Conditions of registration: Date of last inspection 19th August 2005 Brief Description of the Service: Abbeyfield St George’s care home provides care for twenty-six older people who do not fall within any other registration category. The home is decorated, furnished and maintained to a high standard. All personal accommodation is offered on a single occupancy basis. Twenty-four bedrooms have en-suite facilities. Each room has access to telephone and television points. The gardens are well maintained and attractive. There is parking available. The home is situated in close proximity to Southend-on-Sea town centre, local community amenities and facilities. There is close access to local buses and the mainline train station to London. This is an overview of what the inspector found during the inspection. This unannounced inspection was carried out on the 23rd February 2006 with the assistance of the Senior Carer on duty, residents and staff. The registered manager was off duty on the day of the inspection. The inspection took place between the hours of 10.30an and 1.30pm. Feedback was provided to the registered manager following the completion of the inspection. A total of eleven standards were inspected with nine being met, the two requirements relates to the maintenance of the premises. Standard 31, although not met, should not automatically lead to an inspection requirement because the outcome for residents are met. Residents and staff spoken with agreed that the home was well run and managed to a high standard within the home. External administration of the service had deteriorated through lack of response to maintenance issues reported by staff. What the service does well: What has improved since the last inspection?
The completion of the new medical room gives staff and visiting health professionals a better area in which to work and store supplies and equipment. Residents now benefit from a new hairdressing salon. Record keeping suggested at the home’s last inspection has been implemented and allows staff more time to concentrate on other duties. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 and 10. The home’s medication procedures ensured the protection and wellbeing of service users. Personal support in this home is offered in such a way as to promote and protect residents’ privacy, dignity, and independence. EVIDENCE: The home’s medication policies and procedures were inspected and found to be well maintained. Staff who administer medication are all senior carers, and have undertaken the appropriate training, both in house and externally. One senior carer was booked onto a new medication course with ‘Boots’. Records examined regarding the receipt, administration and disposal were found to be correct. Medication returns had been signed by the pharmacist. One resident was taking a controlled drug, Morphine Sulphate (MST) with the records and storage of controlled drugs in place. Some residents were responsible for their own medication with risk assessments in place and residents signatures recorded in a book.

A visiting district nurse confirmed that the standard of support and care afforded to residents was ‘the best’. Residents have access to a telephone in the home or if they wish to have their own, telephone points are fitted in their rooms, all bedrooms are single. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13, 14 and 15. Residents are supported by the home in exercising choice and control over their lives through contact with the community and visitors to the home. The meals prepared are good, offering both choice and variety. The special dietary needs of residents are catered for on an individual basis. EVIDENCE: Visitors are made welcome at the home and residents are able to receive visitors in private. There is an open visiting policy in place and the home maintains links with families and friends. Service users are provided with a range of social and leisure activities and there are links with the local community. Residents’ who have the ability, manage some part of their financial affairs, mainly their daily allowances. The home manages the daily allowances of those residents’ who choose not to. Records and receipts were examined and found to be correct. The home’s cook on duty during the inspection provided the evidence in relation to menus, food storage and checks carried out on kitchen equipment. Residents spoken with were positive about the selection and alternatives on offer. The lunch time period was observed to be well managed by care and

Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18. Staff have a good knowledge and understanding of Adult Protection issues that helps protect residents from abuse. EVIDENCE: The home’s Adult Protection policies and procedures, including Whistle Blowing were in place for staff guidance. The home has a copy of the Southend-onSea’s policy on the Protection of Vulnerable Adults and all staff have received the appropriate training in this area. Staff who have been spoken with over the last two visits continue to evidence a good understanding of how to protect and prevent abuse situations. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 22. The standard of the environment within this home is good providing residents with an attractive and homely place to live, however, the health and safety of staff and visitors using the car park could be put at risk through a lack of maintenance. Also, call systems should be extended to areas such as the upstairs lounge and hairdressing salon. EVIDENCE: The location and layout of the home is suitable for its stated purpose and is well maintained. Renewal, decoration and the refurbishment of the home are carried out periodically in line with agreed budgets. The grounds, particularly the care park, was not condusive to the overall standard of the home, because of old beds stored where car parking is set aside. From discussions with the manager and staff, this has not been addressed as quickly as it should have been. Further incidents of work that needed to be carried out regarding fire doors had been reported and not addressed within a reasonable time scale. This fault has now been rectified. The manager confirmed that these issues would be discussed at her next meeting with the Chairman of the Executive Committee.

The home provides experienced and qualified staff to meet the needs of residents. EVIDENCE: Staff spoken with confirmed that all staff receive induction training and also receive a minimum of three paid days training per year. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 and 35. The management of the home is good with records well managed. EVIDENCE: The manager is a qualified nurse (SRN), experienced and competent in working with elderly people. She is a NVQ Assessor and has decided not to continue with the NVQ Level 4 training and qualification, due to nearing retirement. Staff spoken with said the manager was supportive and had the management skills to ensure the smooth running of the service. The manager is accountable to the Executive Committee and there is a yearly budget and systems in place to ensure the homes viability. Residents have their family or sponsor to take account of their finances with the manager making arrangements for personal allowances. Records were seen to be kept up to date.
