The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The care which the home provides is improving and service users are benefiting from better health care, more activities and outside professional advice. The new manager is very motivated to improve the quality of care provided to the service users.

What has improved since the last inspection?

The new Manager, UBU and staff at the home are addressing the requirements made at the last and previous inspections. Care planning, health care and training for staff have improved. Medication arrangements have changed but improvements are still required.

What the care home could do better:

Staffing levels at the home are being increased by UBU to cover busy periods. The needs of the service users are changing and staffing must be adequate to continue to meet their needs.The previous management and care provided has caused major concerns and the new Manager is committed to improving standards. The Manager`s management hours must reflect the current concerns and UBU`s commitment to bring about change in the home. The premises was clean and well maintained but some areas must be addressed.

The home does not provide nursing care. The home was purpose built in recent years and provides homely ground floor accommodation. It is situated in the centre of Wallsend and therefore close to all local amenities. It is immediately adjacent to No 2 Warwick Road which provides a similar service. Both homes are run by a private company, Northern Life Care, now known UBU, which also provides services in other parts of the country. 4 Warwick Road B53-B03 S358 4 Warwick Road V233333 200705 Stage 4.doc Version 1.30 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. The unannounced inspection took place on 20/7/05, between 9.30am and 2.30pm. The proposed Manager was on duty during the visit and has been in post for 3 months. Staffing levels were as agreed previously with the local authority inspection unit. Time was spent looking around the home to check the cleanliness, maintenance and decoration. The service users are unable to express their views about the home and at the time of the visit there were no visitors. Time was also spent observing the contact between the service users and staff. Two Care Plans for service users were examined. Staff described their approaches to caring for the service users. Training was discussed with the Manager. Arrangements for the administration and management of medication were audited. Good progress is being made to address the serious concerns raised at the last inspection. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 5 The home identifies the needs and aspirations of service users and the reviewed contracts will continue to protect service users. EVIDENCE: Two service user care plans were examined. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7,9 & 10 Care plans are detailed and identify the needs of the service users and actions to meet them. Risk assessments provide guidance to staff and protect service users, but one area requires assessment. Confidential information is secure but could improve in one area. EVIDENCE: Two care plans were examined. Staff discussed their approaches to caring for the service users and interactions between service users and staff were observed. Care plans are comprehensive and look at a full range of needs of the service users. They include information on the support package from the organisation and other agencies. Staff must consistently record that they have read and understood the care plan interventions. Communication with the service users is complex and the various methods developed by the staff are identified. Old care plans should be archived to prevent confusion. Limitations of choice to safeguard the service users are fully recorded. Risk assessments cover aspects of the service users’ lives but access to the kitchen should be assessed (see NMS 42). The Manager is working hard to address the concerns regarding care plans, health care and risk assessments. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13 & 17 Service users have access to a variety of activities both inside and outside of the home. Dietary needs of service users are well catered for with a balanced and varied selection of food available that meets service users dietary needs and tastes. EVIDENCE: Service users are supported to participate in the local community, for instance, visiting places of interest, swimming and discos. Staff have now received training in the use of an electric wheelchair so access to the local community with one service user is easier, although staff said the environment in Wallsend is not always easy to navigate. The home has a 3- week menu plan. A dietician has provided advice and checked the menus for their nutritional content. Advice is sought from eating and swallowing professionals. Appropriate food is in stock and the dietary needs of the service users are being met and monitored. Care plans detail any dietary needs and interventions. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 - 20 The health needs of residents are being met with multi disciplinary working taking place on a regular basis. The medication at this home is being managed but some areas need improving. EVIDENCE: The staff monitor service users health needs which are recorded in the care plans. Good improvements have been made since the last inspection. Routine health checks are carried out and service users attend appointments with relevant health care professionals with staff. Advice is sought from professionals and care plans reflect this. Medication arrangements have improved and a new monitored dosage system is in place. Staff have received basic Boots training on the new system but accredited training has been applied for. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 29 & 30 The areas inspected in the home were clean and safe but some areas require maintenance. EVIDENCE: The home has been decorated but some areas require repair. One bedroom has marks on the wall against the bed. The shower room has a damp patch due to problems with a roof leak. This is due to be repaired. The shower radiator is rusty. The corridor carpet is rucked and bleach stained. Bedrooms are homely and personalised. Occupational Therapists have reassessed equipment used by the service users. Infection control measures are in place with paper towels and hot water in bathrooms and showers. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33 & 35 Staffing levels are as agreed previously with the local authority inspection unit but must be reviewed to reflect the changing needs of the service users. Staff are receiving training which will improve the level of care to service users. EVIDENCE: The home maintains the level of staffing in accordance with previous agreements with the local authority. However the current levels of staffing which are a minimum of 2 care staff on duty during the day and 1 waking night staff with 1 person sleeping in are to increase. From 1/8/05 the staffing levels will increase to 3 care staff on duty between the hours of 10am and 6pm and at busy times. During the inspection the Manager and one carer who were the staff on duty struggled to deal with the needs of the service users as well as the day- today management of the home. One service user required emotional support 1:1 and 2 other service users require 1:1 supervision for personal care such as 4 Warwick Road B53-B03 S358 4 Warwick Road V233333 200705 Stage 4.doc Version 1.30 Page 17 feeding. The other service user received little stimulation due to the immediate needs of the others. The levels of staff must be kept under review by the Registered Provider UBU and the Manager so that they reflect the continuing and changing needs of the service users such as ageing, emotional and safety support Training in response to requirements and development have and are being provided by UBU. Courses include: Tissue viability, eating and swallowing, medication, continence management, rectal diazepam, protection of vulnerable adults, communication, challenging behaviour, risk assessment, menu planning and mandatory health and safety training. JUDGEMENT – we looked at outcomes for standard(s) 37, 39 42 The home is not being managed effectively to safeguard residents and to fulfil their potential. EVIDENCE: The new Manager has been in post for 3 months and is committed to improving the quality of life for service users. No 4 Warwick Road had, and continues to have, problems regarding the staffing, training, culture and care. The Manager currently works 20 hours as a carer and has 20 hours for management tasks. Due to the depth of work that this required to bring the home back up to standard the new Manager must be given the time to do this and develop into the role himself.
