The home is situated in a suburban area in Worthing, with local shops and transport links. The accommodation is provided on ground and first floor levels, and there is a passenger lift. None of the bedrooms have en-suite toilet or bath/shower facilities. The responsible person for the home on behalf of the Care Management Group Ltd is Mr M. Care Homes for Adults (18-65 years)
Page 2 of 8 Buckingham. There was not a manager registered with CSCI in respect of the home at the time of the inspection visit. The fee range is from 1500 pounds to 1700 pounds per week. Care Homes for Adults (18-65 years) Page 3 of 8 What we found:
At the Annual Service Review relatives said that residents are not always fully occupied. We looked at the activity schedules for residents and the records of activities that residents have recently attended. Residents attend a wide variety of activities that include day centres, aromatherapy, reflexology, bowling, swimming and day trips. it was also noted that residents have free time during their days so they can do things that they enjoy such as watching DVDs. We were told that the home has now employed a driver for Saturdays to enable residents to have trips out at the weekends. One parent raised concerns that their relative was not receiving the correct diet, enough fluids or assistance with feeding. We looked at the care plans and menu book for the residents. The menu book records what food was served, what and how much each resident ate of the meal. Fluid intakes are also monitored and recorded. Residents that require a pureed diet have each item of food liquidised separately so the food retains individual flavours. Another parent raised concerns that their relative was not receiving the full fat diet that they require. This was looked into during the visit. We found that there are instructions in the kitchen for the cook regarding this residents diet. Menu books show that a nutritious full fat diet is provided. It was also seen that if the resident refuses food at mealtimes then they are offered another meal later. The weight of this resident is monitored and recorded every month to enable the home to pick up on any problems. The resident has recently been referred to a dietitian through their General Practitioner. Staff spoken with were able to tell us what the dietary requirements for individual residents were. A parent reported that a wheelchair they bought for their relative was not being used. The home showed us that the CTLPD, (Community Team for Physical and Learning Disability), physiotherapist assessed the suitability of the wheelchair and advised that it needs some adaptations and modifications to make it safe and suitable for use by the resident. Until these have been carried out the home has decided that it is safer for the resident to use their old wheelchair. We were told by a relative that a leaking roof had not been repaired. The home showed us evidence including the receipt for the work that this has now been repaired. A parent was concerned that the home does not have enough staff on duty, particularly at weekends, for activities to take place. We looked at the rota and activity records and found that more staff are now on duty at times when residents like to take part in activities. We were also told that the home has now employed a driver for Saturdays to enable residents to have trips out at the weekends. Concerns were also raised that some staff are not able to communicate effectively with residents as they are unable to speak adequate English. Staff who have experienced these difficulties have attended an eight week programme for communication. This includes an assessment on written skills and levels of understanding. At the last inspection a Requirement was made that staff receive training appropriate Care Homes for Adults (18-65 years) Page 4 of 8 to the work they are to perform. We looked at staff training records and found them to be up to date. Recent training attended by all staff includes Fire Safety, First Aid, Protection of Vulnerable Adults, Medication Administration and Manual Handling. Concerns were raised that the standard of care at the home have decreased since the manager has been on long term leave. Currently the Deputy Manager is acting up. We spoke to the Deputy Manager about the support she has received from the organisation and to staff. The acting manager said she has received a lot of support from the organisation that includes managers from other homes in the organisation and frequent visits from the Regional Director. This was confirmed by two staff we spoke with. Both staff felt that the acting manager supports the team and runs the home well. The organisation has arranged for a temporary manager to take over, they were due to start the week after the visit. At present they do not know when the manager will be returning to work. The Requirement made at the last inspection that the Registered Person shall register a manager has not been met. At the last inspection a Requirement was made that the Provider must ensure a safe supply of hot water and that records evidencing this are maintained. We saw records that show that water temperature checks are now carried out weekly. bath and shower temperatures are checked each time and the temperatures are recorded. This Requirement has now been met.
