounced inspection, and was carried out by one inspector over half a day. We visited Field Farm House so that we could check that the home is providing care that meets peoples needs. We looked in detail at the care records for two people who live at the home, and we spoke to staff and people who live at the home. We found that care plans were written in a person-centred way, focusing on the whole person, rather than reducing them to a list of tasks. The care plans were detailed and gave staff the information they need to provide care. We saw that there was a strong emphasis on ensuring that peoples preferences were recorded, such as preferred gender of care staff. Risk assessments and care plans had been reviewed and updated regularly, and there was clear evidence that staff were implementing the care plans. The daily records included detailed information about each persons progress. We were shown handover sheets which make sure that staff coming on duty know of any changes to peoples needs. Care plans are all kept in a central location, and as the home is a large building, this means that they are not easily accessible to staff. The manager explained that the home is in the process of moving the care plans so that it will be easier to access them. We found some minor shortfalls in care planning, such as the lack of clear information to staff about when a medication which had been prescribed on an as and when basis should be given. However, as soon as these were pointed out to the manager, she said that the required plans would be put in place immediately. Records showed that the home requests advice and support from health care professionals such as GPs and District Nurses whenever necessary. Care plans included information about regular reviews by GPs. The home has a friendly and welcoming atmosphere, and people told us that the staff look after them very well. Everyone we spoke to was very positive about the staff, and one person told us they couldnt do more for us, nothings too much trouble. We found that Field Farm House is a well managed home, where risk assessment and care planning is robust, and staff provide a good standard of care. What the care home does well:
Because this inspection was focused on care planning and provision, we may not have identified other areas of good practice. Care plans are person-centred and are reviewed and updated regularly.
