What the care home does well

The home provides a high standard of person-centred care in a pleasant, tastefully decorated, homely and welcoming environment. The home actively promotes the health and well being of the service users and their independence is continuing to increase. There are very good staffing levels, recruitment practices and staff support. Staff training is of a high standard and staff morale is high. The home is well managed and the company keeps up-to-date with new concepts in best care practice. There are excellent systems in place for the home and the company to monitor progress and continue to improve. The home demonstrates exemplary practice in the way it manages challenging behaviour and builds on the strengths of the service users. The quality assurance systems within the Company and the home are excellent.

What has improved since the last inspection?

The home continues to maintain the environment well. It has again been redecorated in the latest styles, chosen by the service users with the support of staff. They have created a very tasteful and relaxed environment. The organisational structure has changed to give the manager more responsibility. This has improved the morale and relationships within the management team. The home continues to improve the already high standard of staff training. The service users have again made progress in a number of aspects of their lives. They are commended for the way in which they have joined in the programmes that the home runs and for their courage in facing their anxieties and fears.

What the care home could do better:

The home met, and often exceeded, the standards that were inspected at this visit. It has an improvement plan in place and is currently working to again improve the way it delivers person-centred care. Therefore, there were no areas for improvement noted during this visit.

Age range is 18 to 65 years Date of last inspection 12th September 2005 Brief Description of the Service: 33 St Johns Church Road is a residential home providing care and proactive support for up to three people with a learning disability and challenging behaviour. The registered providers are Blythson Ltd. The home is a large, late-Victorian town house situated close to Folkestone town centre and local amenities. It provides spacious and tastefully decorated accommodation. There are three large bedrooms, a spacious lounge, a dining room, breakfast room and kitchen. Outside there is a small front garden with parking space and a safe, sheltered, courtyard garden at the rear. The service users participate in a wide range of meaningful activities. The home maintains a consistent and positive approach to the management of challenging behaviour and the promotion of independence. The fees range between £1,500 and £1,860 and are based on the assessed needs of individual service users. 33 St John`s Church Road DS0000032438.V330402.R01.S.doc Version 5.2 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This unannounced visit lasted five hours. All three service users were at home and it was possible to spend time talking with them and observing their interaction with staff. Two members of staff were spoken to in private and indepth discussion was held with Amy Moffatt, the manager. Two relatives and two health and social care professionals also gave their views. Information received prior to the visit was considered and documentation, including care plans and staff files was examined. A tour of the home was made. The home meets, and often exceeds, the National Minimum Standards. All those consulted during this inspection gave very positive feedback. The manager, service users, their supporters and staff are all thanked for the welcome they gave and their assistance throughout this visit. All are commended for the progress they continue to make. This judgement has been made using available evidence including a visit to this service. The home provides the service users their relatives and supporters, with the information they need. Appropriate pre-admission assessments are made. EVIDENCE: The home has a statement of purpose and a service user guide that give very good information about the home. Each service user has a written contract in place. Staff and supporters of the service users understand the terms of residence and advocate on their behalf. The aspirations and goals of the service users are recorded in the care plans. They said that they can choose what they do and that the staff help them make choices. There is a rigorous pre-admission process that includes detailed assessment, visits to the home and discussion with relatives and supporters. EVIDENCE: The home is committed to person-centred planning. This means that individual needs and wishes are taken into consideration when planning care. Each service user has a care plan that fully explains how their needs will be met. The care plans are up-to-date and in good order. The home is currently working to further improve documentation to make it clearer for staff. Risk assessments are in place both for the environment and for activities. The service users are encouraged to take appropriate risks. Service users take part in day-to-day decision making within the home. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. This judgement has been made using available evidence including a visit to this service. The services users live healthy lifestyles and take part in a wide range of activities that they enjoy. EVIDENCE: Activity timetables and care plans show there are plenty of activities available. A tutor attends the home to help the service users with literacy and numeracy. There are lots of opportunities for outings to the local shops, sea and countryside. The service users help with household duties with the support of staff and some have hobbies that they enjoy within the home. They can also go on two holidays each year although not all choose to do so. One service user spoke very enthusiastically about the holiday she went on last autumn. It was good to see how very proud she was that she had overcome her fear of change to do this. 33 St John`s Church Road DS0000032438.V330402.R01.S.doc Version 5.2 Page 11 The service users interacted well with the staff on duty. The staff said that there are good levels of staffing to support all the activities. The home promotes healthy living and ensures that food is of a good quality. The service users work together to plan menus and shop for provisions. Staff support the service users to maintain a high level of personal care and all looked radiant on the day of this visit. It was good to see how they had all progressed in this area of their lives. They are all taking more interest in fashion; all were dressed well and fashionably. One service user has stopped biting her nails and now has beautifully manicured hands. