Conditions of registration: Date of last inspection 26th February 2007 Brief Description of the Service: Number Residential Care Home is registered to provide accommodation and personal care for a maximum of ten people with a mental disorder. Mrs Ranie Safderali Datoo is both the registered manager and proprietor of the home. The proprietor has another small home two doors away from this home in the same road. The home is located in Tottenham, North London, close to shops, public houses, restaurants, and cafés and transport facilities including the Bruce Grove Rail Station. The home consists of two adjacent terrace houses, which have been joined together to create a single home for ten residents. The home provides ten single bedrooms with en suite facilities on the ground and first floors. There are in additional two communal bathrooms and toilets at the home. The home has undergone refurbishment and recently rooms have been added through the extension of the building to provide a large dining room, a sitting/smoking room, staff room, staff sleeping-in room and an office. There is a large lounge adjacent to the kitchen. The front garden is paved and is used as a car park for staff and visitors. The rear garden is also paved and is used by residents to relax in during the summer months. The stated aim of the home is to provide residents with a secure, relaxed, and homely environment in which their care, well-being and comfort are of the prime importance. The fees range from £500 to £1000 for each placement per week, toiletries and newspapers are provided and residents are expected to pay separately for items such as the sky TV channel, hairdressing and dry cleaning. This is an overview of what the inspector found during the inspection. The quality rating for this service is 3 star. This means the people who use this service experience excellent quality outcomes
The inspection was carried out in the morning and lasted approximately four hours. There were ten residents living at the home at the time of the inspection. Residents were having their breakfast or sitting in the lounge when the inspector arrived. Three service users were kind enough to speak to the inspector in some depth. The inspector interviewed two support workers and one senior support worker. Three residents’ and two staff files were examined in some detail as well as written records and policies and procedures. The responsible individual and registered manager Mrs Dattoo aided the inspector during the inspection and showed the inspector around the home. The administrator and all staff were open and helpful throughout the inspection process. What the service does well:
The home keeps comprehensive records of residents’ files and of policies and procedures. The manager demonstrated a good understanding of the needs of the client group. The residents interviewed stated that they were happy living at the home and described their daily activities at the Day Centre and at the home. One resident stated that their key worker was nice. The responsible individual keeps abreast of developments in residential care and is willing to work with care managers and health professionals. The diversity and quality of the recreational facilities provided at the home was impressive. The Responsible individual/manager had a good grasp of the requirements of her management role. Mrs Datoo appeared to communicate a clear sense of direction and leadership which had a positive influence on the quality of care being provided. Policies and Procedures and Health and Safety requirements were up to date. There was evidence of systematic monitoring of safety checks. This judgement has been made using available evidence including a visit to this service. Prospective residents’ needs are thoroughly assessed prior to them moving to the home. EVIDENCE: The records of two residents were examined including the most recent admission. These indicated that people are admitted to the home only after a comprehensive assessment information has been provided to the service to enable them to make the decision that they can meet the person’s needs. The assessment comprises of a needs and risk assessment. Records confirm that prospective residents visited the home prior to making a decision. The residents’ interviewed stated that their needs were being met and that they were able to continue enjoying their meals from their various cultures. The service user guide produced by the home is of a good standard. Residents are given a welcome pack on admission. This outlines the local amenities i.e. clubs, cinema, gym, restaurants in pictorial form. It also includes residents’ rating of these facilities. Residents are supported to agree goals that are documented in their care plans with the ultimate aim being to move towards greater independence. Residents are supported to take risks as part of an independent lifestyle. Residents benefit from having regular meetings in the home to enable them to express their views about the service. EVIDENCE: The inspector reviewed the written records of two residents including the most recent admission. Both files contained comprehensive assessments and care plan reviews. There was evidence that care plans provided guidance to staff in respect of the resident’s ethnic, cultural and religious needs and preferences. This was reflected by the provision of meals and care to meet the cultural needs of residents. Risk assessments were also seen and outlined the potential harmful risks to the resident and measures in place to reduce risks. The inspector met 5 residents and interviewed 3 service users. They all stated that they were supported by staff to engage in their various activities. One resident stated that he could make his views known to staff or in monthly service users’ meetings. The home focuses in identifying and developing residents’ skills with the goal of moving to independent living. A number of residents have moved to independent flats. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15, 16 and 17. People using this service experience good quality outcomes in this area. We have made this judgement using available evidence including a visit to this service. The home provides opportunities for personal development and residents take part in a wide range of social and leisure activities, both in the home and in their local community. Staff support residents to maintain family links. The food provided is culturally appropriate and healthy, and offers variety and choice. EVIDENCE: The evidence was gained from speaking to residents and staff and from documentation in care plans. The objective of the home is to enable residents to lead a more independent life. The inspector noted that residents are encouraged to be involved in the day-to-day chores around the house such as shopping, cooking, washing and cleaning. They go out swimming, bowling, to cinemas, cafes and pubs. At the time of the inspection the residents were getting ready to go out out either for

