This inspection was carried out on 19th July 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Abbey Dean is a homely, well-maintained home with attractive grounds. A good variety of activities are provided for residents to take part in if they wish. Service users feel well cared for and say that the food provided is good.

What has improved since the last inspection?

An extension adding two bedrooms has been completed since the last inspection. The registered providers are in the process of applying for a variation to their registration for these extra rooms. Locks have been fitted on bedroom doors.

What the care home could do better:

Staff records need to include all the information in Schedule 4 of the Care Home Regulations, and newly appointed staff should not begin work until an enhanced CRB and POVA check and two written references have been received to ensure that service users are supported and protected by the homes recruitment policy.

The building is semi-detached and the accommodation is provided on the ground and first floor. Service users occupy single rooms which have en-suite facilities with a toilet and handbasin. A passenger lift is provided to the first floor. There is a lounge, conservatory and dining room on the ground floor. There are well kept gardens with lawns and walkways that are accessible to service users. Mr and Mrs Dean own Abbey Dean. Mrs Dean is the registered manager responsible for the day-to-day management of the establishment. The unannounced inspection was carried out over a period of two and a half hours. The inspector read information held on the service file since the last inspection in February 2005, and read the previous two inspection reports. During the inspection the inspector spoke to all the service users, and two members of staff. The inspector undertook a tour of the premises and looked at four care plans and three staff files. Various record books, policies and procedures were also examined. The registered provider and manager, Mr and Mrs Dean, were on holiday on the day of the inspection and therefore unavailable. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 Service users are assessed prior to admission. Abbey Dean does not provide intermediate care therefore standard six is not applicable. EVIDENCE: Service users’ files inspected held pre-admission assessments that had been used to inform care plans. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 10 The service user’s health, personal and social care needs are set out in an individual plan of care. The health care needs of the service users’ are fully met and service users feel they are treated with respect and dignity. EVIDENCE: Service users’ files included comprehensive assessments covering physical and mental health, falls and nutritional screening. These assessments along with the pre-admission assessments form the basis of individual care plans. Care plans are signed by service users and reviewed every four months. Staff were witnessed treating with service users with dignity and respecting their privacy. Service users said that staff use their preferred names. All bedrooms have telephone points to enable service users to make and receive calls in private. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 Routines of daily living are flexible. Service users are able to maintain contact with family, friends, representatives and the local community. They are also able to exercise control over their lives. EVIDENCE: Abbey Dean provides a wide choice of activities including keep-fit, flower arranging, garden club and trips out in a minibus. Service users said they are able to choose which activities they take part in. A list of activities and services at the local church was seen on display. Service users said they are able to choose the clothes they wear. They also said that an alternative meal is provided if they don’t like what is on the menu. Visitors are welcome in the home and are able to have a meal with their relative if arranged in advance. Service users confirmed this and said they are able to entertain visitors in the privacy of their own room. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,26 Abbey Dean provides a safe, well-maintained environment that is clean, pleasant and hygienic. EVIDENCE: On the day of the inspection the home was seen to be clean, homely and free from offensive odours. Decoration was in good order and furnishings were comfortable and domestic in character. The environment was enhanced by the addition of many potted plants and flowers. The garden was seen to be wellkept and accessible to service users. Laundry facilities are sited appropriately, away from food preparation and storage areas. A maintenance man employed by the home carries out maintenance as required. No maintenance program or records were available on the day of the inspection. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29, 30 Service users needs are met by the numbers and skill mix of staff who are trained and competent to do their jobs. Staff records need to include all the information in Schedule 4 of the Care Home Regulations to ensure that service users are supported and protected by the homes recruitment policy. The registered persons are advised that they must ensure that all newly appointed staff should not begin work until an enhanced CRB and POVA check and two written references have been received. EVIDENCE: On the day of the inspection two staff and a cook were on duty, which was adequate for the assessed needs of the current service users. A staff training record was seen showing that staff have received various training including an induction, Health and Safety, Elderly Abuse, Diabetes Management, Dementia and First Aid. The two members of staff spoken to said they had received induction training. They also said that they have regular supervision although no supervision records were available on the day of the inspection. Not all of the staff files inspected contained two written references, contracts, photos or terms and conditions. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for standard(s) 33 This standard was not seen to be met at this inspection. EVIDENCE: The inspector was told that the views of service users are sought in regard in regard to the running of the home. There were no records of a quality monitoring system or an annual development plan available on the day of the inspection.