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19 & 20 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The home promotes the health and well-being of the service users. EVIDENCE: Records show that personal issues are discussed in a sensitive way. Indirect observation confirmed that the service users are relaxed and confident in talking to staff. Inspection of care plans showed that health and emotional needs are identified and met. All specialist health care is accessed on behalf of the service users. They are supported to attend appointments at the hospital or local GP practice. The systems for the management of medicines in the home are good. The home listens to the views of the service users and their supporters and has sound systems in place for the protection of vulnerable adults. EVIDENCE: Observation of the interaction between the service users and staff showed that the staff are kind, attentive and understanding. The service users said that they could talk to any of the staff if they have any concerns. Relatives said that they could talk to the manager or any of the staff. They said they had no complaints, only praise, for the home. There have been no formal complaints since the last inspection. Day-to-day concerns are recorded and dealt with immediately. There are clear and comprehensive Adult Protection procedures. All staff receive training in the protection of vulnerable adults. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 25 & 30 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The home is very well maintained and has a welcoming and friendly atmosphere. EVIDENCE: A tour of the home was undertaken. All areas are tastefully decorated and furnished. Furnishings are of good quality and soft furnishings co-ordinate well with the décor. The home is well maintained and the bedrooms reflect the personalities of the service users. The home is decorated frequently. It is noted that the décor has changed at each inspection since the home opened. This is because the service users like to maintain a trendy and pleasant environment. They have chosen the colour schemes for their bedrooms and agreed the colours and fabrics for the communal areas. All areas of the home were very clean and sweet smelling on the day of this visit. 33 St John`s Church Road DS0000032438.V330402.R01.S.doc Version 5.2 Page 15 The registered providers undertake regular inspections of the home. Their reports confirm that they are rigorous in their insistence on a high standard of maintenance and cleanliness. They have a continuous improvement plan for the home and send very comprehensive reports to the CSCI. The home has a pleasant, safe and enclosed back garden. The company runs competitions between the homes in the group. In the summer there is a garden competition and in the winter there is a Christmas decoration competition. This judgement has been made using available evidence including a visit to this service. Staffing levels, staff training and recruitment practices are of a high standard. EVIDENCE: Staffing rosters show that there are good staffing levels at all times. Staff said that they have enough time to support the service users in activities. Staff were clear about their roles and responsibilities and said that they have plenty of training opportunities. A written record of this training is kept. Staff said that they love working in the home and that they receive good support and supervision from the manager and the registered provider. The service users have suggested questions that should be included in the interviews for new staff. This judgement has been made using available evidence including a visit to this service. The home is very well managed and the company is committed to selfmonitoring and best care practice. EVIDENCE: Amy Moffatt, the home manager, has only recently been appointed to the post. She has worked for the company for a number of years. As the previous manager, Kerry Castle, has moved to another management post within the company there has been an opportunity for a good handover period. Amy has been able to shadow another manager within the company and attend courses to support her management skills. She is currently undertaking the NVQ level IV and is in the process of applying for registration with the CSCI. Amy talked knowledgeably about care practice and is up-to-date with current thinking in 33 St John`s Church Road DS0000032438.V330402.R01.S.doc Version 5.2 Page 18 best practice. She has maintained an excellent continuing professional development portfolio and this was available for inspection during this visit. The office is well organised and all documentation is in order. All documents requested during this visit were to hand when requested and in order. However, it would help if names were printed under signatures. The providers visit the home often. They send regular, comprehensive reports about the home to the CSCI. There are systems in place for regular quality monitoring. On the day of this visit, Roland Evans, was in the home to support one of the service users during a review meeting. Staff and relatives say that they can talk easily to the manager and providers. There is an open and honest management culture in the home. They say that their views are listened to and acted upon when appropriate. Indirect observation showed that the manager interacts well with the residents and the staff.
