Conditions of registration: None Date of last inspection 22/09/05 Brief Description of the Service: Abbottswood Lodge is a period residence that has been adapted and extended to provide residential accommodation for up to 22 adults with learning difficulties. The property is in a very rural location outside the small village of Swanton Abbott, south of North Walsham. The majority of places are in single rooms and nine of the rooms are located in a self contained unit in the extension. This has its own lounge and kitchenette. People accommodated within the main house generally have higher care needs. The home in addition has spacious communal accommodation and an activities room. This is an overview of what the inspector found during the inspection. Care services are judged against outcome groups which assess how well a provider delivers outcomes for people using the service. The key inspection of this service has been carried out by using information from previous inspections, information from the providers ,the residents and their relatives, as well as others who work in or visit the home. This has included a recent unannounced visit to the home and this report gives a brief overview of the service and current judgements for each outcome. What the service does well: What has improved since the last inspection?
The service management have recognised the importance of formal management training and commenced the registered managers award. Window restrictors have been fitted above the ground floor ensuring greater safety for service users. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,4,5 Quality in this outcome area is adequate. This judgement has been made based on evidence from the site visit and other information available to the commission. To improve quality in this area the service should develop a detailed pre-admission assessment tool which clearly demonstrates how the home can or cannot meet an individuals needs. EVIDENCE: The service has had no admissions since the last inspection, however at the time of this visit a prospective service user was being considered for admission. Full social work assessments were in place for this service user and the admission was being managed against a background of graduated visits to the home over a period of time, before the final decision is taken. The staff maintain a daily record in relation to these visits and the management stated that a plan of care will be developed. Information taken from the preinspection questionnaire demonstrated that the service user guide is available in audio form and that it is made available to prospective service users. Sample service user records were looked at and these indicated that service users have been issued with a contract. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9 Quality in this outcome area is poor. This judgement has been made based on evidence from the site visit and other information available to the commission. EVIDENCE: The services care planning system is somewhat basic and plans inspected had not been consistently reviewed. As such there is little individual goal setting and it is difficult to judge if individual service users are supported to take risks as part of an independent lifestyle, or of the degree to which people are able to exercise choice and control. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,16,17 Quality in this outcome area is adequate. This judgement has been made based on evidence from the site visit and other information available to the commission. The outcomes for most service users are adequate, however the service needs to ensure that all service users have a lifestyle which allows them to fulfil their aspirations and potential. EVIDENCE: The service has a mixed resident group with people of differing ages and abilities cared for. Examination of care plans and discussion with staff and management indicate that the lifestyle for older more dependant service users is in line with their needs and wishes, however the home is less effective in helping to stretch the potential of more able residents. From observation and discussion with service users it is clear that people are treated with kindness and care and the atmosphere within the home is relaxed. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20 Quality in this outcome area is good. This judgement has been made based on evidence from the site visit and other information available to the commission. EVIDENCE: The service continues to meet the health care needs of service users in an effective manner. Recent staff training in communication and signing has helped in the understanding of service users needs and is to be commended. Likewise training relating to dementia is welcome as a small number of the older service users are developing this condition. The home could demonstrate how it has responded to the changing needs of service users in an effective manner, for instance by the purchase of a hoist for a service user assessed to need it. The arrangements for the management and storing of medication were inspected and found to be satisfactory. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22,23 Quality in this outcome area is good. This judgement has been made based on evidence from the site visit and other information available to the commission. The service has no history of complaints and staff are aware of the importance of protecting vulnerable people. EVIDENCE: The service has received no complaints and based on the responses from comment cards is a caring and kindly home. Staff continue to have training in adult protection and all of those interviewed during the site visit were aware of whistle blowing and confident they could report concerns if necessary. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24,30 Quality in this outcome area is adequate. This judgement has been made based on evidence from the site visit and other information available to the commission, and would have been good if the potential risk from hot radiators had been recognised and acted upon. EVIDENCE: Since the last inspection a programme of redecoration and refurbishment has been continued with. The downstairs hall ,corridors and the stairs have all been redecorated and new carpet has been fitted. Repairs have been made to the roof and a second bathroom has been refurbished. Extra banisters have been fitted on the stairs to help service users access the first floor. Window restrictors have been fitted on the first floor in line with the requirement made. All areas seen were clean and the home is comfortably furnished. Service users rooms were personalised and contain appropriate possessions. Given that there are service users who are developing dementia then the service needs to risk assess the heating system and guard radiators that pose a risk of burns. JUDGEMENT – we looked at outcomes for the following standard(s): 32,34,35 Quality in this outcome area is adequate. This judgement has been made based on evidence from the site visit and other information available to the commission. For the service to achieve a good outcome greater effort must be made to promote modern best practice. EVIDENCE: The service continues to have a stable and caring staff team. With the exception of training in support of modern best practice for the client group, the service is providing a good basic training package to support staff in their roles. The home is close to meeting government NVQ targets and should exceed them as more staff complete this training. The employment process was tracked and the service continues to operate a safe and robust system. This judgement has been made based on evidence from the site visit and other information available to the commission. EVIDENCE: Since the last inspection of the service the homes managers have started training to achieve the registered managers award. Both managers continue to show a great commitment to the welfare of those cared for and based on the views of staff on the day and the comment cards received are highly valued. The service has a satisfactory approach to health and safety and the visit embraced inspection of records in support of this. A quality survey has been undertaken as required at the last inspection, however the process was not extensive and a more rounded survey would have embraced a wider selection of stakeholders. Where service users finances are being managed by the home good records are kept and these were inspected and found to be in order.