We were able to view relatives’ comments, for example, thanking the home for the hard work and thoughtfulness shown in organising a recent birthday party. The home has Sky Television with a range of programmes for the residents to watch in the lounge. There were television sets, video players and music systems in the bedrooms inspected and the conservatory is used as a games room with a very good range of board games, table football, table tennis and air hockey. The records of two residents showed the availability of a range of activities in the home in the evenings, that these residents had taken part in and these included: beauty treatments, a news and views group, arts and crafts, board games and music therapy. One resident spends weekends with their parents on a regular basis. The father of the other resident visits weekly. The home’s commitment to ensure residents’ rights and choice is stated in the statement of purpose. There is also a policy in place regarding empowerment, which aims to ensure that residents have access to the full range of local services such as libraries, adult education, transport and health services. Some residents were observed going out and coming back to the home independently or with staff support, depending on their individual needs. The inspector saw the minutes of the meetings that are held regularly with residents and these indicate that they are consulted and informed about a range of issues such as leisure activities, and when staff were recruited, residents were encouraged to give feedback as part of the staffs’ induction process. The home produces a monthly Newsletter, which informs on the highlights of the month, for example, photographs taken at a birthday party and in the snow. The menu was varied and nutritious with a choice of meals at lunchtimes and evenings. The registered person told the inspector that the residents are all encouraged to be involved in the preparation of meals. The inspector noted that residents’ needs and preferences regarding food were noted as part of their individual plans. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19 and 20 People using this service experience good quality in this area. We have made this judgement using available evidence including a visit to this service. Staff provide sensitive and flexible personal support and assist residents to gain access to the appropriate health care. The residents are protected by the home’s policy and procedure for the receipt, recording, storage, handling, administration and disposal of medicines. EVIDENCE: The care plans examined reflected that residents were able to communicate their wishes needs and views. They had some independence in terms of their personal care, and their needs in this area were noted, so that staff are clear about their support needs. The Inspector spoke to a resident whose care plan was also reviewed in detail. The needs relating to the person’s cultural and relgious background and their gender were considered and noted. The resident told us that her specific cultural needs in relation to personal care had been addressed. The resident also stated that she was able to enjoy her favourite cultural meals.

Residents are registered with a local GP and there were monitoring records in place that indicate that they were supported to access health care services such as the optician, dentist, dietician, community nurse and psychiatrist. The inspector briefly reviewed the arrangements for the storage, administration and recording of medication in the home. The medication was kept appropriately in a locked cupboard. Records included medication received and medication returned to the pharmacist. The Boots system is used; there was evidence that staff members had been provided with appropriate training in the administration of medication and the records were well ordered, up to date, and accurate. This judgement has been made using available evidence including a visit to this service. There are clear and effective complaints, adult protection and whistle blowing procedures and residents are safeguarded from abuse and neglect. EVIDENCE: We observed on our inspection that residents were treated with respect and received good support from staff. There was evidence that the service user’s guide was given to new residents as they move in. This sets out how residents can make a complaint. No complaints had been received or recorded by the home and no complaints have been made about the service directly to the Commission. The inspector asked two residents if they knew how to make a complaint. They were both clear that they have had no cause to complain since they moved into the home. They were clear about whom they would complain to if they had any issues about the home. No adult protection issues have arisen, a copy of the adult protection procedure is available in the home. There was evidence that staff members had been provided with training in adult protection. This judgement has been made using available evidence including a visit to this service. The home provides a homely environment. Bedrooms are personalised. The standard of the décor is good and there is adequate space for residents to use. The residents can be confident that the home will be kept clean. EVIDENCE: There were a number of communal areas including a lounge with comfortable sofas, a large, bright dining area/sitting room and another quiet room. All these areas were observed to be used by residents. There is a separate activities room that is particularly well equipped with pool table, table tennis table and lots of other recreational equipment. A paved garden at the back has chairs, tables and umbrella, which the residents use when the weather permits. All the communal areas were clean, tidy, and bright, well decorated and had good quality furniture. The home is not registered to provide services for people with additional, physical disabilities. The facilities and equipment at the home meet the needs of current residents.

