This inspection was carried out on 21st December 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Good. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 5 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

The home provides a caring environment for service users. It also benefits from a stable staff team, who work well together helping each other. Due to the small number of residents the staff know them well and there is a friendly atmosphere .The bungalow is a pleasant domestic environment to work in, it is light, airy and well maintained. The residents have the benefit of a minibus to go out in and regular trips are organised, enabling them to lead as full a social life as possible. The home continues with a full training programme for members of staff, this includes Drugs Administration training.

What has improved since the last inspection?

Improvements had been made in the kitchen. The kitchen cupboards had been removed from above the cooker therefore reducing the fire risk. New flooring (ceramic tiles) had also been put down in the kitchen. Decoration had also taken place. A new washer and dryer had been purchased, which has a pre-wash of 90 degrees C.

What the care home could do better:

The radiator covers require painting as does some areas in the hallway. The decorator has been booked for the New Year. Also an extra shower in the small bathroom would provide additional bathing facilities, which are now required due to the dependency of the residents. Some of the cupboard door fronts in the kitchen would benefit from being replaced. The home`s manager recently left employment at the home and has been replaced by a deputy acting in her place for the last year. A fully established and registered manager is needed to support staff and establish leadership for the home.

Accommodation is provided on the ground floor and is accessible for wheelchair users. The home has three single and one double bedroom, a large lounge area and kitchen/dining room. A small utility room provides laundry facilities. Staff accommodation consists of a single bedroom/office .The home has a garden to the rear and side of the bungalow providing space for the service users to sit out. Car parking is provided at the front of the property. Service users at the home have a range of needs including mobility difficulties. They are supported to attend day services, college and receive therapeutic treatments. In addition to a lively social life with outings and holidays provided for, the home has its own transport and additional staff are employed to support the mobility of service users and their participation in activities. The inspection was unannounced and took place over 2 hours. The temporary acting manager was not on duty. One member of staff was on duty in the home. two others arrived during the inspection. Three residents were attending day centres and two remained at the home, they had just eaten lunch and were watching television in the lounge. During the inspection a partial tour of the bungalow took place, duty rotas, accident reports and menus were inspected. Residents were spoken to and also the staff members. As all of the standards were looked at on the last Inspection only a small selection were assessed. Some requirements from the last Inspection were outstanding but were being dealt with. The decorator had been booked for early in the New Year to complete the hall and the radiator covers. Quotes had been obtained for two new bedroom carpets. Discussions had taken place with D.C.H.S. over the additional shower room. The manager who had been off sick since December 2004 and had resigned last week. The acting manager was still in post. JUDGEMENT – we looked at outcomes for the following standard(s): 17 A varied and healthy diet is offered to the service users. EVIDENCE: Menus were looked at. These are on a four weekly rota. The staff know service users preferences and alternative meals to the menu are available. All are on normal diets. They are all weighed on a regular basis. The shopping is done by the staff and the service users. There were plentiful supplies of food in the home including fresh fruit and vegetables. The weekly budget had been discussed with D.C.H.S but the staff were unsure of the outcome. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20 The home has satisfactory policies & procedures for dealing with medicines. EVIDENCE: All medicines are dispensed by the home staff. All staff are studying for the award of Administration of medicines. The medicine cupboard is kept in the kitchen and locked at all times. No controlled drugs are kept in the home. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 27 28 29 30 The standard of the environment within the home is good providing service users with an attractive and homely place to live. EVIDENCE: Some kitchen improvements had been completed but some of the kitchen cupboard fronts are in need of replacement. The home was clean, well ventilated and well lit. The home had a bathroom equipped with handrails and shower. An extra shower would be beneficial in the small toilet area due to the increasing needs of the residents. Painting of radiator covers is required, as is some painting of the woodwork in the hall area. Quotes for two new bedroom carpets had been obtained. JUDGEMENT – we looked at outcomes for the following standard(s): 31 32 33 Service users are well supported by a stable staff team EVIDENCE: Service users are well supported by a stable staff team and staff had completed the acting manager .All mandatory training. Staff spoken to felt that the arrangements for training were good. Observations during the inspection indicated that staff had excellent relationships with service users and were able to communicate effectively with them. During each day two members of staff were on duty plus the acting manager. At night one member of staff is awake and a member of staff sleeps in also. The duty rotas were inspected and it was evident that the acting manager was not receiving supernumerary time, which is necessary for managerial duties. JUDGEMENT – we looked at outcomes for the following standard(s): 37 42 The home is well managed (by the temporary acting manager) .The appointment of a manager is necessary to provide stable leadership. EVIDENCE: The home is currently without a registered manager. The previous manager had recently left the homes employment and an acting manager has been in place for the last year. It is essential that a new manager be in place as soon as possible, to ensure continuity and maintain standards. The staff accident book and accident report forms for residents were inspected during the visit these contained all necessary information.
