The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 5 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

The home is very well managed with the team leader putting the wellbeing of residents first and foremost. The resident spoken to during this visit said he enjoyed living at 20 Newlands Close. It was evident from looking at care plans and discussions with the team leader the residents are part of the community, have many opportunities to go out and meet other people they feel safe and protected from risks of harm. The resident looked very relaxed in the bungalow, which is very homely. An enthusiastic team leader promotes the high quality of life for residents.

What has improved since the last inspection?

Negotiations between a resident`s family member are taking place because his financial situation, restricted independence and choice imposed by the family member. The resident now has an independent advocate to assist this process. The organisation EVH and the home came to the decision that it was not meeting the needs of a resident, and he was disrupting the lives of the other residents, and he was supported to move on.All documentation relating to staff and health and safety required by regulation are now in the home and are available for inspection.

What the care home could do better:

The home must address the offensive odours, which are now present throughout the bungalow. The previous resident was very heavy on the fixture and fitting within the home, the bedroom he occupied has been redecorated, but the lounge furniture was damaged and needs to be replaced. The home should have a registered manager in post; this was a recommendation from previous inspections.

The people accommodated will be aged over eighteen (18) and under sixtyfive (65) years on admission Date of last inspection 23 February 2005 Brief Description of the Service: 20 Newlands Close is a detached bungalow registered for three service users with learning disabilities. The home is part of the East View Housing Ltd. (EVH). The home as been extended and adapted to provide care for people with mobility difficulties. The accomodation consists of four bedrooms (one bedroom is used as the general office and staff sleep-in room) one bathroom with toilet, shower room and separate toilet. The communal areas are a large kitchen diner, lounge and a conservatory. There is a back garden that is used by the service users and there is an open frontage. The bungalow is situated in a quiet residential close, there are bus routes to the town of Hastings and there are two local shops in walking distance. 20 Newlands Close H59-H10 S21314 20 Newlands Close V231378 120705 Stage 4.doc Version 1.40 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This unannounced inspection took place between 15.00 and 17.00 with the inspector made very welcome to the home by the team leader. The inspector is familiar with the service hence a number of judgements were made from previous knowledge, confirmed by observation and talking with the service user at home and the team leader. A new member of staff was on duty when the inspector arrived, the team leader arrived shortly after, to undertake supervision. The home is managed by the team leader, and she intends to make application to become the registered manager. There are only two service users in residence at the time of this inspection, the third service user has now left because to home could not fully met his needs, and he had disrupted the home, distressed other service users and damaged fixtures and fittings. The service users wish to be referred to as residents within this report. What the service does well: What has improved since the last inspection? Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2 & 3 Prospective service users and their representatives are provided with a good level of written information about the service. EVIDENCE: A prospective service user would have access to suitable written information to inform them of the service and what it is like to live at 20 Newlands Close in the form of a statement of purpose and service user guide. Following the recent experience the team leader will ensure that the home can meet the prospective resident’s needs and they are compatible with the other residents so they all can live together, and have respect for each other. The previous resident was taken as an emergency, his was very active, younger and not as able as the other men and had very little in common. JUDGEMENT – we looked at outcomes for standard(s) 6 & 7 The residents’ care and support needs are well managed enhancing the residents’ independence and overall quality of life. EVIDENCE: During this inspection the individual care plans were not inspected in depth. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,13 & 16 The residents have every opportunity to have a fulfilled, responsible, valued and socially active life. EVIDENCE: It was evident from discussions with the resident at home and the team leader that they participate fully in the local community. One resident has 1:1 time with staff to explore his interests and involving more community presence. The other resident was at the day service during this inspection, but the inspector was assured he continues to enjoy his day service and other hobbies and interests, which includes the Nautical Corp that he attends weekly. He also has asked for support in finding a job. The inspector saw the weekly menus and the evening meal was a fish dish, which the team leader would be cooking later. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19 & 20 The residents’ health and care needs are very well managed and take into account service users individual preferences and needs. EVIDENCE: It was from observation during this inspection and the reviewing of records residents are treated as individuals and that this is respected in care giving and support. Health care is well managed with regular health checks undertaken with visits to chiropodist, dentist, optician and other health professional when required. Since the last inspection the home has purchased a new medication cabinet, No resident administer their own medication, therefore all medication is administered by the staff, and full training is given to all care staff on dealing with medicines. The home has regular Pharmacy audits, and the reports are usually positive. Further investigation are being undertaken into the incontinence of a resident, and the team leader must ensure that this is managed, as the home now has an offensive odour throughout and this not acceptable for the resident or for others living and working in the home. 20 Newlands Close H59-H10 S21314 20 Newlands Close V231378 120705 Stage 4.doc Version 1.40 Page 12 Concerns, Complaints and Protection The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 & 23 The residents would be able to raise concerns, and therefore are protected as far as possible from the risks of harm or abuse by an informed and trained staff team. EVIDENCE: All staff undertake training in the protection of vulnerable adults also the home has polices on the prevention of abuse. The team leader confirmed that the staff have read such policies, but their understanding of the policies were not explored with staff on this occasion. The inspector during this visit did hear a staff member speak to the resident in harsh and disrespectful way, this was feedback to the team leader who was aware of other incidents and intends to deal with the situation. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,25,26,27 & 30 The residents enjoy living at 20 Newlands Close it is safe, fairly well maintained and homely. But is now showing signs of wear and tear due to the heavy treatment from a previous resident. EVIDENCE: The inspector did a tour of the home escorted by the team leader, and found it was clean and warm in the living areas, but there was an offensive odour throughout the home. In general the home is well maintained but there are signs of wear and tear due to the heavy treatment of a the previous resident. The bedroom occupied by the previous resident has been decorated to a very high standard with matching curtains and bed linen. The shower area still is in needs deep cleaning to the tiles, floor and grouted areas. The residents enjoy sitting in the lounge watching television, but the suite of furniture needs to be replaced, due to the chewing to the arms and the ripping of the seating of the previous resident. 20 Newlands Close H59-H10 S21314 20 Newlands Close V231378 120705 Stage 4.doc Version 1.40 Page 14 Staffing The intended outcomes for Standards 31 – 36 are: 31. 32. 33. 34. 35. 36. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,34 & 36 The residents are supported and cared for by a competent team leader who is the acting manager. EVIDENCE: The team leader informed the inspector that the home had experienced staff shortages, but new staff have been recruited and the majority had settled well. The support worker who was on duty when the inspector arrived, told the inspector that she was new to the home but had many years experience working with people with a learning disability. However, her manner toward the resident during this inspection was sharp and disrespectful. The inspector informed the team leader, who also had had concerns regarding the manner of this staff member, and had come in early on this day to have supervision with the member of staff. It is therefore evident that the team leader is aware and vigilant and confronts situations that are not appropriate and not meeting the needs of residents. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 37,38,40 & 43 The residents are safe in the home and the staff are trained and supervised. The team leader is well motivated and competent ensuring the home is run well in the best interest of the residents. She is well respected by the residents and staff and it is evident that she has a management style that is open and approachable. EVIDENCE: The team leader has worked with this service user group for many years, and is currently working towards her NVQ 4 and then the Registered Managers Award. She will be making application to become the manager of 20 Newlands Close and make application to the CSCI to be the registered manager. The team leader demonstrated through discussion a very clear understanding of the current needs of residents. Monitoring health and safety in the home is to a good standard, with weekly health and safety checks taking place, and equipment serviced as required to maintain a safe home and facilities.
