What the care home does well

The home provides a pleasant, calm and settled environment for residents. Residents were observed to be calm and happy during the course of the inspection and to be relaxed and comfortable in the presence of staff. The report prepared for residents review and the presentation of information is clear and informative.

What has improved since the last inspection?

The care plans, residents goals and behaviour guidelines have been reviewed and updated to ensure residents current needs can be met by staff. A policy regarding same gender care is now in place. Residents wishes regarding terminal care and death have been sought and recorded, ensuring residents needs can be met by staff. The pictorial complaints procedure has been updated to include the name of the current CSCI inspector ensuring residents have up to date information. These issues were all Requirements at the last inspection. The presentation of review reports has been developed to include pictures and information regarding residents, giving people at the review the opportunity to see how individual residents spend their time at the home.

What the care home could do better:

The social history for one residents requires updating to ensure staff have access to correct information regarding the individuals family.The garden requires attention to ensure that it is safely maintained for residents to use. The toilet, which makes a loud noise when flushed, requires attention to ensure residents continue to live in a peaceful environment.

Six people are currently residing at the home. The home is owned by a housing association and managed and staffed by the organisation, United Response. The home is a detached property in a cul-de-sac off of Worple Road in Wimbledon. It is not identifiable as a care home and is in keeping with neighbouring houses. It is close to public transport, local shops, churches and leisure facilities. Accommodation is provided over two floors with five bedrooms and a bathroom available on the first floor and the remaining bedroom, bathroom, lounge, dining room and kitchen on the ground floor. The home has a large garden. There is a car for staff to use. The home is staffed twenty-four hours a day. This unannounced inspection took place over the course of four hours one afternoon by one regulatory inspector. The inspection consisted of examination of records, inspection of communal areas of the home, two residents bedrooms, speaking with residents, staff and the registered manager. The inspector had the opportunity to spend time with all six residents and two members of staff. Residents made positive comments regarding the home, their bedroom and staff and were comfortable and relaxed at the home. The inspector took a meal with residents, which was well received and of appropriate quality and quantity. One resident said ‘I like the food and that it is usually good like this’. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1 & 2 Prospective residents receive appropriate information regarding the home and services provided, enabling them to make an informed decision regarding moving into the home. Residents needs are assessed, however further work is required to develop goals for each resident. EVIDENCE: The Statement of Purpose has not changed since the last inspection when it included information required to assist residents, their relatives and placing social workers to make an informed decision regarding moving into the home. Current residents have been at the home for over four years, on admission needs assessments were completed which have been updated and developed into care plans. Further work is required to develop individual goals for each resident to ensure they have clear direction to work towards each year. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 8 & 9 Residents needs are appropriately assessed and developed into a care plan. Further work is required to develop goals for each resident. Residents are encouraged and assisted in making decisions regarding their lives. Residents participate in the day-to-day running of the home. EVIDENCE: Case files are separated into three sections covering finances; health and well being and lifestyle. These files have been updated with old information archived, ensuring staff have access to clear up to date information regarding residents. Care plans are in place and have been reviewed. Care plans include sufficient details for staff to meet residents needs. Communication profiles, residents likes and dislikes, personal care needs and risk assessments are included in case files. Residents have not identified goals to achieve in their care plans and this should be completed to ensure goals are identified and can be worked towards. Residents are involved in daily decisions regarding meals, activities and holidays at the home. Risk assessments are in place for individual residents ensuring staff have up to date information regarding residents safety needs. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 14 & 15 Residents take part in age and culturally appropriate activities. Residents are assisted and encouraged to maintain contact with family and friends. EVIDENCE: Residents attend day centres, sessions and clubs during the week, depending on their needs and choices. One resident said ‘I enjoy attending the day centre’. One resident was observed to be pursuing activities of their choice during the inspection. One resident reported that they had been out shopping with staff, that this is a regular occurrence and that this is something they enjoy doing. Residents are assisted to maintain contact with family and friends by staff at the home. One resident said that they enjoy seeing their family, which is enabled by staff at the home. Letters to the home from relatives confirm that they greatly benefit from the support from staff with contacting their relative at the home. Friends and relatives are invited to parties and events at the home. