Conditions of registration: Date of last inspection 5th September 2005 Brief Description of the Service: 93-95 Canning Road is a registered care home providing personal care and accommodation for a maximum of 6 adults aged 18-65 who have learning disabilities. There were 6 service users living in the care home at the time of the inspection. The Registered provider is the Harrow Consortium for Special Needs and the Responsible Person is Mr. Robert Seaman. The Registered Manager is Mr. Ross Hensman. Paddington Churches Housing Association (that takes responsibility for its maintenance) owns the building. The care agency is Harrow Mencap that employs the staff in the care home. The home is located in a quiet residential road in Wealdstone on the outskirts of central Harrow. It is close to shops, pubs, transport and other community amenities. All the home’s bedrooms are single, and none have en-suite facilities. This is an overview of what the inspector found during the inspection. The unannounced inspection commenced at 8.15am on a dull Tuesday November morning. There was one member of staff on duty in the care home and another arrived at the same time as the Inspector. The Inspector was pleased to meet and speak with all the residents. Some residents spoke at some length about current issues and aspirations for the future. Generally satisfaction was expressed with their lives, the support and accommodation available in 93-95 Canning Road. One resident wished a number of issues to be raised with a member of staff and Mr Hensman. Staff suggested ways to the resident in which the issues could be addressed. The Inspector viewed communal areas of the building and noted a significant number of requirements. The Registered Provider must ensure that the care home is adequately maintained. The Inspector was made most welcome and would wish to thanks those in the care home for the assistance received. What the service does well: What has improved since the last inspection?

The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7 & 9 Residents have care and support plans in order to ensure that a consistent service is available to them in 93-95 Canning Road. Residents are supported to make decisions by staff in the care home. Residents are supported to take risks within a risk management culture. EVIDENCE: Both standards 6 & 9 were assessed at the previous unannounced inspection. Staff had been working on the care and support plans with a review having been carried for most residents. Key workers meet with individual residents on a monthly basis with a record maintained of outcomes. Mr Hensman stated that work was being done on the care plans to rationalise the information they contain more easily accessible. Certainly the care plans in their current form presented as fairly muddled. All residents have named key workers from within the staff team in 93-95 Canning Road. 3 residents kindly advised the Inspector who their key workers were. One resident kindly stated that they had attended a review meeting in the care home. Care plans include information about the residents’ decisions and choices.

These conversations subsequently involved staff and the Care Manager at the residents’ request. Staffs’ response was to listen to the residents’ decisions and offer advice on possible action plans. Staff spoke about residents’ ordinary living routines and choices. A sample of individual and work place risk assessments were viewed. Most had recently reviewed as required. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 15 & 16. Residents are supported by staff to take part in a range of individual activities both within the care home and the community. Residents have formed their own friendships both inside and outside of the care home and also have family contacts. Staff present as listening to residents and offer support to enable them to make choices. EVIDENCE: On the day of the unannounced inspection 5 residents went out to various day placements. One resident was supported (by staff) to access chosen facilities in the local community. Some residents spoke positively about their day placements and the sort of things they do there. One resident spoke about the clubs that they attend in the evening. One resident spoke about a holiday that they had had earlier in the year. This was not an experience that the resident wished to repeat. One resident kindly showed the Inspector some educational certificates that they had just received in the post. Some residents spoke extensively about their plans for the future.

One resident was receiving a visit from a relative on the day of the unannounced inspection. A member of staff was heard knocking on a resident’s bedroom door before entering. One resident indicated that they could lock their bedroom door. Staff were observed in conversation with residents during the unannounced inspection. There was discussion with residents with about their plans for the coming day. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 20 Staff enable residents to access preventative healthcare services in the community. Residents are largely protected by the home’s policies and procedures for dealing with medication. EVIDENCE: Staff maintain records of the various healthcare appointments attended by residents. One resident spoke about seeing their GP. The care home’s medication systems were viewed during the unannounced inspection and outcomes discussed with Mr Hensman. Their contracted pharmacist supplies a blister pack system of medication administration to the care home. Mr Hensman spoke about plans to work with residents to enable them to self medicate, subject to risk assessment. There is a medication cupboard that presents as quite small. It did, however, contain stocks of some repeat items that had been stored since April 2005 and needed to be returned to the pharmacist. It must be ensured that unused items of medication are returned to the pharmacist within the prescribed guidelines. A record is maintained of medication administered to residents. This was largely fully recorded though there were some signatures missing. This was discussed with Mr Hensman. A full record must be maintained of medication

There were recorded examples of each member of staff’s signatures that administers medication to residents. This was to be updated. There is written guidance for staff on the circumstances in which medication is to be administered to residents on a PRN basis. Mr Hensman indicated that all staff have done medication training. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 23 Residents are largely protected from abuse by the care home’s policies and staff training. EVIDENCE: There remains an outstanding requirement regarding the care home’s ‘Complaints Procedure’. Staff advised the Inspector that they had undertaken training in Protection of Vulnerable Adults. The Inspector noted that there was detailed policy guidance on the office notice board for staff on how to deal with Protection of Vulnerable Adults issues. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 & 30 Residents’ are offered accommodation in 93-95 Canning Road that is largely (but not entirely) comfortable, adequately maintained and safe. Residents’ accommodation in 93-95 Canning Road is largely clean and hygienic. EVIDENCE: The unannounced inspection included all the communal areas of the home. A number of requirements remain outstanding since the last unannounced inspection. Requirements noted are recorded below. The desk drawers in the office were observed to be broken. The building was found to be warm, clean and tidy with a lived-in feel. The care home offers residents a large lounge and kitchen/dining room that are light and bright. The home was found to be largely free of offensive odours. Furnishings and fitments largely presented as in good condition. Staff were undertaking cleaning tasks during the unannounced inspection. The building is purpose-built and in keeping with others in the immediate vicinity. Laundry facilities are located on the first floor. Washing does not need to be brought through areas where food is stored, prepared, cooked and served. A window was open to ventilate the laundry. It contained washing and drying

Areas of flooring, however, were quite dirty in the corners that were hard to clean. The inside of the sink unit was very dirty and this may indicate that the dryer venting is not working. JUDGEMENT – we looked at outcomes for the following standard(s): 32 & 35 Residents are supported by a full staff team that, during the inspection, presented as competent and qualified. EVIDENCE: Records indicated the range of training undertaken by the staff team in 93-95 Canning Road. This includes induction and medication training. The Inspector observed staff talking with residents about a range of ordinary living issues. A recruitment and selection policy was viewed within the care home’s documentation that indicated that all the required checks are carried out on applicants for employment within 93-95 Canning Road. JUDGEMENT – we looked at outcomes for the following standard(s): 27 & 39 Residents’ views have not yet been sought in order to review the quality of their care and support in 93-95 Canning Road. Residents’ health and safety are not fully protected in 93-95 Canning Road. EVIDENCE: There remain outstanding requirements in respect of Standards 39 & 42 from the previous unannounced inspection. The Inspector issued an Immediate Requirement Notice in respect of the necessity of the care home having a Certificate of Worthiness for the electrical installation and hard wiring. The Inspector learnt that the care home’s electrical installation and hard wiring had failed the previous test with necessary remedial work to ensure the safety of the building not having been undertaken subsequently.
