The house stands in its own private garden and is situated close to all local amenities. This is an overview of what the inspector found during the inspection. This unannounced inspection was conducted on 12th August 2005 with an additional visit to the home on 20th September 2005. What the service does well: What has improved since the last inspection?

The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4,5 The homes Statement of Purpose and residents contract provided residents and prospective residents with details of the services provided. The assessment procedure ensured that only those whose needs could be met would be admitted to the home. EVIDENCE: There had been no recent admissions to the home however examination of case files indicated that, prior to admission, a comprehensive multi-disciplinary assessment had taken place to ensure the home could meet the needs of the prospective resident. It was apparent that consultation with relatives had been an important part of the pre-admission process and that careful matching had taken place to ensure that all persons living in the house were compatible. The manager said that any future admissions would be preceded by an indepth multi-disciplinary assessment as well as assessment of need conducted by staff from Ormesby Road. He stated the prospective resident and his/her family would be invited to visit the home to meet current residents and staff. Overnight stays would also be encouraged, followed by a trial period that would give time for the new resident to decide if they liked the home and wished to live there. JUDGEMENT – we looked at outcomes for standard(s) 6,7,8 The homes care planning process ensured resident’s needs were identified and met. Residents were consulted about all aspects of their life and were supported to lead an independent lifestyle, within the context of their individual plan. EVIDENCE: Each randomly selected care plan was well organised and provided a comprehensive, up-to-date and detailed picture of the resident and his/her needs, aspirations, likes and dislikes. Assessments had been carried out and action plans devised to cover all areas of daily living and personal need, including social need. Regular reviews and constant monitoring and reassessment ensured changing needs were identified and met. The level of disability of residents living in the home severely limited their active participation in the care planning process or in the day-to-day running of the home. However it was evident, from examination of care plans and associated documents and through conversation with the manager and staff, that all residents were supported to make choices and to be involved in the decision-making process at an appropriate level. Staff encouraged them to take part in simple daily activities, e.g. tidying their room and helping to make their bed. Where residents were unable to verbally communicate, the

Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,13,14,15,17 Residents were presented with opportunities to lead fulfilling lives and were encouraged and supported by staff to take part in appropriate leisure activities in the home and the local and wider community. Staff encouraged and assisted residents to maintain family and friendship links. EVIDENCE: Care plans and daily records revealed that residents were encouraged and supported to achieve maximum independence, subject to their individual plan and risk assessments. The plans showed how they were presented with opportunities to maintain and develop their social, communication and independent living skills. Specialist support was given by: speech therapist, physiotherapist and other appropriate professionals when required. All of the current residents attended daytime activities at a variety of venues: Resource centre in Middlesbrough, Day Care Centre in Skelton, Church hall in Ormesby. A wide range of leisure activities was offered, with each resident having a programme of activities suited to their individual abilities and wishes. In-house activities such as TV, video and music were available to residents.

The house car enabled residents to enjoy the facilities offered in the wider community and frequent visits were made to places of local interest such as parks, the coast, theatres, town centre shops etc. Staff recognised the importance of residents maintaining family and friendship links and helped them to keep in touch by assisting with telephone calls, sending cards for special occasions such as birthdays and Christmas. Although no relatives visited the home during the inspection, staff said that family members maintained regular contact, were frequent visitors and showed a keen interest in the progress of their family member. The manager and staff said that relatives and friends were encouraged to visit and were always made welcome at the home. Menus and the record of food served showed residents were offered a varied and balanced diet. Alternatives were available should someone not wish to have the meal of the day. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19,20,21 Healthcare and personal needs were met by staff who provided support in a sensitive and flexible manner in accordance with the wishes of the individual resident. Appropriate healthcare professionals provided advice and support. EVIDENCE: Conversation with members and staff and examination of care plans and daily records indicated awareness of the importance of providing personal care in a sensitive and flexible manner, and of consulting with residents and supporting them to maintain as much control over their own lives as possible. Constant monitoring of health was undertaken and healthcare needs addressed by residents own general practitioner and other community based professionals e.g. community and specialist nurses, dentist, chiropodist etc. None of the residents had been assessed as being able to control their own medication. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,25,26,27,28,29,30 The environment was good, providing residents with an attractive, comfortable and well-maintained place in which to live. EVIDENCE: Number 58 Ormesby Road was originally a lodge for the nearby Normanby Hall. The building has been converted and extended so that it now provides comfortable and pleasant accommodation that is conveniently situated for all local amenities such as shops, public houses, churches and the public transport system. The internal and external fabric of the building was maintained in good condition, décor throughout the house was pleasant and cheerful. Furniture was domestic in design, suitable for purpose and in keeping with the character of the property. All necessary areas of the building (including the kitchen) were accessible to residents, subject to individual risk assessments. Communal areas were light and airy. Bedrooms were comfortably and appropriately furnished and had been individualised by the inclusion of personal effects such as pictures, photographs, ornaments, TV, CD player etc. One bedroom had an en-suite facility, all other rooms had a wash hand basin.
58 Ormesby Road B51-B01 S121 58 Ormesby Rd V244382 120805 Stage 4.doc Version 1.40 Page 17 No specialist disability equipment was required to meet residents current needs, however constant monitoring and reviews ensured that any change in their physical needs would be recognised and appropriate action taken to ensure they could be met. All areas of the home were centrally heated and radiators had been covered with suitable guards to ensure a low surface temperature. Hot water outlets accessible to residents had been fitted with pre-set valves to provide safe water temperatures. First floor windows had been fitted with restrictors. Lighting was domestic in nature and emergency lighting had been provided throughout the home. The numbers and suitability of lavatories and bathing facilities met the National Minimum Standard. The home presented as being clean, hygienic and free from offensive odours. It was observed however, that the floor covering in the ground floor bathroom was stained – this should be replaced. JUDGEMENT – we looked at outcomes for standard(s) 32,33,34,35,36 The home has a robust recruitment procedure, an effective training programme and all staff received good management support and supervision. EVIDENCE: Three members of staff were interviewed during the inspection; each spoke enthusiastically about their work and said they received good management support and training to enable them to meet resident need. They confirmed that prior to commencing employment they had been required to attend an interview and provide references and evidence of any qualification gained. They said that they had not been allowed to start working at the home until a satisfactory Criminal Records Bureau check had been undertaken. Each displayed a suitable knowledge of individual residents, their needs and how they could be met. Examination of personnel records and staff interviews confirmed that prior to commencement of employment staff underwent an external four-day induction course.