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 & 21 Residents receive appropriate support to meet their needs. The aging, illness and death of a resident are handled with respect and dignity. EVIDENCE: Assistance with personal care tasks is completed in privacy. The home aim to provide same gender care to individual residents when this has been requested. Residents needs are met by policies and procedures for staff regarding assisting residents with personal care tasks. There are no set times for residents to get up and go to bed, however if residents have a day centre or club to attend, staff will wake them in the morning to ensure residents are ready on time. Policies are in place regarding the illness and death of a service user, ensuring staff are aware of their responsibilities and actions they should take. One case file examined contained the individuals wishes regarding illness, terminal care and death, enabling staff to respect these wishes and act appropriately. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 Complaints are well managed at the home, with residents, relatives and placing social workers able to access the procedures. No residents raised concerns regarding the home and the services they receive. EVIDENCE: The complaints procedure is included in the Statement of Purpose and Service Users Guide to the home and is available to each resident in pictorial format. One resident said ‘if I am unhappy I would speak to the manager’, another resident said ‘I am happy here’. Records would be maintained of complaints received at the home. The CSCI has not received any complaints regarding the home. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 25, 27 & 28 The home provides a homely, comfortable and safe environment for residents to live in, however attention is required to one toilet and the garden to continue to provide a good environment. EVIDENCE: A tour of the home identified that it is decorated and maintained to a good standard. The home is suitable for its purpose, is accessible to current residents and is comfortable and homely. Residents have their own single bedrooms, which were observed to have been personalised to the individuals tastes. One resident said ‘I like my bedroom, I chose the colour it is painted and the posters’, another resident said that they like their bedroom and another resident was observed to be relaxed amongst their belongings in their bedroom. A bathroom with toilet is available on the ground floor and first floor with a separate toilet on the first floor. Attention is required to reduce the unacceptable noise level when the first floor toilet is flushed. Residents have access to a purpose designed garden with quiet areas for relaxing, scented flowers and plants for a sensory experience and a patio with garden furniture for enjoying the good weather. The grass needs cutting and the garden requires general attention to ensure it is maintained to a safe standard for residents to enjoy. The home is in keeping with neighbouring houses and is situated close to public transport links to Wimbledon shopping centre and leisure facilities with local shops and amenities also available. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33, 35 & 36 Residents needs are met by a group of longstanding and newly appointed members of staff who demonstrated detailed knowledge of residents needs, providing consistency of care to residents. EVIDENCE: The published staffing rota identified that two members of staff are on duty during the day with one member of staff awake and one member of staff asleep but on call at the home at night. These staffing levels were observed to be appropriate at the time of the inspection. The manager reported that the home is fully staffed with the support of relief staff employed by the organisation. The deputy manager is on maternity leave. The organisation has a training and development programme which is available to all staff. Staff have completed training in first aid, health and safety, medication, prevention of harm, challenging behaviour and autism. This ensures that residents receive support from staff who are suitably trained. New staff complete the Learning Disability Award Framework (LDAF) induction. The manager reported that staff receive supervision from senior staff who have completed training in supervision. This ensures that residents receive support and assistance from appropriately supervised staff. JUDGEMENT – we looked at outcomes for standard(s) 37, 39, Residents benefit from a well run home, the manager has a clear understanding of the service, the residents and their needs and the areas for improvement. The management of staff is in line with organisational policies and procedures. EVIDENCE: Staff meetings are held with the minutes available to all staff. The manager reported that she is in the process of completing the Registered Managers Award and has been managing the home for over five years and has a detailed understanding of residents needs and how to meet them. The area manager for the organisation visits the home every month and speaks with residents and staff, examines records and writes a report, which is available at the home with a copy sent to CSCI. This ensures residents have the opportunity to speak with managers regarding the services they receive. The organisation has representatives who visit the home to ascertain residents comments regarding the care and services they receive at the home.
