Conditions of registration: Adults with a learning disability aged 18 -65. Number six (6). Date of last inspection 25th January 2005 Brief Description of the Service: 54 Somerville Gardens provides care for up to six adults with learning disabilities. The premises consist of a large detached house in a residential street in Leigh on Sea. There are five bedrooms on the first floor and one on the ground floor. There are plans for one service user to move to the large downstairs office and the office to be moved upsatirs. There is a bathroom and toilet ion the first floor, and a shower and toilet on the ground floor. There is a large kitchen and living/dining room. The garden is adequately sized. The home is within easy walking distance to the local shops and public transport. The home has pleasant sea views. This is an overview of what the inspector found during the inspection. The inspection was a routine unannounced visit to the home and took place when three residents (service users) and two staff were available. The visit lasted throughout the morning and lunchtime period. The house was toured which included the living/dining room, kitchen, bedrooms and two people living in the home were chatted with. The registered manager and another member of staff were on duty. What the service does well: What has improved since the last inspection? What they could do better:
The home needs to carry out a good method of getting views of all people involved at the home including residents at the home, staff, GP, Nurse, day service staff, for example via a Quality Assurance system. They should make sure all residents at the home are given a copy of a new contract – these have up to date information relating to room occupancy, care planning and so on and have relevance to the National Minimum Standards that are expected in a Care Home for Adults with Learning Disabilities.

The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 4 and 5 54 Somerville Gardens provides effective information for prospective service users to decide whether the home can meet their needs. Assessments are detailed and reflect the needs of the service users. EVIDENCE: The home provides information to prospective service users about the services and facilities available at Somerville Gardens. One person interested in moving into the home has already visited the home. They have gone for a meal with one service user and one staff. Service users have been consulted on their opinions of the prospective resident and whether they think the person could move in. A revised statement of purpose reflecting that there is a service user aged over 65 has been developed and sent to the CSCI, although the Manager was unable to locate this during the inspection. All service users living at the home have been provided with a statement of terms and conditions called a Licence Agreement although none of these reflect the new National Minimum Standards for adults in care homes. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 8, 9 and 10 The home effectively supports service users to control the decisions in their lives. Service users are encouraged to maintain their independence within an effective risk managed process. Paperwork is up to date and relevant. EVIDENCE: It was pleasing to note that the home demonstrated that service users are involved in all aspects of the running of the home. The home has secured the services of an advocate to support service users make decisions in their lives. This includes one service user who is planning to move to a downstairs bedroom. The plan to enable a prospective service user to move in has included the views of the current group of resident service users being taken into consideration. One service user spent time presenting their person centred plan for inspection and discussing the contents showing personal photographs of family and of them enjoying various activities. This service user discussed the process as the ‘keyworker sits with you and talks you have a cup of tea and write down likes and what you can do’. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11, 12, 13, 14, 15, 16 and 17 The home effectively enables service users to maintain appropriate and fulfilling lifestyles. Nutritious and varied meals are available. Appropriate paperwork is in place and well kept. EVIDENCE: Service users at the home attend formal day services and adult education classes to study subjects such as reading, writing and painting for example, if they wish. Staff at the home support their attendance should this be required. One service user spoken with stated they enjoy socialising at a local PHAB club, going out for lunch and is on the waiting list to start horse riding. Family and friends are made welcome and relationships are encouraged and supported. Meals provided are of good quality and relevant records are kept. The menus are devised on a daily basis depending on service user choice. Special diets are well catered for. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19 and 20 54 Somerville Gardens provides good personal and healthcare support to individuals living at the home. Where possible service users are supported to maintain independence and autonomy regarding this. Paperwork to evidence good work by staff is well maintained. EVIDENCE: Documents held for each service user was well kept and showed that the home supports the service users with their personal and healthcare needs, this includes documenting health care appointments and of any changing needs such as progressing confusion or dietary advice for example. It was clear that staff at the home are aware and alert to any changes in service users healthcare needs and contact the relevant people to assist such as social workers and advocates. The home has a key worker system in place and service users can choose the member of staff where practicable. Two service users are encouraged to sign their own medication administration record (MAR) sheets but none currently retain their medication. MAR sheets were well maintained. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 and 23 The home responds well to service users comments and assures service users that they will be safeguarded from harm, abuse or neglect. EVIDENCE: The home maintains a complaint log. A recent complaint made by a service user has been resolved satisfactorily. No complaints have been received by the CSCI. The Manager has recently attended training in protection of vulnerable adults and other staff are booked to attend. The Manager stated she was confident that staff were aware of signs and symptoms of abuse. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 25, 26, 27, 28, 29 and 30 The home provides sufficient space and facilities to meet service user needs. EVIDENCE: The home is presented as clean and attractive. Service users are encouraged to decorate and furnish their rooms according to their own wishes. There are plans to change the downstairs office into a bedroom and move the office upstairs. There are a few alterations the home wishes to put in such as a wash hand basin and move the call system upstairs. This change will enable staff to have their own washing and changing facilities. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32, 33, 34, 35 and 36 The home provides and maintains an effective staff team, which demonstrates the ability to meet the stated purpose of the home and meets the needs of the service users who live at the home. EVIDENCE: The core team of staff have worked at the home for some time. One new member of staff started at the beginning of June. Paperwork for this person was in good order. Whilst the new member of staff has had no previous experience, they appear to have settled in well and is motivated and booked on training. Supervision of staff takes place every six weeks. The manager confirmed that she also receives supervision. Training opportunities are good and staff are booked onto various courses such as first aid, people moving people loads, epilepsy and fire safety for example. Service users benefit from the ethos, leadership and management approach of the home. Service users are confident their views underpin all self-monitoring, review and development by the home. Service users’ rights and best interests are safeguarded by the home’s policies and procedures. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 37, 38, 39, 40, 41, 42 and 43 The Manager and staff ensures that the home continues to fulfil its stated aims and objectives. The general running of the home is inclusive and based on the needs and wishes of the service users. Policies and procedures are explicit of the organisations expectations of the management of paperwork. The home has not undertaken gathering the views of all interested people in the conduct of the home and service delivery. EVIDENCE: It was clear that the home is well run and that health, safety and welfare of the service user is maintained. Records are well maintained on each individual service user reflecting visits to health care professionals, for example. Good safety practices were apparent such as clear fire exits, safety signs and relevant documents well maintained.

This has not included the views of interested parties such as family members or the service users day service provider and health care professionals for example. The Manager explained that she is in discussion with a senior Manager within the organisation and that a quality review will be carried out in the near future with a report of the findings made available.
