Conditions of registration: Date of last inspection 18.01.05 Brief Description of the Service: 5 George V Avenue provides long term care for four Service Users with learning disabilities. The Home is a large semi-detached property that has been extended. The Home is situated close to the local amenities and is on the bus route. The Registered providers live in the Home, contributing to the family atmosphere. Service Users attend a variety of clubs and activities. This is an overview of what the inspector found during the inspection. This was the home’s announced inspection. The inspection was conducted by one inspector and the duration of the inspection was 5 hours over one day. The Home representative was the registered provider. Additional time was spent in planning the inspection and report writing. All of the service users were out and involved in daytime activities away from the home. Time was spent speaking with the registered manager/provider of the home and looking around the premises. A partial tour of the premises was conducted, documents, medication and records were examined and service users files were case tracked. Three service users and one relative completed inspection comment cards. What the service does well: What has improved since the last inspection? What they could do better:
There are only a couple of minor issues which are to store external medicines such as creams, on a separate shelf to internal medicines (tablets and liquids) in the medicine cupboard. The nutritional assessment need to be reviewed when the service users needs are up dated every six months or sooner.

Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4,5 The statement of purpose and service users are excellent, providing service users with details of the service provided enabling an informed decision. The contract is produced in an appropriate format. EVIDENCE: The statement of purpose is regularly up dated and amended copies sent to the commission. It was assessed last year as containing all of the required information. The Service Users Guide has been produced in picture format and large print in simplified language. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7,8,9,10 The care plans are individual and provide information on each service users needs and how to meet them. Service users are supported to make choices and decisions about their everyday life. EVIDENCE: The Inspector saw and read Service Users individual files, these contained care plans, risk assessments and personal goals and achievements. Service Users are actively involved in the production of their individual files. Regular reviews are conducted. Service Users are actively supported to make decisions and choices. Any possible limitations are acknowledged and recorded appropriately. The daily records indicate that service users are actively supported to fulfil their wishes, with recognition of potential risks being recorded. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,13,14,15,16,17 Leisure activities are appropriate and individual. Daily routines are flexible. EVIDENCE: Individual goals are made with the Service User and progress made to achieving it is recorded. The service users attend day centres and clubs. The service users have an active social life, being actively involved within the local community. They are often invited to parties and events arranged by the local community. Outings and activities are planned around personal and group preferences. The registered provider explained how links with family and friends are maintained. Records were seen to support this and also included evidence of additional professional support being sought for service users when appropriate in developing personal relationships. The daily routines are flexible and individual. Mealtimes and menu remain flexible to allow for personal preferences and changes in social activities. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19,20,21 The health needs of service users are well met with evidence of good multi disciplinary working taking place. EVIDENCE: Through the regular observations and assessing of health care needs by the registered persons, service users have received the appropriate treatment when required. Records provided evidence of accessing a variety of healthcare professional when appropriate and needed. The overall medication practices are good with the only issue being raised was the storing of internal and external medicines together. The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22,23 The home has a satisfactory complaints system and evidence that service users views are listened to and acted upon. EVIDENCE: The home has produced its complaints procedure in three different formats to meet the individual needs of the Service Users. No new complaints have been received. The Registered Persons acknowledge all concerns raised however small. The homes Adult Protection procedure has recently been reviewed and the registered persons have attended Adult Protection training. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,25,26,27,28,29,30 The building is well maintained providing a homely environment for service users. The home was clean and free from offensive odours. EVIDENCE: A partial tour of the premises was conducted; the registered provider continues to strive to improve the standard of accommodation provided. There is a maintenance programme identifying both small and major tasks to be undertaken. The home provides a large communal room, which is a lounge and dinning room, providing more than the minimum space of 4sqm per Service User. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,34,35,36 Sufficient care hours are provided by appropriately trained carers. EVIDENCE: Currently the registered providers work full-time to provide the care required. They also live within the home. The lifestyle structure is similar to a family unit. Two volunteers and the other two registered providers provide some added support and relief. JUDGEMENT – we looked at outcomes for standard(s) 37,38,39,40,41,42,43 The home is well managed with policies and procedures used to support practice and quality assurance procedures to assess and maintain standards. EVIDENCE: The registered providers have completed the registered managers award. Skills and knowledge gained through this training is reflected in their management style. They are organized and thorough in maintaining high standards in every aspect of the home. The views and wishes of the service users are considered at all times and valued at all times.
