This inspection was carried out on 24th October 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Good. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home is very small only having two Service Users and one vacancy, which has resulted in a very homely atmosphere with individual support for each of the Residents. The two Service Users have lived at the home since it opened and most of the staff have also worked at the home for a long time. The home is in the process of having the office completely redecorated, along with the Utility room. Other areas of the home were all nicely decorated and the whole home was comfortably furnished. One of the Service Users is 99 years old and plans are already being made for their 100th birthday. The Residents said they were very well cared for and the Manager and the member of staff were observed being kind and supportive. The home have organised activities that are varied depending on the weather and what the Service Users want to do. Daily newspapers are read to the Residents and one of the Service Users commented that they had enjoyed the recent coverage of a national event.

What has improved since the last inspection?

The previous inspection had one Requirement around the issue of the decoration of the Utility room and the development of a programme of decoration. During the inspection decorators were working in the home and the Utility room had been done and the office was being done. The Manager said that they have regular assessment of maintenance and then arrange for it to be done. The home continues to offer the Service Users a very high level of care, with a very dedicated staff group.

What the care home could do better:

There were no Requirements or Recommendations from this inspection, the manager is very keen to continue to improve and maintain a high standard of care.

The home is a corner house, in a residential area of Ilford, close to a busy main road and within easy reach of local facilities and transport links. All the bedrooms are located on the first floor and a stair lift is available for easy access. There is a separate lounge and dining area downstairs, with a small well-maintained garden to the back of the house, which is accessed via the dining room as well as the kitchen. There is a communal bathroom and toilet upstairs as well as a shower facility and toilet downstairs. The inspection took place over two hours and was carried out as part of the homes yearly inspection programme. The previous inspection was an Announced inspection and was carried out in April of this year. That made 1 requirement, which has now been met. A tour of the home was undertaken and a number of documents looked at with the Residents participating fully in the process. The Manager was present during the inspection and their participation was very much appreciated. A member of staff came in towards the end of the inspection, to cook the lunch. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 and 3 Each Service Users has a written contract, statement of terms and conditions and each prospective Resident receives a comprehensive assessment. EVIDENCE: During the inspection the homes Service User Guide and a Statement of Terms and Conditions was looked at. The Terms and Conditions had a space for the Service Users name to be added and it documented that Residents will be initially placed for four weeks on a trail basis. The Conditions covers the areas of care, service including the fees, who is paying the fee, the rights and obligations of the Service User and conditions of occupancy. As an example the Conditions say, “Fees include all care, accommodation, costs, food and drink, heating and lighting and laundry done on the premises. It then goes on to say that “Fees do not cover newspapers, toiletries, hairdressing and other things like chiropodist.” The assessment of any prospective Service User is carried out by the Manager and involved the Service User, family/advocate and information from the placing authority. An assessment of a possible Resident for the vacant room Acorn Care Home DS0000025881.V260848.R01.S.doc Version 5.0 Page 9 was looked at and covered areas of social care, general health, medication, continence and mental state. The assessment commenced with personal details and the general health area covered past history, as an example this Service User had a history of thyroid problems and had had a number of falls at home resulting in a broken hip. The assessment looked at mobility needs as a result of the falls at home and documented that they walked with a Zimmer frame and they needed assistance to rise from the sitting position. This assessment allowed the home and the Service User and family to be confident that the home could meet this person’s needs. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8 and 10 Service Users each have an individual care plan and their health care needs are fully met. Service Users’ privacy and dignity are respected at all times by a dedicated workforce. EVIDENCE: The home policies and procedures for the Service Users care plans and one of the Service User’s care plans was looked at. The plan started with a description of the Resident and background information with their initial assessment. There was a medical section with their medical card, GP details and previous medical history. The section headed routine was documented; the Service Users just likes to sit and watch television and enjoys deep breathing and simple exercises. Other areas covered were next of kin, funeral arrangements, communicating, eating and drinking, mobility and personal care. Further areas covered were continence, sleeping and social history. The area around personal care, documented that the Service User was very fastidious in their personal care needs and needed to be allowed time to be independent. The Service Users file also covered areas of health care and documented visits from Dentist and the arrangements for the hygienist to come to the home. The Service Users, staff and the Manager have all known each other and worked together for many years, resulting in a much more intimate and personalized interaction. Service Users were able to access all areas of the home and were observed to be enjoying the interaction with the staff. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 and 15 The home encourages and facilitates the Service Users contact with family and friends. The Service Users receive a balanced and healthy diet that they enjoy which is served in convenient and pleasant surroundings. EVIDENCE: The Service Users’ care plans documented the contact with family and friends. The Service Users have lived in the same home with the same staff since the home opened. It was documented on the care plan that a family member visited each week and the Manager said that the family of the Service User who is 99 are arranging for them to receive a telegram from the Queen on their 100th birthday. The Manager arranged for a member of staff to come in during the inspection, so that the Service Users would not have to wait for their lunch. The meal was seen as balanced and healthy, one of the Service Users had been involved in making a trifle. The member of staff not only was cooking the lunch but was also interacting with the Service Users and made them a cake for their tea. As the home’s office was being decorated, the dining room was used for looking at documentation and one Service User participated. Once the inspection was over, the lunch was then served to the Residents in the dining room. The dining room was next to the kitchen and there was a serving hatch, which allowed the Service Users to see into the kitchen. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 Service Users’ complaints are listened to and taken seriously and they are protected from abuse by the training of staff and the homes policies and procedures. EVIDENCE: The manager said that the home has virtually no complaints, but have a policy and procedures and the policy is made available for the Service Users and visitors. There is a complaints book, however there were no complaints documented in the book. As the home is so small and the Service Users have such a good relationship with the Manager and the staff, any concerns are dealt with immediately. All staff have completed their NVQ 2 and have had training on Adult Abuse, the Service Users are very well protected from any possible abuse. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20, 24 and 26 The home provides safe and comfortable indoor and outdoor facilities; each Service User has their own bedroom with their own possessions. The home is clean, pleasant and hygienic. EVIDENCE: During the inspection a tour of the home was undertaken and with the permission of the two Service User, each of their rooms were looked at. Each of the bedrooms was reached via a stair lift and was nicely decorated and personalised. The home had a comfortable lounge with easy access to the dining room, kitchen and toilet facilities. The home’s office was being decorated and the utility room had recently been decorated. The home is now monitoring areas in the home that need the routine maintenance and renewal of fabric. The home is kept very clean and hygienic and there is a very homely atmosphere, were the Service Users feel comfortable. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 and 30 The home has good ratios of staff to Service Users and all staff have received training and are competent to do their jobs. EVIDENCE: The home has a stable team of staff, with only a small turnover. The staff files for two members of staff; one being the newest member of staff, were looked at during the inspection. The files showed that the home has undertaken all the necessary recruitment checks to ensure the protection of the Residents. CRB checks have been undertaken for all members of staff and references are available on all the staff files. Staff receive supervision and there is an in-depth induction booklet completed by the member of staff and the Manager. Staff files had certificates for Occupational health and safety, adult abuse and protection, oral care, death and dying and NVQ 2. The Manager is part of the staff team but does not work weekends unless it is needed and all the other four staff members work part time. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35, 36 and 37 The home’s record keeping, policies and procedures safeguard Service Users, rights including financial interests. All staff receive regular supervision and support from the Manager. EVIDENCE: During the inspection the homes policies and procedures were looked at and seen to safeguard the Service Users. There are policies and procedures on complaints and Adult Abuse as well as medication and the Service Users financial interests. The Service Users financial record book were looked at, where their incoming and outgoing money is recorded with receipts. Records are kept in the office normally, but were temporally stored in the dining room cupboard whilst the office was being decorated. The Manager supervises staff and is present in the home most days of the week and gives the staff support and documented supervision.