The responsible Individual stated that it did not affect the safety of the current residents. A risk assessment is recommended to determine the right course of action. Residents have their own single bedroom with en-suite facilities. The bedrooms were not measured at this inspection. All bedrooms have sufficient and comfortable furniture and were decorated and equipped to reflect the backgrounds and interests of their occupants. The residents spoken to informed the inspector that they were very happy with their rooms. An additional communal bathroom and toilet are provided at the home. All the washing facilities were clean and tidy on the day of the inspection. All parts of the home, which were inspected, were very clean, hygienic and free from offensive odours. This judgement has been made using available evidence including a visit to this service. There are sufficient numbers of staff employed to meet the needs of the residents. Residents are protected by the home’s recruitment practices, and staff benefit from the provision of relevant training. EVIDENCE: The registered manager stated that several staff members have completed NVQ level 2 & 3 in care and some are currently undertaking NVQ level 3 and NVQ 2. The Inspector interviewed one senior care worker and 2 care workers on duty. The personal files of two members of staff were also reviewed. There are good records of the training that staff have received and each member of staff has an individual training plan. There are good systems in place to ensure that staff member’s training needs are highlighted and that they are receiving the core training and the specialist training that they need to meet the needs of the residents in their care. The inspector saw the rota, which indicated that there are sufficient staff provided to meet the needs of the residents. There are 7 full time care staff including 3 senior care workers and two part time care workers. The
Number Residential Care Home DS0000068765.V374213.R01.S.doc Version 5.2 Page 20 registered manager is on shift between 9 am and 5 pm Monday to Friday. There is no cleaner. The care workers involve residents in the cleaning of the home as part of working towards their independence. The rota showed that three staff cover the early shifts and two staff are on duty in the afternoons. At weekends, one extra member of staff is provided to support residents. A sleep-in and a waking member of staff cover the night shifts. The registered person maintains staff records in the home as outlined in the National Minimum Standards including a written application, CRB details, written references, and a recent photograph of each staff member. The senior care worker has 7 years experience and holds a NVQ level 3. She demonstrated knowledge of mandatory and specialist training including The Mental Capacity Act 2005. She supervised care workers and was supervised by the manager and records of supervision were available. One of the two care workers interviewed had completed the NVQ level 2 and the other member of staff had started the NVQ level 2. In addition, both members of staff had undertaken training in medication, fire safety, food hygiene, COSHH, First Aid, risk assessment and care plans. Both care workers interviewed stated that as students they worked a total of 20 hours per week. They work at the home for 8hours every Monday and do another shift in another Number care home. It is recommended that care workers work primarily in one home to ensure a better continuity of care for residents. This judgement has been made using available evidence including a visit to this service. The management of the home is geared to ensuring residents’ progress and independence. Service users are confident that their views are taken into account when the home is planning any review. The safety of residents is well promoted and protected. EVIDENCE: The registered manager,Mrs Datoo, is also the responsible individual and has managed the home since it first opened in 1989. She has had prior experience of working with people with mental health needs. She possesses the City and Guilds Advanced Management for Care and NVQ level 4 in Care. Mrs Datoo has undertaken the Registered Manager’s Award. Certificates of attendance of courses showed that she has attended a number of training programmes. The Responsible individual/manager had a good grasp of the requirements of her

Mrs Datoo appeared to communicate a clear sense of direction and leadership which had a positive influence on the quality of care being provided. Policies and Procedures and Health and Safety requirements were up to date. The manager was co-operative with the inspector in providing all necessary information and documents needed for this inspection. Quality assurance questionnaires showed that residents’ views were taken into account when planning any development of the home. The home acted upon the suggestion of organising a Barbecue with the theme of “Cowboy and Western Night”. There is a fire safety policy and an emergency plan in place. The fire records indicated that weekly fire alarm checks had been carried out. Fire drills have been carried out regularly with residents’ participation once a month. Risk assessments for residents’ use of the kitchen have been completed. The necessary safety inspections had been carried out on the portable electrical appliances and gas installations. The electrical wiring system check had been carried out and is next due on 8th May 2009. There was strong evidence of a management system that systematically records and monitors safety checks to ensure the wellbeing and safety of residents.
