Conditions of registration: Date of last inspection 25/01/2005 Brief Description of the Service: Jervaulx Road is a modern, purpose built, two storey building, comprising of three separate units interlinked by internal corridors housing staff sleep in rooms, office and laundry facilities. Each unit provides comfortable living accommodation for six residents with a learning disability. Each unit provides a distinct service, one unit is for those residents requiring long term care, another for respite, and the third provides short term rehabilitation and preparation for living within the community. Accommodation is provided in eighteen single rooms, all of which meet with spacial requirements of National Minimum Standards. Bedrooms do not contain ensuite facilities. Communal space in two of the units consists of a kitchen/dining room and a lounge. The third unit consists of a dining kitchen and two lounges. Kitchens are domestic in nature and accessible to residents. Externally there is a lawned garden with shrubs, rockery seating and patio area. This is an overview of what the inspector found during the inspection. This unannounced inspection started at 12.25pm and lasted for four and a half hours. A discussion took place with four residents and the person in charge of the home during the inspection; no relatives were present during the inspection. Discussions with residents were limited due to the nature of their disability. Numerous records including care plans, policies/procedures maintenance and staff records were examined. A brief tour of the premises was carried out. The Manager was not present at the home during the inspection, however, staff should be complimented on their efficiency throughout the inspection process. Policies and procedures as highlighted at previous inspections are required to be reviewed and updated in order to meet with National Minimum standards and Care Home Regulations 2001. Immediate action must be taken to ensure that all residents have an appropriate plan of care. The laundry wall finishes and flooring requires addressing. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 Appropriate assessments of prospective residents are carried out to ensure that the home can meet their needs. EVIDENCE: Residents are admitted to the home following an assessment that is carried out by a Social Worker or Health Care Professional. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 9 The home is good at meeting the needs of the residents, however, not all residents have an individual plan of care, which clearly sets out how the assessed needs will be met. Residents are supported to take responsible risks to ensure that independence is promoted. EVIDENCE: Two plans of care were examined during this inspection, both of which were observed to contain a photograph of the resident. One plan of care was that of a resident receiving long-term care. This plan of care found to be informative, included a detailed assessment of needs and evidence of resident involvement and choice. The second plan of care was that of a resident who had been in the home for a number of months and is preparing for independent living. Examination of records found that there was only limited information on file for this resident, no plans of care or any information in respect of preparation for independent living. This was pointed out to the person in charge of the home during the inspection who agreed to take immediate action to rectify this.

It was evident following speaking to residents that they are encouraged to make decisions and exercise freedom of choice. One residents spoken to during the weekend said, “I like to have a sleep in on a weekend. When I get up I go and get a paper from Bells Stores”. Residents are enabled to take responsible risks. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 15, 16 Resident’s daily lives are enhanced by the social activities provided by the home and the welcome it extends to residents visitors. EVIDENCE: The person in charge of the home during the inspection said that the majority of residents staying at the home attended day services Monday to Friday. She also stated that one resident is currently doing a computer course and that a computer is available within the home environment for use by residents. The home has their own car to transport residents; recent trips have included Whitby, the cinema and tea out at a restaurant or pub. Residents staying on the long stay unit do have an annual holiday, this year the residents have chosen a trip to Blackpool. Although residents are staying in the same hotel they have each planned different activities and things to do. Residents spoken to during the inspection said that they were looking forward to the planned trip. 17 Jervaulx Road B51 B01 S31330 17 Jervaulx Rd V239411 180705 Stage 4.doc Version 1.40 Page 12 One resident spoken to during the inspection said, “I’m going to see the new Batman movie and I’ve just been to see Cliff Richard”. One resident had recently taken part in the bowling event at the Special Olympics. One resident staying on the respite care unit, said “I like it here” Residents spoken to said that contact with family and friends is encouraged and that visitors are made to feel welcome at any time. Independence is encouraged for those residents who are able. Some residents are able to assist with tea preparation, washing up and ironing. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 20 The home is good at meeting the physical and emotional health needs of residents by ensuring that residents receive their care in the way that they choose to. Arrangements for dealing with medication are appropriate for the needs of the residents and are managed in a safe manner; minor improvements to record keeping would further enhance this. EVIDENCE: Residents spoken to during the inspection said that staff at the home respected their privacy and dignity whilst assisting with care needs. Staff interaction with residents on the day of inspection was observed to be relaxed, respectful, understanding and enabling. It was evident from the inspection that residents are able to exercise choice and that there is flexibility in routine. One care plan examined during this inspection contained detailed information about the resident, care requirements and how the resident wanted their care to be delivered. 17 Jervaulx Road B51 B01 S31330 17 Jervaulx Rd V239411 180705 Stage 4.doc Version 1.40 Page 14 The person in charge of the home during the inspection said that currently there are not any residents who are able to self medicate, however, if there was to be in the future, a risk assessment would be carried out to determine if self medicating was possible and safe. The home has a policy/procedure in respect of medication systems that are in place at the home. Since last inspection the home has worked extremely hard to improve medication systems that are in place in the home, appropriate systems are now in place. Records were examined to confirm that appropriate records were kept of medication coming into the home and that staff were keeping a record of medication returned to pharmacy, however records of returned medication was on loose leaf paper. JUDGEMENT – we looked at outcomes for standard(s) 22 The home has an effective complaints procedure, which enables residents to make any complaints they feel necessary. EVIDENCE: The home has a clear complaints procedure, details of which are also available in the statement of purpose and service user guide. Residents spoken to during the inspection said that the Manager and staff at the home are approachable and if they felt the need to complain then they would do so. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) EVIDENCE: The above standards were not inspected, however, requirements highlighted at the last inspection in January 2005 were re-visited. Exposed hot water pipes in residents bedrooms had been boxed in to ensure safety of residents, the toilet on the respite unit had been re-decorated, however corporate policies and procedures in respect of control of infection had not been updated. 17 Jervaulx Road B51 B01 S31330 17 Jervaulx Rd V239411 180705 Stage 4.doc Version 1.40 Page 17 Staffing

Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) EVIDENCE: The above standards were not inspected, however a requirement highlighted at previous inspections in respect of staff records being stored in the care home has been addressed. It has been agreed by the Commission for Social Care Inspection that staff records can be held centrally at Redcar & Cleveland Borough Council Offices, and that the CSCI will view records on an annual basis at the Council offices. Since last inspection the home made a number of changes to the home environment in order to accommodate a crisis unit. This change has resulted in a move of the home’s laundry area. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 40, 42 Staff receive training relevant to the job that they do, however, fire training is not adequate to ensure safety and protection of staff and residents. Not all policies and procedures have been reviewed and updated to ensure resident’s rights and best interests are safeguarded. EVIDENCE: The home operates a rolling programme of servicing appliances and equipment. Records examined at random confirmed that the home’s central heating, boilers, fire extinguishers and fire alarm system are serviced on a regular basis. 17 Jervaulx Road B51 B01 S31330 17 Jervaulx Rd V239411 180705 Stage 4.doc Version 1.40 Page 19 Records were available to confirm that staff receive regular training in respect of moving and handling, first aid and other training relevant to the working environment. Records were available to confirm that staff receive fire training, however, this training is provided to staff by them watching a video and then answering questions. A discussion took place regarding the suitability and effectiveness of the video. Records were available of regular fire drills in which both staff and residents participate. Redcar & Cleveland Borough Council also carries out health and safety audits at the home at regular intervals.
