What the care home does well

A relaxed homely feeling, it is clear that the Residents are in control of the running of their lives in all areas, but require minimal emotional support from the Manager and staff who visit daily, this is mainly to reassure them that they are managing well.

It is owned by Care Management Group Ltd, a large organisation owning several homes elsewhere in the country and a number in the immediate locality. It provides accommodation for three service users, each in a single room. There is a garden to the rear and limited on-road parking to the front. The home is not staffed on a regular basis, as the service users are very largely self-caring. It is supported by the manager and some staff from a nearby home. The home functions as a supported living establishment, rather than as a care home. 99 Tonge Road H56-H05 S23797 99 Tonge Rd V221576 070605 Stage 4.doc Version 1.30 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. Graham Cummings carried out an Announced Inspection on Tuesday 7th May 2005. The Inspector arrived at 09:45 and left at 12:35. The Pre Inspection Questionnaire had been fully completed and returned to the Inspectors office prior to the Inspection. The Inspector met with the Manager Maggie Carter and spoke to the 3 Residents living at the home and saw documentation. All 3 of the Residents have being living at the home since it was opened 17 years ago and prior to this they were living together at another home now owned by Care Management Group. The 3 Residents are all able to live semi independently and their individual skills allow the home to run smoothly. There are no staff employed specifically to work at 99 Tonge Road but the home is overseen by the Manager and staff from another home owned by CMG less than a mile away, on average the input is 25hours a week. All 3 Residents have mobile phones and have the numbers of the home just up the road, the Managers mobile and home number for emergencies, this does not appear to have been abused by the Residents. The atmosphere was relaxed and the 3 Residents were encouraged to be open and honest by the Manager. At some point during our discussions all 3 Residents expressed their wish to continue living at the home, one Resident said that ‘they were very happy there’ another said ‘they enjoyed living at the house’. The Residents also commented on the manager and staff who support them, one said ’we have a marvellous Manager’ ‘been there when we need her’ another said ‘ she kicks butt when we need it’. The 3 Residents have individual timetables, which were seen by the Inspector, these had all been discussed and agreed with the Residents. Paperwork relating to fridge /freezer temperatures, pest control, water temperature, fire alarm tests was seen, one of the Residents had taken responsibility for keeping the records up to date. JUDGEMENT – we looked at outcomes for standard(s) 4 Prospective Residents visit the home prior to placement. EVIDENCE: Although no placements have been made at this home, the Residents living here have been involved in meeting and speaking to prospective placements that are being made at the home that oversees 99 Tonge Road. JUDGEMENT – we looked at outcomes for standard(s) 7,8 Each Resident is aware of their role and responsibilities in the home. The Residents know their independence will be maximised and promoted. Residents know that their goals and aspirations will be supported by the home. EVIDENCE: Residents are responsible for their daily decisions and routines, the menu is the responsibility of the Residents, they have also worked out a weekly house rota for cooking, shopping and times for using the bathroom in the mornings. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,13,14,15,16,17 Links with the community are good and support and enrich Residents social and educational opportunities. The Residents run their home and are respected and their responsibilities recognised in their daily lives. The meals in this home are good offering both choice and variety. EVIDENCE: The Residents have become part of the local community having all lived there since the home opened 17 years ago, they are regular visitors to the local pubs and shops. One of the Residents has worked at Age concern for 7 years and is also head gardener at the main home, another Resident has been working for the Red Cross and recently been promoted and is now able to handle money and be left on the till, the third Resident works at the home just up the road and gets paid for domestic work. All 3 Residents attend college for different courses. Hobbies include sewing, crochet, knitting, attending the Salvation Army, working on computers and watching soaps on TV. The Residents have agreed to put £100 each and the Home is going to put £300 to it to purchase a computer for joint use in the home, everyone is looking forward to this. Activities include going to the cinema, swimming, visiting the pub and bowling. All 3 Residents go on a yearly holiday to Minorca where they rent a villa with a 99 Tonge Road H56-H05 S23797 99 Tonge Rd V221576 070605 Stage 4.doc Version 1.30 Page 11 pool and all modern facilities, they have been going for the last 5 years and have become confident enough to go out in the local area by themselves. Two of the Residents have close Relationships with people from the main home and they visit 99 Tonge Road for meals and sometimes for the day at weekends. All 3 Residents have family members who they contact when they want and one Resident usually goes to their Mothers house for a weekend every month. A four week rota was seen and offered a varied and nutritious diet, the menu was chosen by the Residents and they do their own shopping and cooking. One Resident expressed a wish to stay at the home and not move on to individual living as she enjoyed living with her friends. 