It consists of three floors that are connected by two passenger lifts. The ground and second floor provides accommodation to service users who require general nursing care and the first floor is dedicated to service users who are mentally frail. Each floor has its own dining room and two lounge areas. The home is built on a sloping site and the main reception, kitchen, laundry, hairdressing salon and garden lounge are located on the lower ground floor (at the rear of the bulding). There is off-road car parking provided both at the front and rear of the home. The home is located in close proximity to shops and other amenities including public transport. This is an overview of what the inspector found during the inspection. The unannounced inspection visits took place on 1st November and 29th December 2005. The home manager was present on both occasions. The Operations Director was present during the 2nd visit. The inspection began with general discussion with management followed by a tour of the units. Documents and other relevant materials were inspected and copies obtained. A number of service users, visitors and staff were interviewed. The home has been under new management since May 2005. Staff are adjusting to the change in management. New initiatives are being implemented. Positive feedback was received from the majority of the relatives and service users about the care given. However, a relative felt that her mother was not receiving ‘appropriate nursing care’. The matter is currently being investigated. (Please see below for details of the inspection findings). The staffing level in the Dementia Unit has been increased by one. The home has a ‘Sensory Room’ that is now open to service users. A trained member of staff always accompanies a service user who wishes to use the facility. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3, 5. Prospective service users and their relatives are given an opportunity to visit and to assess the facilities and suitability of the home. They have the information to make an informed choice. A full assessment is carried out before a prospective service user is admitted to the home. EVIDENCE: The relatives spoken to confirmed that they were given the opportunity to assess the home and a Service User Guide was given to the new admission. The home manager assured the inspector that a thorough pre-admission assessment is carried out and that the home will only admit a service user whose needs can be met. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9. Each service user has a written care plan. Some of the care plans are inadequate and are currently being revised. There were errors made in the administration of medication. EVIDENCE: The home manager carried out an internal audit of the written care plans and found that some of them were lacking in content. The key workers, who are trained nurses, are responsible for the written care plans of the service users in their care. It is recommended that staff attend training with regards to writing care plans. The home manager recently investigated incidents a number of medication errors made by 4 nursing staff who failed to signed the Medication Administration Record (MAR) Charts. Disciplinary procedures were initiated. None of the medication errors resulted in any adverse effects on the service users. On the day of the inspection, the MAR charts were correctly completed. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13, 14, 15. The service users are given every opportunity to participate in all aspects of routine living. They are encouraged to maintain links with their family and friends. The meals provided are nutritious and choices are given. EVIDENCE: The menu was reviewed by a dietician in February 2005 and appropriate changes were made to ensure the meals provided are balanced and nutritious and choices are available. The service users interviewed were ‘quite pleased’ with the meals served. A number of service users said that they ‘would like to see more fresh fruit on the table’ so that they can help themselves as they wish. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 17, 18. The home has a Complaints Policy and Procedure. However not all complaints have been responded to within 28 days. EVIDENCE: Feedback received from a relative suggested that the home has not responded to a complaint within 28 days. The home manager was away at the time but she has assured the inspector that she will review the matter and improve the way complaints are handled in future. The home follows the Adult Protection Procedure issued by Hertfordshire Social Services. The manager ensures that all staff are aware of the Adult Protection Procedure and the Whistle Blowing Policy. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 21, 22, 23, 24, 26. Service users live in a clean and pleasant environment. The facilities provided are adequate and well maintained. The home has a ‘Sensory Room’ that is now open to service users. A trained member of staff always accompanies a service user who wishes to use the facility. EVIDENCE: The premises appeared homely and clean. On the day of the inspection, the workman was busy redecorating the bathroom on one of the floors. There is a rolling maintenance programme. The home manager confirmed that service users who require nursing care are nursed on adjustable beds (profiling beds) with appropriate mattresses in place. The bedrooms inspected were clean and comfortable. The items on display in the bedrooms reflected the lifestyles of the occupants. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 30. Staff are deployed where needed to ensure the needs of the service users are being met. Appropriate training is arranged for the staff. EVIDENCE: The staff number in the Dementia Unit has been increased from 5 to 6. There is no increase in the number in the Residential and Nursing Units. The home manager confirmed that staff are deployed where needed. She and the deputy manager are both hands-on. The home manager ensures that staff receive adequate and appropriate training in order to meet the needs of the service users in their care. Arrangements have been made for 2 members of staff to attend training sessions on Wound Care. (Please see Statutory Recommendations). Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 36, 37, 38. The home is under new management. Procedures are being reorganised to improve services. The health, safety, and welfare of the service users are safeguarded by the record keeping, policies and procedures of the home. EVIDENCE: The home manager (former deputy manager) has been in post since May 2005 and the new deputy manager has been in post since July 2005. Staff are adjusting to the change in management. Team working is encouraged. The home manager is currently coaching her staff to revise, update and improve the contents of the written care plans. All relevant policies and procedures have been revised and updated and these are available to staff for their guidance. Regular audits of nursing tasks such as wound care, risk
