Conditions of registration: 1 - The age/age range of the persons to be accommodated will be 18 - 65 years. 2 - Two named individuals may be accommodated who are 17 years of age. 3 - Of the ten service users accommodated two named service users currently in residence may also fall within the category MD Mental Disorder in addition to LD Learning Disability. Date of last inspection 27th September 2004 Brief Description of the Service: 3 Clareville Road, Caterham is a detached three storey house within the Regard Partnership Ltd developed to provide accommodation for 10 service users with with learning disabilities. The home is sited in a reaidential road with similar properties, and is within walking distance to local shops and amenities in the town centre of Caterham. Service users bedrooms all with wash hand basins are situated on the ground and upper floors. There is a toilet on the ground floor, and all bathing and showering facilities are situated on the upper floors. This would limit the homes suitability for less mobile service users. This is an overview of what the inspector found during the inspection. This was the first inspection to be undertaken in the Commission for Social Care Inspection year April 2005 to March 2006. The inspection was announced and was carried out on the 14th July 2005. The inspector in allowing sufficient time to get to Caterham had a good journey and arrived at the home at 8am. The Inspector is very grateful to the manager, staff and service users for the warm welcome, which was extended, and the invitation to commence the inspection an hour earlier than planned. The inspector was able to observe the hand over from the night staff to the day staff, which included the rigorous counting and signing for service users pocket money. The inspector was invited to view the recently (last night) finished snooze room that the night staff completed. This is truly a pleasant room in which service users will spend many calming and relaxed hours. The first part of the inspection was spent in discussion with the manager and deputy manager informing them of the new style reports, followed by reviewing a random selection of service users files, daily record books, care plans and medication records. This was followed by a review of care staff folders. The second part of the inspection was spent talking to service user and care workers and touring the home and gardens. Generally, the staff and service users spoken to on the day of inspection were aware of the inspection, were happy about the home and management and spoke about the home with pride. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1 2 The open management style of the manager empowers service users and care workers to have up to date and adequate information about the home. Arrangements are in place to ensure full assessment takes place prior to service users being admitted to the home. EVIDENCE: One service user told the inspector he was going shopping to buy a present for another service user, who was having a birthday party tomorrow, but it was a secret and the inspector should not let the service user know about the party. Review of randomly selected service users files demonstrated that pre admission assessment took prior to the service users being admitted to the home. This assessment is then followed by a more comprehensive assessment of care and social needs in which the service user, the relative or social worker and the key worker work together to plan and agree how to meet the assessed needs. The homes statement of purpose and service user guide needs to be updated in line with Care homes regulation 2001 Schedule 1. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 9. The home continues to encourage, support, and enable service users to make decisions and take risks in order to live active and full lives with the support of care workers as needed. EVIDENCE: Care plans included daily living activities of house keeping, personal care, shopping, bowling and other activities suitable for their age range. The observed close bond of the service users at Clareville road enables them to help and support each other with some tasks they might find challenging or do not particularly have an interest in doing e.g. dusting or vacuuming. The service users who have the skills and interest support each other in a companionable way. Care workers are always at hand to support and offer supervision to any service user who needs help whilst at the same time being mindful of the service users choice. The inspector observed good risk assessment being carried out for service users, but service users or relative did not sign these or social services to denote consultation had taken place and agreement reached. A requirement was issued on this standard.

Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 13 The home offers opportunities for the service users to make choices about their lifestyles, encourages and supports service users to have a wide variety of social contacts, which enhances their self worth and achievements EVIDENCE: The observed relationship between service users and care workers was relaxed and friendly, creating a warm and homely feeling. Service users were encouraged and supported to be as independent as they were able and they all appears to lead busy and interesting lives which includes attending college, day care, leisure activities shopping, swimming, bowling home time for skils development and church. One of the service users was packed ready to go home for the long weekend with her family. Evidence indicated that service users rights were only limited where risk were assessed as being unacceptable. Service users confirmed that care workers helped them to keep in touch with their relatives and friends.

Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 20 The home continues to offer individualised care to the service users to ensure that their needs are met in a person centred way. EVIDENCE: Care plans reviewed included clear guidelines on any intervention of support each service user required with personal care. Physical and emotional needs of the service users were also detailed in the care plans and daily records book, which included visits to the doctor, dentist and review of care on a yearly basis. The homes medication system including the policies is in place. EVIDENCE: The home has a complaint policy, which needs to be updated to include CSCI, address and telephone number. A requirement was issued on this standard. JUDGEMENT – we looked at outcomes for standard(s) 30 The home continues to offer a homely comfortable and safe environment for the service users. EVIDENCE: The home is clean and is maintained by service user supported by care workers. There is a sense of service users ownership of the home, and their individual bedrooms reflect their lifestyles and preferences. The service users keep their rooms clean and were happy to show their rooms and special items such as microwave (this has been risk assessed as safe) pictures photos TV and musical systems The toilets and bathrooms in the home were clean and well maintained and offered privacy. JUDGEMENT – we looked at outcomes for standard(s) 35 The home continues to employ a stable and efficient staff team who provide a competent quality of care to the service users. EVIDENCE: The staff team at Clareville Road are well supported by the managers and they have regular staff meetings to discuss the ongoing care needs of the service users and the home generally. Minutes of these meetings are recorded and documented. Regular supervision of care workers occurs and this is recorded in the care workers’ files. Care workers training and updating are up to date and certificates to support these training are included in their personal files. JUDGEMENT – we looked at outcomes for standard(s) 42 The management of the home is efficient and effective. The safety and welfare of the service users are promoted and protected. EVIDENCE: The Registered manager holds a nursing qualification and has considerable experience of working with vulnerable service users.