99 Tonge Road H56-H05 S23797 99 Tonge Rd V221576 070605 Stage 4.doc Version 1.30 Page 12 Personal and Healthcare Support The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19,20 Residents do not require personal support. Residents physical and emotional health needs are met. The medication at this home is well managed promoting good health. EVIDENCE: Residents living at 99 Tonge Road do not require any personal support and are able to support themselves with minimal encouragement and prompts. All 3 Residents spoken to were happy and appreciative of the support given to them. The input from the Manager and staff who visit the home is mainly for emotional support and encouragement and not personal or physical support. The Residents carry out all of the daily living tasks themselves and are proud of their achievements. One Resident medication is kept in the main home and the Resident signs on a weekly basis for her medication which is then kept in a locked cabinet in the Residents room. No other medication is kept in the home and if Paracetamol or Aspirin are required these are administered from the main home. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22,23 Residents views are listened to and they are protected from abuse, neglect and self harm. EVIDENCE: Residents informed the Inspector that they live happily together with few arguments or disagreements; they said that they felt that there were no more than a normal family would have. The Manager said that the Residents sorted out their disagreements and she was usually told about them after the event. The Residents do have access to the main home and the Managers home and mobile telephone numbers should they ever require advice or assistance. It was clear throughout the Inspection that residents were able to voice their own opinions and ideas that the Manager responded to in an appropriate manner encouraging them to think through their suggestion and coming to a conclusion. The Residents have all attended a Health and Safety course and have attendance certificates for this. One Resident has taken on the Responsibility within the home to carry out weekly Health and Safety checks, these are recorded and were seen by the Inspector. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,30 The standard of the environment within the home is good providing Residents with an attractive, clean and homely place to live. EVIDENCE: The home was well decorated and furnished. The lounge had ample seating for the Residents and visitors. The Residents, as part of their daily responsibilities keep the home clean and tidy. JUDGEMENT – we looked at outcomes for standard(s) 35 The Manager and Staff have a good understanding and training to meet the needs of the Residents. EVIDENCE: The Inspector noted the positive relationships that have been formed between the Manager and Residents. The Manager and Staff who supported 99 Tonge Road training records were seen, they included: Food Hygiene, Fire, Manual Handling, Medication, Abuse in the Care Home, Legislation change, Care Planning, Health and Safety and NVQ’s levels 2 and 3. Service users are confident their views underpin all self-monitoring, review and development by the home. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 37,38,39,42,43 Residents benefit from a well run home. The systems for Residents consultation are good and Residents views are sought. The Care Management Group review aspects of its performance through a programme of consultations with Residents throughout the company. EVIDENCE: The Residents run the home themselves covering all aspects of daily living, the routines and responsibilities have been agreed and shared between them. Throughout the inspection, discussions with the Manager and 3 Residents all corroborated the fact that the home is run by the Residents for the Residents. The Care Management Group last year sent out a questionnaire to all Residents living at the Companies homes and the results have recently been published they covered areas such as choice, dignity, staffing, environment and more. The Inspector was satisfied that the Residents health, welfare and safety is promoted through regular meetings and discussions with them. The Residents benefit from a Manager who is competent and well liked and respected by them, comments from the Residents included ‘we have a marvellous Manager’ 99 Tonge Road H56-H05 S23797 99 Tonge Rd V221576 070605 Stage 4.doc Version 1.30 Page 17 and ‘she’s been there when we need her’.
