This inspection was carried out on 12th September 2005.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Residents are provided with very good personal care and support by an experienced and well-trained staff team. The home has a relaxed, informal, and pleasant atmosphere. The environment is modern and well maintained with pleasant landscaped garden.

What has improved since the last inspection?

Further training has been provided for staff, who are now all progressing towards their level 3 NVQ in care. Further improvements have been made to the environment and a new care plan has been introduced to provide a brief summary of assessed needs.

What the care home could do better:

The home must introduce a standard approach for their assessment, care planning, and risk assessment procedures. The current system does not ensure that all residents have an easy to follow plan as to how their needs can be met and risk assessments are not being completed. Further improvements are also required to the environment.

The home is in the Hall Green area of West Bromwich within walking distance of local shops and bus routes. It was purpose built and is situated at the end of a private drive with extensive grounds and ample parking facilities. Accommodation is provided over two floors which are divided into four separate units each with it’s own lounge, dining room, and kitchenette. In addition to the main staircase and fire exits a passenger lift provides access to the first floor. The majority of the rooms (36) have en-suit bathroom facilities and there are ample showers, baths, and toilets provided with the necessary adaptations. There is a day centre service provided for 12 people with their own large lounge, toilets, shower, kitchenette and conservatory. Service users living at the home are welcome to attend and join in the activities. The inspection took place over 7 hours and included discussions with 6 residents in private and in each of the 4 lounge areas. All staff on duty and 3 visitors were spoken with informally, and an interview took place with the manager. The care plan files for 4 residents and the medication records were also examined. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4,5 Residents are very pleased with the home and the standard of care they receive. However the admission procedures vary greatly and are not always very good. Not enough information is available to ensure their personal care and health needs can be met or sufficient plans in place to ensure their safety is protected. EVIDENCE: Intermediate care (standard 6) is not provided. The home provides a copy of their service users guide in every bedroom. The statement of purpose and the other information required in the guide including a copy of the contract and this inspection report are normally kept in reception and are available on request. Discussions took place with 3 residents who had recently moved into Allerton Court and 2 of their care records were examined to ensure the correct procedures had been followed. Allerton Court E55 S4793 Unannounced Allterton Ct V248372 120905 Stage 4.doc Version 1.40 Page 8 Those spoken with reported that they had visited the home on sometimes more than one occasion and had a good look around before deciding to move in on a trial basis. They said ‘the girls are very helpful’, and ‘I feel at home here’. One stated the reason for choosing the home were the good views, lovely meals, and the range of activities on offer. The care records for one resident contained very little information. There was a copy of an assessment carried out by social services 2 months earlier but was not relevant to their current needs and did not contain all the required information. No copies of the homes assessment or care plan were available and no risk assessments were carried out despite them having falls prior to and during their stay. The second file did contain an assessment carried out by the home however no care plan had yet been implemented. Residents funded through Social Services have a funding contract completed. Everyone should also have a statement of terms and conditions. This must state the room number, which they will be living in. They must also receive written confirmation following their assessment that the home can meet their needs. Although this varied these were not found on all the files examined. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10 Residents are well looked after and receive a high level of personal care. It was also evident from talking with them that their needs were being met. Improvements are needed however to their written plans to ensure risks to their health and safety such as falling and developing pressure sores are reduced. EVIDENCE: The care plans for 4 residents were examined. The standard varied from one service user to another and included 2 that had not been fully implemented. The homes care plan provides a summary of why the resident is in need of care. For example ‘to assist with washing and dressing’, and ‘to provide support in a residential setting’. A document called a ‘Personal Data Sheet’ is used both as an assessment and care plan. A brief care plan for each resident has been introduced to provide staff with a guide to meeting their needs. These were found to be easy to follow but only gave general guidelines such as ‘needs assistance with washing and bathing’. There was also evidence that not all staff knew they were available. Reviews were also not been carried out every month, or as required. However they were sometimes not dated and were completed using incorrect or conflicting information. Health needs were being monitored using a tracker sheet however these were not always updated. The medication storage facilities and records were examined. The home has a good policy for the administration of medication and each resident has a photograph, description of side effects and reason for having the treatment. Accurate records are maintained for the receipt and return of all drugs to the pharmacist and the administration sheet was complete with no gaps found. All staff were seen to treat residents with dignity and respect. Personal care and medicals treatments are carried out in private and permission is sought before rooms are entered. Comments received during the day included ‘I am very lucky to be here’, ‘I wouldn’t want to move’, and ‘You can do what you like and come and go as you like’. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,&15 Residents have sufficient opportunities to engage in planned activities with staff available to provide individual support if required. All feedback received praised the standard and variety of food and drink provided. EVIDENCE: Residents described being involved in a wide range of activities, which take place in the day centre (situated in the home but not inspected by the commission). Additionally staff often engage with 1:1 activities including shopping trips. The home has it’s own mini-bus which enables other outside activities to take place including trips to the Walsall lights. The local vicar also visits and residents are able to attend services as required. Visitors can be seen at any reasonable time (or at other times with prior agreement) and can be seen in the privacy of their own room if required. Residents have been able to keep their own pets such as a budgie and the home has it’s own cat. There are extensive grounds with landscaped gardens, excellent views over the surrounding area and plenty of seating. Residents were seen to be sitting outside and making the most of the sunshine. Residents meetings are held every 3 months to discuss any changes and receive feedback. They can handle their own finances and furnish their rooms to individual tastes. Details on how to contact an advocate are displayed on the notice board. The planning of meals and the options available are agreed at residents meetings. There is a cooked breakfast and choice of 2 main cooked meals at lunchtime and a vegetarian option. Fresh fruit is also available in all the lounge areas and drinks are served regularly and as requested. The kitchenettes on each unit enable snacks and drinks to be available at all times. Individual dietary requirements are also provided.
